Enter your member number, your certification number and the password (four digits of your last name). Then click “Login”.

The transcript is a summary of all PDU’s that have been entered. Click on PDU Reporting Form (red arrow) to report new PDU’s.

Select the activity category from the drop down list and select appropriate category.

For Great Lakes sponsored events add the Provider ID C011and then find your course. Then select the course.

Complete the course information, check the “I agree” box (red arrow) then click preview.

Verify that the information is correct , print the information and then click “Finalize Submission” (red arrow). Note: There is a review process by PMI that may take some time to complete. The PDU’s will not show on the transcript page when they are first reported.
