The patient was referred with mild symptoms in 22 which had been root filled and crowned several years ago.

Radiographs revealed inadequate endodontic treatment and large peri-apical lesion. The case involved removal of all GP using hand c-pilot files, reciproc file and eucalyptol oil.

Irrigation protocol included -2% sodium hypochlorite, 17% EDTA and 2% Chlorhexidine final rinse with sonic activation using the endo-activator.

Obturation was completed with GP and bio-ceramic sealer.

Endodontic re-treatment was completed with excellent outcome. Radiographic review is planned in 6 months. This case highlights the importance of a very thorough irrigation protocol along with optimum obturation and increased

apical preparation size to achieve best chance of success in endodontic re-treatment.
