A burn is an injury to skin by heat, cold, electricity or radiation and is usually dry. A scald injury is by something wet like hot water , oil or steam. These are very painful and can result in red skin, blisters, swelling or charred skin.

First degree burn are those that affect the superficial skin layers. They are red without blisters and painful. They heal within a few days and there is no scar. Sunburn also falls in this category and repeated sunburns increase the risk for skin cancer.

Second degree burns are those that affect the superficial skin and also some of the underlying tissues and result in red painful blisters which are bubbles of fluid under the skin. Healing takes longer and scars are seen.

Third degree burns affect the entire skin and usually is painless as the nerves are also damaged. The skin is burnt away and the tissue underneath will appear pale or blackened . Healing requires medical intervention like skin grafting and is usually complicated by scars , contractures and amputations.

Fourth degree burns involve skin and underlying tissues like fat muscle and bone. They are black, charred ,dry and painless. Treatment requires surgical excision and amputations. Multiple complications including infections and multi organ failure can even cause death.

Chemical burns are seen in assault with acid and also ingestion of acid in attempted suicide cases.

The severity is calculated by the extent of burns in the body. The Total body surface area (TBSA ) affected is used to classify to minor, moderate or major burn.

Burns are generally preventable and treatment depends on the severity as mentioned above. Cooling with tap water may be helpful as First Aid. Superficial wounds and minor burns can be managed at home but major burns require prolonged and specialised care in burn centres.

India has very high rates of burn injuries usually in young women . Unsafe kitchens, loose clothing and domestic violence are various reasons .
