By this Deed, I the undersigned Joy Peace Mukisa of Kisasi Kanisa Zone, Tel: 0772-692 683, daughter of Mr. Joseph Mukasa and Mrs. Catherine Mukasa of Kifamba, Rakai District, do hereby absolutely renounce, relinquish and abandon the use of my former various names of Joy Mirembe Mukisa, having acquired it after changing from Joyce Namaganda Mukasa Mary on the 25th day of August, 2008, and in lieu thereof, I have assumed the name of Joy Peace Mukisa; and in pursuance of such change of aforesaid hereby declare that I shall at all times thereafter in action and proceedings and in dealing and in transactions upon all occasions whatsoever sign and style myself by the names Joy Peace Mukisa and I hereby authorise and request all persons to do designate and address me by such names only.
