STATEMENT OF WORK
Tulsa County-Fiber-TC-218 W. 6th Primary Fiber
_LynxSystems, LLC (LS) and/or its subcontractor:
• 250 SF Concrete RR/Streets: Remove and replace 1 full panel of concrete
per COT ROW requirements
• 75 SF Concrete RR/Sidewalk: Remove and replace sidewalk as required
per COT ROW requirements
• 30' Trench and Place: Excavate, provide and place 211 PVC, remove all
spoils off site, provide and place quickset FF in street excavation and class A
ag-base compacted in lifts in sidewalk per COT requirements
• 1 Enter existing Vault: Core drill, install conduit, reseal vault with water
plug around duct
• 1 Building Entry: Core drill, install conduit through wall provide and place
link seal, grout inside and out with water plug
• 1,125' FOC Install: Pull in customer provided 24 ct. FOC between library
basement and 218 W. 6th (from edge of building to library vault is approx.
625' + 100' slack (2 street vaults)+ 100' (library HH under pavers into
basement)+ 300' (estimated at building to get into and up to comm room by
others)
• All permit applications will be submitted and all City of Tulsa requirements
will be followed.

• Price does not include any City of Tulsa permitting fees, concrete
degradation fees, traffic disruption fees or any other fees. The City of Tulsa
has agreed to waive fees.
LynxSystems, LLC (LS) will:
• LynxSystems will provide the following for installation by Underground
Enterprises: 1,125' of 24-strand- singlemode-aerial/duct rated loose tube
fiber optic cable and 1,125" of #12 tracer wire. This fiber will be installed
between the Civic Center parking garage and the 218 W. 6th
building
basement.
• LynxSystems Will provide the following at the Civic Center Parking Lot &
Courthouse: (l} 24-Strand singlemode-lnterlocking armored
Indoor/Outdoor-Plenum Rated Fiber optic cable from the transition point at
conduit entrance from Denver Ave. into the parking garage, through the
existing Tulsa county raceway into the courthouse and up to the first floor
MDF. It is assumed that parking spots can be blocked and this can be
completed during normal business hours.
• The 24-strand armored fiber from the courthouse and the 24-strand fiber
from 218 W. 6th will be fusion spliced in a lockable wall mount splice
enclosure with (2) 12-strand splice trays.

• LynxSystems will route the 24-strand armored fiber optic cable to the
designated rack. A rack mount-2RU Fiber Enclosure with (2) 12-Fiber-6-
Duplex-LC fiber adapter panel will be installed. The 24-stand singlemode
fiber will be terminated on LC Unicam connectors. Blank panels will be
inserted in unused panel ports. The armored cable will be grounded at the
rack end to an existing ground bar.
• LynxSystems Will provide the following at the 218 W. 6t h Building: (1) 24-
Strand singlemode-lnterlocking armored-Indoor -Plenum Rated Fiber optic
cable from the transition point at conduit entrance from Denver Ave. into
the basement, through the basement to the existing riser pathway and up to
the 4th floor data center.
• The 24-strand armored fiber from the 4th floor and the 24-strand fiber from
the Civic Center parking garage will be fusion spliced in a lockable wall
mount splice enclosure with (2) 12-strand splice trays in the basement.
• LynxSystems will route the 24-strand armored fiber optic cable to the
designated rack. A rack mount-2RU Fiber Enclosure with (2) 12-Fiber-6-
Duplex-LC fiber adapter panel will be installed. The 24-stand singlemode
fiber will be terminated on LC Unicam connectors. Blank panel will be

Testing
• The 24 Strand fiber optic cables will be tested rack-to-rack with a Fluke DSX-
5000 or DSX-8000 fiber certification tester. This is not an OTDR test.
Exclusions
• All pricing is good for 30 days only.
• LynxSystems, LLC and/or UE will not be responsible for private utilities not covered under "call OKIE" locating system and that are not properly located to within 12" either side of utility.
• LynxSystems, LLC will not be held responsible for any outages due to the installation included in this quote.
• Core drilling of floors is excluded from this quote.
• Ground Bars at Racks are excluded from this quote.
• Moving existing patch panels and switches in the racks is excluded from this quote.
• All work will be completed during normal business operations hours: Monday-Friday, 7:00AM-5:00PM. Any requirement to work other hours will result in an addit ional labor charge.
• AutoCAD drawings are not included in this quote.
• Costs for engineering are excluded from this quote.
• OTDR testing is not included in this quote.
• Repair, replacement or troubleshooting of existing fiber optic cable, fiber optic connectors and fiber optic fusion splices completed by others is excluded from this quote.

Summary
• This quote is based on the information provided Tulsa County Personnel.
All work in the downtown conduit system must be approved by the City
of Tulsa. This is a good faith estimate of the costs involved to complete
the installation with the information that was provided.
TERMS & CONDITIONS
• LynxSystems, LLC has provided you with an investment summary and fee estimate based upon our knowledge of the current requirements and project
goals.
• A Change Order will be issued by LynxSystems, LLC for work that falls outside the agreed-upon scope of this engagement, and your approval/signature will be required to initiate the work.
• The fees stated in this engagement will be honored for 30 days from the date of this proposal.
• The fees do not include "Additional Items" cost, their applicable sales tax and/or shipping charges.
• Fees and expenses will be billed as incurred.
• Payment of our progress billing is expected within 30 days after invoice date.

Tulsa County Insurance Requirements:
• 1. Insurance: LynxSystems, LLC and any subcontractors will maintain valid
liability insurance for the duration of this project. Prior to any entry required
to perform the work outlined herein, LynxSystems, LLC (or its authorized
agents and/or contractors), shall obtain and deliver to Tulsa County an
insurance certificate (in form and substance reasonably acceptable to Tulsa
County), evidencing that LynxSystems, LLC (and its authorized agents and/or
contractors), shall have in full force and effect a policy of commercial general
liability insurance with limits of not less than $1,000,000 combined single
limit, covering liabilities for personal injury, death, and property damage
arising out of activities contemplated in the Statement of Work by
LynxSystems, LLC (and it's authorized agents and/or contractors), which
policy shall: (i) name Tulsa County as additional insured; (ii) be underwritten
by a reputable insurance company licensed to do business in the State of
Oklahoma; (iii) include a contractual liability endorsement with respect to
LynxSystems, LLC indemnification obligations hereunder; and (iv) if readily
available to the insured and without cost, not be cancelable or subject to
amendment without at least thirty (30) days' advance written notice to Tulsa
County.
• 2. Indemnification: LynxSystems, LLC shall indemnify, defend and hold
harmless Tulsa County from any liability in connection with any of the
activities contemplated in the Statement of Work, including any work
performed by its authorized agents and/or subcontractors.
