• Thirty participant interdisciplinary consensus conference
• Goals:1. Determine minimum data elements to be included
in a medication list;
2. Identify and discuss barriers and resolutions to the use of medication lists; and
3. Develop a framework for further research and a national social marketing campaign.

• ASHP Foundation sponsorship of a University of Pittsburgh study entitled “Development of a Continuity of Care Record: Bridging the Medication Use Gap from Hospital to Home”
• Collaboration with the National Transitions of Care Coalition• Inclusion in the American Pharmacists Association Medication
