HOWEVER: YOU MUST WEAR IT CORRECTLY!

Low and tight on your hips! Not on your waist. Top of belt should be 1in. below top of your hip bone. Exactly how low and how tight to wear SPINE POWER™ is something you should experiment with.

Varying position and tightness can have an impact on effectiveness.

Hold around your hips, buckle in the left hand, vinyl tab in right hand, foam pads next to body.

Slide belt under and then back over white plas-

tic gripper. Don’t pull tight yet.

Shift belt around so buckle sits well behind left hip. To tighten use t h e P U S H / P U L L method as shown. With left hand, hold loose

belt end. With right hand, hold the bottom of belt. Then at the same time, pull with left hand, and push with right hand.

The best way to avoid back pain is to do regu-lar stretching and strengthening exercises to keep muscles and ligaments strong and flexi-ble. Many have found yoga to be the best solution.

When wearing SPINE POWER™, you must still exercise care and caution for your back. You should always practice standard back safety rules. For example:

• Lift with your legs, not with your back

• Bend at the knees, not at the waist

• Be careful how you turn or twist when carrying

So many people could stop their suffering if only they knew of Spine Power™. Do you know someone with back pain? Do them a favor, and give them Spine Power™ and you’ll be giving them a treasured gift.

Tighten until gripper clicks 3 or 4 times. The correct final position for the buckle is on the buttock, just behind the left hip.

Note: always loosen belt before adjusting it or foam pads could

become damaged. Once in place fasten the loose belt end with the Velcro tab.

The exclusive foam finger pads help keep the belt in place. The pad next to the buckle is fine - don’t move it . The pad should sit on your right hip. Here’s how to position it. Re-move pad carefully so you don’t tear it. Put belt on and adjust to final position. Put a paper clip on the belt at the center of your right hip. Remove belt and center pad over paper clip.

Wear the belt on a non-slip fabric ( like cotton underwear). If you wear it on the skin, we recommend removing the foam pads and covering the buckle pad with a piece of fabric, as rubber can sometimes irritate the skin.

First undo Velcro tab. Then, pull the steel bar back. Or slide your left thumb between the steel bar and the belt, at the same time, push back.

Remove foam pads ... Carefully …… they must be soft for your comfort …, therefore they are fragile and can tear. Wash built by hand in warm water. Do not wring. Hang to dry. Do not put SPINE POWER™ In the dryer .

Genuine Leather offers a rich visual and tactile feel; coupled with the elegant scent of leather that sets the patient impression of your treatment room to the highest level.

Heritage™ offers a rich leather-like, soft feel, with the added benefit of being a non-porous material.

“SENSATION” provides a rich palette of beautiful colors, and has several new jewel tones that are sure to spark interest in the clinic. Colors include: Desert Rose, Cobalt, Chocolate, Ash, Dark Green, Taupe, Black, Grey Teal, Mauve, Dusty Jade, Greystone and Adriatic Blue.

Zenith® adjusting tables often outlast the life of a practice and you can expect that some design changes

have occurred since your table was manufactured. • Take a moment to inspect the entire table while you’re retrieving the serial number for your

order. • Next, choose from complete CUSHION SETS (new covers pre-assembled over new foam fillers

and new boards) or COVER SETS (pre-sewn die cut flat goods with coordinating trim edging). • It’s just that simple.

All cushion orders must include a serial number or estimated age to help assure proper fit. Note: if your tables were manufactured prior to July 1983, new base boards need to be drilled / fitted on-site. Also, if your table does not have a pelvic drop, then be sure to order the adapter kit, #800-579-00. Enclosed find SENSATION color selections, leather colors are similar.

Terms of Sale

1. Nature of Document. This document is the acknowledgment of a contract between Williams Healthcare Systems, LLC (the "Company"), and the party to whom this form is directed (the "Buyer") providing for the sale of products specified on the reverse side (the "Products") to Buyer on these Terms of Sale; however, if this document is construed to be an acceptance, acceptance is expressly conditional on the Buyer's agreement to all of the terms and conditions contained herein.

2. Termination by The Company. The Company reserves the right to unilaterally terminate the contract for sale of the Products at any time the Company determines that Buyer's credit is not satisfactory.

3. Cancellation by Buyer. Orders, which are canceled prior to shipment, will be subject to a cancellation charge of 15%. Orders for custom Products may not be canceled once manufacture has commenced.

4. Prices. Products scheduled for shipment within 30 days of the acknowledgment of the contract by the Company will be priced at the time of acknowledgment. Products scheduled for shipment beyond 30 days from the acknowledgment will be priced at the time of shipment. All prices are F.O.B. Elgin, Illinois, exclusive of taxes and duties. Errors or omissions in prices are subject to correction.

5. Payment. Unless otherwise specified by the Company, payment terms will be net cash 30 days from date of invoice. A charge of 1 ½ % be charged per month may be charged to Buyer will be imposed on overdue accounts. Buyers will be liablefor all expenses (including attorneys' fees) incurred in collecting any overdue accounts.

6. Freight. The Company will utililize Buyers carrier [Freight Collect], unless the Buyer instructs otherwise. Claims against carriers must be filed by Buyer on all F.O.B. Shipments.

7. Inspection. Buyer must inspect all Products and notify the Company within 10 days of receipt if there are any obvious defects or shortages. If the Company is not so notified, the Buyer will not be entitled to any recourse for those defects and shortages.

8. Returns. Prior written permission must be obtained from the Company before any merchandise may be returned. All returns will be subject to a 25% restocking charge and will be credited at original invoice value only. All returns must be prepaid.

9. Delivery. Delivery dates are estimates only. Time shall not be of the essence unless specifically agreed in writing.

10. Warranty. The Products (and parts thereof) are warranted to be free from defects in material and workmanship for a period ofONE YEAR from shipment date. If notice of a claim is made within the one year period, the Company will either repair or replace, at its election, all parts that are defective. Repairs by third parties and/or all returns must have been previously authorized by the Company. The Product may be covered by additional warranty terms as explained on the factory warranty.

The Warranty does not cover labor charges on removal of defective parts or installation of repaired or replacement parts. All parts returned must be properly identified. Freight both ways is the responsibility of the user or the Buyer.

Defective conditions caused by user, third parties, acts of God, abnormal use or stress or any other matters unrelated to the Company and beyond its reasonable control are not covered.

This Warranty is exclusive and is in lieu of all other warranties, express or implied, including the IMPLIED WARRANTIES OFMERCHANTABILITY AND FITNESS FOR A PARTICULAR PURPOSE.

11. Consequential Damages. In no event shall the Company be liable for loss of use, revenue or profit or for any other inciden-tal, special or consequential damage.

12. Delays. The Company shall not be liable for any delay or failure to perform if caused by an event or contingency beyond its reasonable control, irrespective of the nature thereof.

13. Modification; Governing Laws; Entire Agreement. No modification of the terms and conditions specified in this document shall be binding upon the Company unless agreed to by the Company in writing. The contract shall be governed by, and construed in accordance with, the laws of the State of Illinois. If accepted by Buyer these provisions shall constitute the entire agreement of the parties with respect to the sale of the Products by the Company to Buyer.

14. Arbitration. Any controversy arising under the contract, or the breach thereof, shall be finally settled by arbitration in Chicago, Illinois under the rules of the American Arbitration Association, and judgment upon the award may be entered in any court having jurisdiction.

15. International. All product prices listed are in US dollars. Export shipping crates are available for $240.00. Converting wiring to 220/240 volt is available for $295.00.
