We rejoin the action as the team work on the young man stabbed in a fight, and Bruce rejoins the team for his last shift, flying with the helicopter.

Amber: Whats this guy been stabbed with? Police officer: Flick knife, its gone. O’Hanal: Gone straight into the right lung, lost approx. a unit out of the tube. Amber : Ok lets start infusing a unit of O-Neg, Amp of epi...

Patient : Why the F*** you helping him he started it, ARGH!! Police officer : Oi leave the medic alone or the’ll be trouble! Patient : F*** you, jumped up ...(swings out)Police officer: Medic MOVE! TASER TASER TASER!

Peter : What the!, Bruce go see if that idiots stable!Bruce : 10 : 4.

Peter (on Phone) : Hello can i speak to the charge nurse... Hi Jo, Its Peter from the Air Ambulance Bravo team, We’ve got a patient approx. 21 stabbed through right lung, hypotensive, Hypovalemic, were going to bring him to you as a Trauma. Also got a 22 year old with big facial lacs, just been tasered by PD, also coming your way.

“Hi Folks this is Gaz 21, been stabbed through the right chest and been hit in the face, no other apparent injuries, had a right chest tube inserted, Intubebated after RSI in field, BPs been falling enroute, last at 90 over 65, Pulse 125, been throwing us PVCs, Had 90meg of Lidocaine, 600megs of Propofol, and 12 of Sux.”

1 amp of Adrenaline down the ET, 2 Units of O- in the field, 4l normal saline.Ane: Ok swap it out so we can work on this lung, Endoscope and 8.0, get me a scope with a mac first.Nurse: Foleys in! ED doctor: Ok lets order a chest film, orbits, head CT. More O- and hang some dopamine.

Bruce: Think he’ll make it?Peter: Probably not, be a miracle if he did.

Bruce joins new members of HEMS at a flight safety lecture ahead of his final shift, this time aboard the helicopter.

The next day.

“Ok folks now you’ve heard the blurb we’ll head out on the helipad, the helicopter is based her at the moment while QE move to their new emergency department.”

“Here it is, MD Explorer, 902, Notice our versions modified 4 rotors, and dual mechanically linked controls, word of warning, just because theres no tail rotor, does not mean it isn’t still a hazard, there is a huge amount of hot air moving round that tail at times, take care during hot loads and unloads”

“Also notice the road, setup with low traffic lights and a crossing, when coming in the road is stopped to allow a trauma team to move from and to majors with the patient”

“Wow! Folks watch out, move back thats the siren!”

Amber: Hi again Bruce, Meet Todays pilot John, and our flight Paramedic Dave, Hope you enjoy your last day, take some pictures up there if you can.

Siren Sounds! John: We got one, hit and run, requested by rapid response car, 30 year old male multi Trauma.

Amber : Where exactly are we going?John: Tattenhall, Right on the edge of our patch.Dave : Looks like a difficult landing here, we might need to run.

“Medivac 1 to HEMS Ops, requesting information about landing at incident”

“HEMS OPs to Alpha on Medivac 1, Police are in attendance and have cleared a road for you to land, they advise us there are no obstructions to your blades over”

John: Ok I see it, Police and RRV! setting you down as close as possible. Clear Dave?Dave: Clear on the starboard, 4, 3, 2, 1, down.

Amber: Hi there how are we doing?Patient: Hurts like hell everywhere...Amber: I know, we are give you some meds and send you off to sleep for a little while. Dave, Sux and Propofol.

Police officer: (on radio) Yeah, suspect vehicle is a blue focus, and must have a huge bullseye on the screen...“Hello there, its Dr Brown HEMS Registar, we’ve got a 31 year old male we want to bring to you by air, hes got query head trauma, unstable pelvis, splinted right open Tib/Fib fracture, RSIed, Intubeated and given manitol”

Dave: Ok folks grab on, my count, 1 2 3! Amber: This is Ben, 31, hit by car at around 30 Mph, He’s got quite a nasty hemotoma on the back of his head and had a GCS of 12 and was altered and combative, got a very unstable pelvis, open fracture to the right tib/fib, no other apparent injuries, Pulse 100, BP 90 over 65 had an RSI, 300 of Manitol, 2 litres of normal saline...

Frankie: Of folks, lets get a trauma series, FBCs, LFTs, BA, Tox, Type and match for 6, we want a head CT pronto, lets keep him sedated while we assess the head injury, Somebody page neuro again! get them down here.

Radiographer: OK Shooting!ED Doctor: Somebody get me a blood gas kit!

Amber: Where the hells Dave!John: Ambulance bay... You know what he’s like.

SierenAmber: YO! I told you, don’t bugger off like that! What we got!John: Teenager, fall from height, still up the side of a building site.

Firefighter: Hey Major! Bruce: Major? Dave: Didn’t you know, She’s in the Army, medical officer Bruce: That explains a lot.Amber: How are we for getting up to him safety wise?

Lt Tracey: OK guys, hes gotta come down before we can work on him!Amber: Ok, Pass up a longboard! How the hell did he get here?Firefighter: Climbed up on the roof apparently, tripped and fell down to here, about 20 foot. Amber: Add Acute lack of intelligence to chief complaints. Dave! we gotta get him down and to a theatre quick, right pupils blown and i’m worried he’s gonna fit on the way down. vitals are all over the shop as well. Neuro are going to have a very busy day.Dave: Ok i’ll make the call, trauma team and neuro.

Dave: This guys a total mess. How are we going to get him down to the street? Can you guys get a platform up here?Lt Tracey: No go, going to have to go down on lines, Johno! Get rigged up and you can go with him.

Johnson: Yo! watch him, hes spinning out! Doc! got a problem here!

Amber: Push some Diazepam now! We can’t have him seizing in the air!

Amber: Ok this is unknown male approx 14, probable basal skull fracture, few broken rigs, blown right pupil, erratic rhythms, pulse now 111, BPs 60 palp, Seized twice, started to see fluid in the canals in the air, looks like we’ve got a donor i’m afraid. Thats us for today, i’ll call in later find out what the situation is.

John: Hope you enjoyed you shifts with us, bit of a sad end but we thought you might like to keep this.
