• Reboiler out and product out interchanged• Themowell clashing with down comer• Thermowell not in liquid phase.• Level instrument wrong placement.• Feed to reboiler pan wrongly oriented.• Demister below safety valve.• Liquid draw off not getting liquid.• Not able to enter thru manhole.
