PROJECT DESCRIPTION:
THE PROJECT CONSISTS OF RENOVATION OF A PORTION OF THE MIAMI UNIVERSITY CAMPUS WHICH IS
BOUNDED BY WITHROW STREET TO THE NORTH, THE WITHROW STREET PARKING STRUCTURE TO THE SOUTH,
THE PARKING STRUCTURE ENTRY DRIVE TO THE WEST, AND THE PARKING STRUCTURE EXIT DRIVE TO THE
EAST.
THE PROJECT INCLUDES A COMPLETE RENOVATION OF THE WITHROW STREET PARKING LOT AND A PARTIAL
RENOVATION OF THE MIAMI INN PARKING LOT.
THE IMPROVEMENTS WILL BE CONSTRUCTED WITHIN PROPERTY OWNED BY MIAMI UNIVERSITY.
TOTAL SITE AREA = 1.27 ACRE (55,485 SQUARE FEET)
TOTAL DISTURBED AREA = 1.22 ACRE (53,232 SQUARE FEET)

RUNOFF FROM THE PROJECT SITE WILL DISCHARGE TO THE MIAMI UNIVERSITY STORM SEWER SYSTEM.
SEQUENCE OF CONSTRUCTION:
1. OBTAIN ALL REQUIRED PERMITS.
2. INSTALL SILT FENCE, AND OTHER BMP MEASURES ACCORDING TO PLANS AND AS DETERMINED BY A/E
AND/OR MIAMI UNIVERSITY.
3. INSTALL CONSTRUCTION ACCESS DRIVES AT ALL ENTRANCE/EXIT LOCATIONS FROM EXISTING ROADS.
4. REMOVE LAWN AND TOPSOIL ACCORDING TO PLANS.
5. DEMOLISH/REMOVE EXISTING STRUCTURES AND UTILITIES ACCORDING TO PLANS.
6. PERFORM MASS EXCAVATION OPERATIONS, GRADE THE SITE AND INSTALL UTILITIES.
7. INSTALL INLET PROTECTION AT ALL NEW STORM SEWER STRUCTURES AS THEY ARE CONSTRUCTED.
8. CONSTRUCT PARKING LOTS.
9. PERMANENTLY STABILIZE AREAS TO BE VEGETATED IMMEDIATELY AS THEY ARE BROUGHT TO FINAL
GRADE.
10. ONCE THE SITE HAS BEEN FULLY STABILIZED, REMOVE SILT FENCE, INLET PROTECTION DEVICES, AND
OTHER TEMPORARY BMP MEASURES.
STORM WATER POLLUTION PREVENTION PLAN NOTES:
1. FOR PROTECTION OF UNDERGROUND UTILITIES, CALL THE OHIO UTILITIES PROTECTION SERVICE
AT 1-800-362-2764 A MINIMUM OF 48 HOURS PRIOR TO EXCAVATING.
2. ALL MATERIAL AND CONSTRUCTION WILL BE IN ACCORDANCE WITH THE CONSTRUCTION
STANDARDS AND SPECIFICATIONS FOR THE PROJECT. ALL WORK WILL BE IN COMPLIANCE WITH
ALL APPLICABLE FEDERAL AND STATE STANDARDS AND REGULATIONS. IF THERE SHOULD ARISE
ANY CONFLICT BETWEEN ANY NOTES AND/OR DETAILS ON THE APPROVED DRAWINGS OR
SPECIFICATIONS FOR THE PROJECT, THIS NOTE WILL SUPERCEDE AND GOVERN UNLESS THERE IS
A WRITTEN SIGNED DOCUMENTATION STATING OTHERWISE.
3. ALL CONSTRUCTION AND DEMOLITION DEBRIS (C&DD) WASTE SHALL BE DISPOSED OF IN AN OHIO
EPA APPROVED C&DD LANDFILL AS REQUIRED BY OHIO REVISED CODE (ORC) 3714.
4. NO KNOWN TOXIC OR HAZARDOUS MATERIALS EXIST ON SITE. IF SUSPICIOUS OR HAZARDOUS
MATERIAL IS DISCOVERED DURING CONSTRUCTION OPERATIONS, THE CONTRACTOR SHALL
IMMEDIATELY NOTIFY A/E AND OWNER. ALL TOXIC AND HAZARDOUS MATERIALS SHALL BE
LEGALLY DISPOSED OF PER LOCAL LAWS AND REGULATIONS.
5. IF SOIL CONTAMINATION OCCURS DUE TO PETROLEUM OR OTHER CHEMICAL SPILLS, ALL
CONTAMINATED SOILS SHALL BE TREATED AND/OR DISPOSED OF IN AN OHIO EPA APPROVED
SOLID WASTE MANAGEMENT FACILITY OR HAZARDOUS WASTE TREATMENT, STORAGE OR
DISPOSAL FACILITY (TSDF).
SOIL EROSION AND SEDIMENTATION CONTROL NOTES:
1. UNDER THE RULES OF THE FEDERAL WATER POLLUTION CONTROL ACT (40 CFR 122), THE OHIO WATER
POLLUTION CONTROL ACT (CHAPTER 6111.04), AND OHIO EPA PERMIT NO. OHC000004, THE CONTRACTOR
SHALL ACT AS THE “PERMITTEE” AND WILL BE FULLY RESPONSIBLE FOR ADMINISTERING THE EROSION
CONTROL PROGRAM THROUGHOUT THE DURATION OF THE CONTRACT. UNDER THIS ACT, THE
CONTRACTOR WILL BE RESPONSIBLE FOR SUBMITTING THE NOTICE OF INTENT FORM ((NOI)(NPDES
GENERAL PERMIT)), PAY ALL ASSOCIATED FEES, AND ABIDE BY ALL REQUIREMENTS IDENTIFIED UNDER THIS
ACT AND AS NOTED ON THE PLANS AND SPECIFICATIONS. THE NOI SHALL NOT BE TRANSFERRED TO A
SECOND PARTY WITHOUT PRIOR WRITTEN AUTHORIZATION FROM THE OWNER. THE NOI SHALL BE SENT TO
THE FOLLOWING LOCATION:

THE CONTRACTOR SHALL BE RESPONSIBLE FOR SUBMITTING THE NOI FORM AT LEAST 7 DAYS PRIOR TO

2. ALL PENALTIES AND FINES ASSOCIATED WITH VIOLATIONS ASSOCIATED WITH NOT FILING THE NOI OR
DISCHARGING POLLUTANTS SHALL BE THE SOLE RESPONSIBILITY OF THE CONTRACTOR AND ITS
SUBCONTRACTORS.
3. THE A/E WILL PREPARE THE STORMWATER POLLUTION AND PREVENTION PLAN (SWP3) THAT WILL BE
MAINTAINED ON SITE AT ALL TIME BY THE CONTRACTOR. THE CONTRACTOR SHALL BE RESPONSIBLE FOR
SIGNING THE SWP3 AND MAKE ALL NECESSARY REVISIONS IN ACCORDANCE WITH ORC SECTION 6114.04,
OAC CHAPTER 3745-1 AND THE TERMS AND CONDITIONS OF THE NOI. THE CONTRACTOR SHALL PROVIDE
AN EXECUTED COPY OF THE SWP3, INCLUDING ALL REVISIONS, TO THE A/E.
4. THE PERMITTEE SHALL MAINTAIN A WRITTEN DOCUMENT CONTAINING THE SIGNATURES OF ALL
CONTRACTORS AND SUBCONTRACTORS INVOLVED IN THE IMPLEMENTATION OF THE SWP3 AS PROOF
ACKNOWLEDGING THAT THEY UNDERSTAND THE CONDITIONS AND RESPONSIBILITIES OF THE SWP3. THE
DOCUMENT AND ALL SIGNATURES SHALL BE OBTAINED PRIOR TO THE COMMENCEMENT OF CONSTRUCTION
AND SHALL BE KEPT ON SITE WITH THE SWP3.
5. UNDER THE RULES OF THE OHC000004 PART III.G.2.i, THE CONTRACTOR SHALL PROVIDE A “QUALIFIED”
INSPECTOR WHO WILL BE RESPONSIBLE FOR ALL REPORTING AND COMPLIANCE WITH THE SWP3. THE
INSPECTOR'S DUTIES SHALL INCLUDE:
· PERFORM ALL SWP3 INSPECTIONS, REPORTING, AND LOGS. INSPECT ALL EROSION CONTROL MEASURES
WEEKLY AND WITHIN 24 HOURS AFTER ALL RAIN EVENTS THAT EXCEED ½ INCH OVER A 24 HOUR DURATION.
· FOLLOWING EACH INSPECTION, COMPLETE, SIGN AND DATE THE PREPARED CHECKLIST IN ACCORDANCE
WITH PART III.G.2.i.
· MAINTAIN ALL CHECKLISTS ON SITE FOR INSPECTION BY THE STATE WHEN REQUESTED. INSPECTION
RECORDS SHALL BE KEPT FOR 3 YEARS AFTER TERMINATION OF CONSTRUCTION ACTIVITIES IN
ACCORDANCE WITH PART V.G.
6. ALL EROSION CONTROL MEASURES SHALL MEET THE STANDARDS AND SPECIFICATIONS IN THE CURRENT
EDITION OF OHIO'S RAINWATER AND DEVELOPMENT MANUAL AND RELEVANT INFORMATION AND DETAILS
PROVIDED ON THE SWP3. THE FOLLOWING MINIMUM CRITERIA SHALL BE MET BY THE CONTRACTOR:
· TEMPORARY STABILIZATION: COMPLY WITH OHIO EPA PERMIT NO. OHC000004 PART II.B TABLE 2:
TEMPORARY STABILIZATION. TEMPORARY SEED AND MULCH IN ALL AREAS THAT WILL BE INACTIVE FOR 14
DAYS OR MORE AND WHICH DO NOT REQUIRE IMMEDIATE SOIL STABILIZATION. ALL DISTURBED AND
ERODED EARTH SHALL BE REGRADED AND SEEDED WITHIN 7 DAYS. CONTRACTOR SHALL SUBMIT
TEMPORARY SEED MIX FOR APPROVAL.
- ANY DISTURBED AREA WITHIN 50 FEET OF A STREAM AND NOT AT FINAL GRADE SHALL BE
TEMPORARILY SEEDED AND MULCHED WITHIN 2 DAYS OF THE MOST RECENT DISTURBANCE IF THAT
AREA WILL REMAIN IDLE FOR MORE THAN 14 DAYS.
- ALL OTHER DISTURBED AREAS GREATER THAN 50 FEET FROM A STREAM AND NOT AT FINAL GRADE
SHALL BE TEMPORARILY SEEDED AND MULCHED WITHIN 7 DAYS OF THE MOST RECENT
DISTURBANCE IF THAT AREA WILL REMAIN IDLE FOR MORE THAN 14 DAYS.
· PERMANENT STABILIZATION: COMPLY WITH OHIO EPA PERMIT NO. OHC000004 PART II.B TABLE 1:
PERMANENT STABILIZATION.
- ANY AREA THAT WILL LIE DORMANT FOR ONE YEAR OR MORE SHALL RECEIVE PERMANENT
STABILIZATION WITHIN 7 DAYS OF THE MOST RECENT DISTURBANCE.
- ANY AREA WITHIN 50 FEET OF A STREAM AND AT FINAL GRADE SHALL BE SEEDED WITHIN 2 DAYS OF
ACHIEVING FINAL GRADE.
- ALL OTHER AREAS GREATER THAN 50 FEET FROM A STREAM AND AT FINAL GRADE SHALL BE
SEEDED WITHIN 7 DAYS OF ACHIEVING FINAL GRADE.
7. THE CONTRACTOR SHALL CONDUCT ALL EXCAVATION, FILLING, GRADING, CONSTRUCTION, AND CLEAN-UP
OPERATIONS IN A MANNER SUCH THAT SEDIMENT, GENERATED BY WIND, WATER, OR VEHICLES, IS NOT
DISCHARGED INTO ANY STORM SEWER, DRAINAGE DITCH, RIVER, AIR, OR UNDERGROUND UTILITY SYSTEM.
WIND, WATER, AND VEHICULAR EROSION OF SOIL ON PROJECT SITE DUE TO CONSTRUCTION ACTIVITIES
FOR THIS PROJECT WILL NOT BE PERMITTED. PREVENT EROSION OF SOIL AND DEPOSITION OF SEDIMENT
ONTO OTHER PROPERTIES FROM THE PROJECT SITE DUE TO CONSTRUCTION ACTIVITIES FOR THIS
PROJECT. PREVENT WINDBLOWN SOIL FROM LEAVING THE PROJECT SITE. ANY MUD/SEDIMENT FLOW INTO
STREAM CHANNEL AND/OR WETLANDS MUST BE PROMPTLY REMOVED. IF SEDIMENT EXTENDS BEYOND THE
PROJECT LIMITS, THE CONTRACTOR SHALL BE RESPONSIBLE FOR CLEANUP AND RESTORATION OF ALL
SURFACES AND UTILITY SYSTEMS TO THE CONDITION THAT EXISTED PRIOR TO THE CONTRACT AWARD.
8. STAGE WORK TO MINIMIZE THE AREA OF EXPOSED SOIL, THEREBY REDUCING THE OPPORTUNITY FOR SOIL
EROSION.
9. SOIL EROSION AND SEDIMENTATION BEST MANAGEMENT PRACTICE (BMP) MEASURES WILL BE INSTALLED
PRIOR TO START OF ANY CONSTRUCTION AND WILL BE MAINTAINED AT ALL TIMES UNTIL CONSTRUCTION
HAS BEEN COMPLETED, INCLUDING ALL GRASS BEING WELL ESTABLISHED AND/OR PERMANENT EROSION
AND SEDIMENTATION BMP MEASURES ARE IN PLACE.
10. ALL BMP MEASURES WILL BE INSTALLED TO THE SATISFACTION OF THE OHIO ENVIRONMENTAL PROTECTION
AGENCY (EPA). THE OHIO EPA MAY REQUIRE WORK TO BE STOPPED AND THE STORM DRAINAGE OUTLET
TO BE PLUGGED, IF CONDITIONS BECOME UNSATISFACTORY.
11. REPAIR ALL BMP MEASURES AS REQUIRED DURING CONSTRUCTION. THE CONTRACTOR IS RESPONSIBLE
FOR INSTALLING AND MAINTAINING ALL MEASURES. ANY BMP MEASURE THAT REQUIRES REPAIR OR
MAINTENANCE SHALL BE REPAIRED WITHIN 3 DAYS OF THE INSPECTION. ANY BMP MEASURE NOT MEETING
THE INTENDED FUNCTION OR MISSING MUST BE REPLACED/INSTALLED WITHIN 10 DAYS OF THE INSPECTION
AT NO COST TO THE OWNER.
12. ANY CATCH BASIN THAT RECEIVES STORMWATER FROM CONSTRUCTION ACTIVITIES SHALL RECEIVE INLET
PROTECTION.
- PERIODIC REMOVAL OF COLLECTED SILT AND SEDIMENTATION IS REQUIRED TO MAINTAIN
EFFECTIVENESS OF THE SILT TRAPS, FILTERS, AND BASINS. IF SEDIMENTATION OCCURS, INSTALL
OR CORRECT PREVENTATIVE MEASURES IMMEDIATELY AT NO COST TO THE OWNER. REPLACE ALL
MEASURES DETERMINED TO BE NON-FUNCTIONAL AT NO COST TO THE OWNER.
13. CONTRACTOR IS TO DESIGNATE A SITE DUMP/WASH AREA TO BE APPROVED BY A/E PRIOR TO STARTING
CONSTRUCTION FOR SUCH PURPOSES AS WASHING OUT CONCRETE TRUCKS AND DUMPING
NON-HAZARDOUS WASTE MATERIALS AND FOR MIXING OR STORING CHEMICAL COMPOUNDS FOR
CONSTRUCTION (I.E. FERTILIZERS, CONCRETE, ASPHALT, ETC.), SUBJECT TO THE SUPERVISION OF THE
OHIO EPA. DUMPING OR DISCHARGE OF ANY WASTE MATERIALS TO ANY LUCAS COUNTY SEWERS IS
PROHIBITED. HAZARDOUS WASTES ARE TO BE REMOVED OFF SITE AND PROPERLY DISPOSED OF
CONSISTENT WITH ALL FEDERAL, STATE, AND LOCAL REGULATIONS.
14. KEEP DUST DOWN AT ALL TIMES, INCLUDING DURING NON-WORKING PERIODS. SPRINKLE OR TREAT WITH
DUST SUPPRESSANTS, THE SOIL AT THE SITE, HAUL ROADS, AND OTHER AREAS DISTURBED BY

17. ANY MATERIAL STOCKPILES LEFT OVERNIGHT OUTSIDE A PROTECTED AREA SHALL BE SURROUNDED BY A

IMMEDIATELY PRIOR TO MIAMI UNIVERSITY TAKING RESPONSIBILITY OF THE SITE, THE CONTRACTOR SHALL
