My Forms
Click My FormsStep 2 of 12
After typing in the form numberClick the Search button
From My Forms portal, select the form you needStep 3 of 12
Click on the form you need,The words will change colorwhen you put the curser overit in order to access form.
When you click the Search button a separate window will open with your search results

Click on the signature box
Once you click on the form Pure Edge will open. You must completely fill in the form before you insert your digital signature.

CAUTION: Routing of documents between Rated Soldier, Rater, Reviewer, Intermediate Rater, and Senior Rater remains IAW AR and DA PAM 623-3.
Click OK
After clicking the signature box you will get a notice letting you know that this is as binding as an original signature.

Click Sign
You will then get a box where the digital signatures are stored

CAUTION: If your “Sign” box is not illuminated then ApproveIt Desktop Software has not been loaded on your system.
* It can be loaded from AKO if you have permissions or it can be ‘pushed’ by our DOIM to your system.
Click “Sign using…Then “Certificate/Smart Card
On the next screen click on the box “Sign using… Then select Certificate/Smart Card

Select Certificate
Click “OK”
Next you will select your Certificate. If you have two, it does not matter which you pick. Then click OK.

Click on “Sign”
The next screen verifies your identity.

Click “OK”
Next screen Validates Signature.

Click EMAIL andYour outlook will Open with fileattached
Signature complete. You may now email to the next approval authority.
