Directions (TAD) will assist with all testing procedures

It is important to read it carefully prior to testing

Test Security Agreement 4
Associate Test Coordinator Security Agreement is provided by the DTC or may be downloaded from the EED website http://www.eed.state.ak.us/forms/home.cfm
1. Review carefully and initial each indicator2. Ask for clarification before final signature
Once you sign the agreement, you are responsible for all test security in your building
You are acknowledging that you will follow all guidance, regulation, and instructions associated with the HSGQE
3. Return the signed copy to the DTC

Step One: Receive and Inventory Test Materials [pp. 2-3 TAD]
1. Confirm receipt of materials with the DTC
2. Compare the materials received to the school packing list
3. Compare ranges of security numbers on the school security range sheets against test books received
4. Check quantities of materials
5. Verify the accommodated materials type and quantity received
6. Notify DTC of any discrepancies

Materials for students receiving accommodations were ordered through the Online Enrollment System1. Large Print 2. Braille3. Audio CD (audio tapes are no longer available)4. Sign Language DVD
These materials can also be requested through the DTC Braille and Large Print accommodated materials require
transcription of responses into a Form E Standard Test Book (procedures for transcription are in the Test Administration Directions)
IMPORTANT: Reading Audio CD consists of 2 volumes (2 separate discs). Ensure both discs are provided to students during testing.

DVDs now contain advanced programming, which includes chapters to locate specific groups of test questions.
Audio CDs now have a jewel case insert, which provides the track number for each test question.
Detailed instructions for administering the Audio CDs and Sign Language DVDs can be found in your Test Administration Directions.

All test proctors must be certified teachers Proctors should not be testing own biological
child (same for adopted or foster care) One proctor for every 30 students Proctors must sign test security agreements Three subtests administered in this order
1. October 5 – reading
2. October 6 – writing
3. October 7 – mathematics Distribute, collect, and inventory testing

All testing begins between 7:00am and 9:30am on the designated dates
Students are not permitted to leave within the first two hours of testing
No students are permitted to start after the first two hours of testing
All tests must be administered according to the directions
Have a plan for electronic devices and clarify it with all students and proctors
Have a plan and roster for all returning adults with Certificates of Achievement
Have a plan for students testing at lunch
Have a plan for students finishing testing

Eliminate distractions such as bells, telephones, or announcements
Confirm testing rooms have proper lighting and instructional materials are removed from the walls
Confirm students needing accommodations are addressed
Establish a policy for student cell phones Make sure all CD players have electricity or
batteries and working

All test books must have either a precode label or a district/school label
Test books returned without a label will be considered unused and WILL NOT BE SCORED

Apply a label to the front cover (precode or district/school)
When using District/School labels, bubble in all student information (see TAD for details)
Returning adults with Certificates of Achievement – bubble in what is known

The precode label saves proctors time and eliminates error in student identification
If a precode label is used, the bubbles on the front cover of the test remain blank In cases where students
fill in bubbles – the label overrides any marks (no need to erase the bubbles)

Note the label includes the HSGQE subtests to be taken It is a quick reference for
proctors to use If only the subtests are
inaccurate, the label may still be used – it will not impact any data collection

District/school labels are used in place of a precode label It will assign the student to the
correct district and school for reporting test scores
When a precode label contains incorrect student-identifying information, replace it with the district/school label
When a district/school label is applied, all bubbles on the test book cover must be completed

Do Not use a “Do Not Score” label if there is a bubble marked in the “Teacher Use Only” field on the
front cover
All test books must be secure and tracked
Use “Do Not Score” labels under the following circumstances:1. A label is not positioned correctly
on the test book (redo with another test book and district/school label)
2. A precode label was placed on the test book but the student did not show up for testing.
3. An inappropriate label was placed on the test book (may need to transcribe student answers)
4. Test should not be scored

Use security checklists to track secure materials at all times1. Associate test coordinators use
school security checklists to confirm tracking with proctors daily
2. Proctors use the classroom daily check in-check out worksheet to monitor tracking with students hourly
Secure test materials must be monitored constantly to avoid loss and possible security breach
Students receiving accommodations with Braille, Large Print, or keyboard text, must have their work transcribed into a standard Form E test book

After testing is complete reread all 15 steps to return materials on page 40-42 in TAD
Inventory student test books and ensure the following: Reference sheets are removed Each test book to be scored has a label Test books with “Do Not Score” labels are
accurate “Teacher use only” field is complete where
applicable (consult DTC before invalidating) Braille and Large Print accommodated tests have
been transcribed Package all used and unused test materials in
shipping boxes to DTC as instructed in TAD Notify DTC that boxes are en route
