Graduation Transition Plan• All B.C. secondary school students who are
enrolled in Grade 10, 11 or 12 as of September 1, 2007, must complete Graduation Transitions.

Why?• Graduation Transitions is intended to prepare students for a
successful transition to life after secondary school. In order to meet this goal, Graduation Transitions encourages students to:– take ownership of their own health and learning– examine and demonstrate connections between their learning and their future– create a plan for their growth and development as skilled,
healthy, knowledgeable, participating citizens– exhibit attributes of a model B.C. graduate

What?• All B.C. secondary school students must demonstrate they have
met the following requirements for: – Personal Health - maintain a personal health plan and participate in at
least 150 minutes per week of moderate to vigorous physical activity.– Community Connections - participate in at least 30
hours of work experience and/or community service and describe what was learned.
– Career and Life - complete a transition plan and present significant accomplishments.

Personal Health• Engage in at least 150 minutes a week of regular
physical activity in addition to PE 10.• Develop a healthy living plan appropriate to your
lifestyle that describes:– Sound nutritional habits– Regular exercise routines– Emotional health management– Positive health decisionsPage 6
Documentation• 150 minutes a week of physical activity– Walking to and from school– A school sport– A sport outside school [club teams]– Walking your dog– Running– Going to a gymPage 7
Community Connections• Participate in at least 30 hours of work
experience and/or community service.• This can be at your workplace.• This can be volunteer time at the school
or in the community.

the experience connects with your developing employability and life skills.

What to keep for documentation• For 30 hours of work or volunteer experience:– Letter from your supervisor– Certificates for participation [volunteer activities]– Photos – Pay slips

Career and Life• Develop a plan that indicates you are prepared
to successfully transition from secondary school.• Identify and communicate skills that can
be transferred to life after school.• Present your transition plan to a designated
teacher/administrator at the school.Page 11
Documentation• Create a plan to transition to work or post-
secondary including:– A budget– Information about jobs and/or programs that work
for you

Who?• Students are responsible for planning and preparing
their education, career, and life goals through Graduation Transitions.
• Schools are responsible for ensuring that students have opportunities to meet the Prescribed Learning Outcomes.
• Teachers are responsible for assessing and providing feedback to students. Page 13
Remember• A large part of the Graduation Transitions is
self-directed.• You have a personal responsibility to collect
evidence and keep it in a safe place.• Your Graduation Transitions is a reflection
of who you are and who you want to be.Page 14

Your Assignment• Create a “Word Cloud” with words from the Graduation Transition Plan. Total marks available are
30!• Your Word Cloud must contain the words Graduation Transition Plan in large text or 3 marks will
be deducted. This will serve as your title! • I have provided a sample for you on a subsequent slide.• If you include a minimum of 30 words that describe what you have learned about the Graduation
Transition Plan you will qualify for 20 marks out of the total 25 available for the word list.• If you want to maximize your mark, provide an additional 10 words for a total of 40 which
qualifies you for the full 25 word list marks.• In addition, there are 5 marks available for adding colour to this assignment. No colour=zero out
of 5 for this portion.• You must provide a numbered list of the words you use on the back of the paper I will provide for
