Infall2015,HospiceGeorgianTriangle(HGT)initiatedasimplifiedreferralprocesswithphysicianstofacilitatetheearlyidentificationofpalliativepatients.Usingthequestion:“wouldyoubesurprisedifyourpatientdiedinthenextsixmonths?”physiciansmakeadirectreferraltoHGTusingthePracticeSolutions/EMRsystem.ThisreferralprocessispartofHGT’sIntegratedPalliativeCareProgram,whichaimstoprovideonepointofentryforpalliativepatientsforsupportandconnectionwithappropriatecommunityresources.Inthispresentationweaddress:implementationofthereferralprocess,physicianengagement,andtheresultsforpatients.WewillalsodiscusstheuseofVolunteerNavigatorsandlessonslearned.

Palliativecareinlong-termcarehasbeenhappeningforaverylongtime.Now,inagrowingareaofhealthcare,homesarebeingaskedtoincorporateapalliativeapproachtocarefromthepointofadmission.This75-minuteworkshopwillexplorewithparticipantsWHATthisreallymeans,WHYthisapproachisimportantandWHYweneedtocontinuethiswork.WewillalsoshowcasecurrentworkbeingdoneintwohomesinNSManddiscussHOWtobegintheprocessofimplementinganewapproachtocareanddiscusstheculturechangeaspectsofthiswork.

Patient-centeredcareisdeterminedbyhowalignedmedicalcareiswithwhatisimportanttothepatient.Forthistohappen,itisimperativetohavetimelydiscussionswithhospitalizedpatientswhoareatahighriskofdying.Failuretodosomayleadtounwantedmedicalintervention,prolongeddeathwithincreasedsuffering,complicatedgriefforfamilymembersandincreasedhealthcarecosts.Barrierstoeffectivecommunicationaremulti-facetedandincludepatient,physicianandsystem-relatedfactors.Inthispresentation,Sherrywillreviewcasestudiesanduseaninteractiveexercisetoexplorehowtospeakwithpatientsandfamiliesabouttheirgoalsandvalues.

Obtaininginformedconsentandengaginginadvancecareplanning(ACP)isanintegralpartofprovidinggoodhospicepalliativecare,andyetsometimeswedon’thaveenoughinformation(orwehavemisinformation)onwhatthisprocessisallabout.Theneedforaclear,consistentunderstandingofsubstitutedecision-making,providingconsenttotreatmentandhowitallfitstogetherwithintheOntarioLegalframeworkhasneverbeensoimperative.Inherstraight-forward,shootfromthehipstyle,JudithwillbringclaritytosomeofthemythsandmisconceptionswemayunknowinglybeperpetuatinginourworkwithACP.

ThisworkshopwillexaminetheneedsofAboriginalpatientsatendoflife.Usingmaterialfromherresearchteam,guidedbyAboriginalEldersandcommunitymembers,CarrieusesvideoasamethodofknowledgetranslationinordertoallowElderstosharetheirwordsusingthetraditionalmethodofconveyingtraditionalknowledgeorallyandexperientially,ratherthanfilteredthroughresearchers’analyticinterpretation.Thevideoisfollowedbyabriefpresentationthatusestheresearchtoexaminewhatcanbedonetocreateculturallysafe,patientandfamilycenteredcareforAboriginalpatientsatendoflife.Thepresentationallowsparticipantstodelvemoredeeplyintosomeoftheconceptsexplainedinthevideo,providestimeforself-reflectionandprovidestheframeworkforaquestionandanswerperiod.

Whenpeoplehaveadvancecareplanningconversations,theyoftendiscusstheirdesiretobeorgandonors.However,thesewishesmaynotbecarriedoutforvariousreasons.Thismayleadtoconfusionandcomplicatedgriefforfamily.Thisworkshopwilladdresssomeofthelimitations,barriersandalsosomeofthepossibilitiesoforgandonationforthosepatientswhohavealife-limitingdisease.Cathywillclarifytheorgandonationprocess,aswellaswhatcanbedonatedandwhen.Mostimportantly,Cathywilladdresshowtomanagepeople’sexpectationswithrespecttothedetailsofbeingadonor.
