What is
Obesity Obesity is a medical condition in which excess body fat has accumulated to the extent that it may have an adverse affect on health. It is a life-long progressive, life threatening disease which can significantly reduce life expectancy
When weight reaches extreme levels, it is called MORBID OBESITY which is prone to develop numerous medical, psychological and social consequences
It can be defined based on the Body Mass Index into various categories as shown below
BMI Classification:
BMI = wt (kg) / [ht (m )]2
BMI is a useful general guideline and is a good estimator of body fat for most adults between 19 and 70 years of age
However, it may not be an accurate measurement of body fat for body builders, certain athletes, and pregnant women
Healthy weight is defined as BMI equal to or greater than 19 and less than 25
BMI greater than or equal to 25 is overweight
BMI greater than or equal to 30 is obesity
BMI 35 or 40 is severe obesity
BMI of 35 or 40 - 44.9 or 49.9 is morbid obesity
A BMI of 45 or 50 is super obesity
Obesity is also defined by waist size - over 90 cm in males and over 80 cm in females

Bariatric Surgery / Weight Loss Surgery
Bariatric surgery is refer to any surgery performed on stomach or intestine with intent to cure obesity and its co morbidities like diabetes, hypertension, high cholesterol levels etc.,
Principles of the surgery are
• Reducing the appetite by lowering the blood levels of the ghrelin, the appetite hormone
• Limiting the capacity of food intake at one point of time so that the body can metabolize the ingested food faster
• Reducing the absorption of food consumed hence by limiting the deposition of additional fat
• Increasing the burning capacity of the body (Basal Metabolic Rate) hence by throwing away the excess fat
Treating
Obesity The basic principles in treating obesity are
• Diet and Healthy Eating: Healthy eating and eating less calories with
increased physical activity and modified life style habits is the easy way to reduce weight. A registered dietitian can help to design a diet suitable for the individual's need along with the physical activity plan
• : Weight-loss medicines of various kinds are available in Medications
the market, but at GEM OBESITY & DIABETES SURGERY CENTRE we do not recommend them as most of these preparations have more adverse side effects compared to their benefits

Surgical Procedures

Laparoscopic
Gastric bypass
4 5
Laparoscopic Adjustable Gastric Banding - LAGB
Laparoscopic Adjustable Gastric Banding is a reversible procedure for weight loss. This
laparoscopic procedure involves the placement of a silicone band around the upper part of the
stomach. That results in the creation of an hour glass stomach, with a small outlet that allows food
to enter into the lower part of the stomach
This procedure is especially beneficial for large eaters, or those who have tried to lose weight
several times but haven't been able to. Generally this procedure is recommended for younger
patients, as it is reversible. With this procedure one can lose about 40-50% of their excess weight
within 18 months, but patient understanding and compliance is the most important
Sleeve Gastrectomy - SG
Sleeve Gastrectomy is an irreversible procedure that curtails the size of the stomach, this
operation surgically removes two thirds of the left side of the stomach. This results in a new
stomach which is roughly the size and shape of a banana and looks like a sleeve or a tube.
Among the bariatric procedures, sleeve gastrectomy is the most commonly performed
procedure. Sleeve gastrectomy is a restrictive form of weight loss surgery that limits the capacity
of the stomach to 60 cc - 150 cc. Unlike other forms of bariatric surgery, the outlet valve and the
nerves to the stomach remain intact
This procedure also eliminates the hunger inducing hormone called 'Ghrelin', hence patient feels
full after consuming small amounts of food, and lose weight quickly
ROUX en Y Gastric Bypass - RYGB
Laparoscopic Roux en Y Gastric Bypass procedure is a weight loss surgery that involves the
creation of a small stomach pouch with the help of staples that restricts food intake. In addition the
initial segment of the small intestine is bypassed. As a result the food directly enters the jejunum.
This leads to malabsorption of consumed food. This procedure also eliminates the hunger
inducing hormone "Ghrelin". All these mechanisms contribute to significant weight loss
“It can also successful in control of type II diabetes in the short / long term"
Single Incision Multiport Bariatric Surgery - SIMP
Single incision surgery or single-incision laparoscopic bariatric surgery is one of the most
spectacular innovations in the field of minimal access surgery. As the name suggests single
incision surgery is performed through a single incision in the navel. It is essentially a scarless
procedure as the scar gets hidden deep into the navel. Among the bariatric procedures, sleeve
gastrectomy is the most commonly performed procedure using this technique

How can
affect you?Obesity Obesity is rarely just a physiological problem. This disorder has far reaching effects; physiological, psychological and social. On the physical front, obesity can lead to several complications that could be fatal or incapacitating if left untreated
Some of the most common diseases that are directly and indirectly linked to obesity are
Type II DiabetesType 2 diabetes also called Non-Insulin Dependent Diabetes Mellitus
(NIDDM) has the strongest association with obesity. In type II diabetes, the
body does not respond correctly to available insulin. In other words, a person
becomes insulin resistant. People who are overweight are more likely to have
insulin resistance, because fat interferes with the body's ability to use insulin.
Cardiovascular Disease and HypertensionObesity is one of the important factor for this Cardiovascular Disease (CVD)
includes Coronary Heart Disease (CHD), stroke and peripheral vascular
disease. These diseases account for up to 33% of deaths in most
industrialized countries - a figure that's increasing rapidly in developing
countries as well
CancerSeveral studies have found an association between obesity and the
incidence of certain cancers, particularly of hormone-dependent and
gastrointestinal cancers. Obese women have been proved to be at greater
risk of breast, endometrial, ovarian and cervical cancers. There is also
evidence for higher risk of prostate and rectal cancer in men. Obesity also
increases the risk of colon cancer by nearly 3 times in both men and women
OsteoarthritisDegenerative diseases of weight-bearing joints such as the knee are
common complications of obesity. The cause of this is mechanical damage to
joints due to the excess weight. Lower back pain is also more common in
obese people and may be one of the major contributors to obesity-related
absenteeism from work
Psychological AspectsObese people are stigmatized world over. It is perceived as an undesirable
physical appearance, and supposedly embodies several character defects.
Even children often perceive obese children in a negative manner. It is not
uncommon to see patients breakdown even during a consultation
Sleep ApneaSleep apnea is a chronic condition that disrupts sleep. In this condition there
are one or more pauses in breathing or shallow breaths while sleeping. These
breathing pauses can last from a few seconds to minutes. They often occur
5 to 30 times or more per hour. Typically, normal breathing then starts again,
sometimes with a loud snore or a choking sound. Even snoring is a sign of
sleep apnea at it early stages
This results in poor sleep quality that makes one feel tired during the day.
Sleep apnea is one of the leading causes of excessive day time sleepiness
