This mother of two truly enjoys motherhood and all that comes with it . “Being a mom means being responsible, but it’s also a blessing and an honor . It’s so much love,” she says . In her home, there is plenty of love to go around . With the birth of their newborn daughter, Mayara, everything seems to be complete now . But the journey wasn’t easy .

When their now 2-year-old was born, her delivery experience was “unsupported and rushed .” The doctors pressured her into having an epidural and a C-section .

For her second pregnancy, Mercedez became a Community of Hope patient and was impressed with the overall atmosphere and how the doctors treated their patients . Mercedez participated in our unique services, including group prenatal care . “Having other moms around you that understood what you were going through made my experience this time around much better,” she notes . “We consistently encouraged one another and that’s what I loved .”

After a less stressful pregnancy and delivery, Mercedez and Mayara are now home with their family . Dad, Marcus, noticed the difference, saying “being supportive goes a long way .”

Dear Friends,

I hope you are as excited to read this as we are to share . 2017 and so far 2018 have been exciting times at Community of Hope . We’ve launched big projects – re-opening the Marie Reed Health Center and opening The Commons at Stanton Square – after years of work and your support . Thank you .

We are pleased to see deep impact in our work, which will only be enhanced by these expanded sites . Twice a year our team examines 130 data points to measure the effectiveness of our efforts . We pour over them, examining trends and tweaking our approach . Another core set of data is studied by our board each month . Today we are releasing some of these data on our impact with you . We hope you’ll share our excitement when we see our initiatives working .

The big highlights are that almost 1,000 new patients from east of the river trusted us with their care and family homelessness saw a double digit drop . Read on to see where you were a part of a deep impact . Thank you for your help in making this possible .

LEADING CHANGEThere is encouraging news for families in Washington, DC . For the second year in a row, family homelessness has decreased – this time by almost 21% . Through partnering with government and other nonprofits, we are developing effective policies and practices around ending homelessness . One great example is our Homelessness Prevention Program – one reason that families at risk of homelessness are staying stably housed .

Over 900 families on any given day in our city still do not have stable housing . In 2016, the median household income in Ward 8 was only $34,327, half the median for the District . DC has also lost 60,000 units of affordable housing since 2003, contributing to an affordable housing crisis impacting low-income families .

Our housing programs bring hope, easing the effects of these stark inequalities . Our case managers, housing specialists, and other staff provide exceptional guidance and resources . Your generosity helps families gain and maintain housing stability and find strength during difficult times .

We work to end homelessness for families by supporting them through five targeted strategies:

• homelessness prevention – mediation and support help families stay stably housed

• emergency shelter – a few months’ stay with employment and housing search support

• transitional housing – a year’s stay with substance abuse, employment and housing search support

• rapid re-housing – short- to medium-term support with employment search

• permanent supportive housing – long-term support to stabilize families experiencing chronic homelessness .

In 2018, the National Alliance to End Homelessness awarded Community of Hope the Excellence in Ending Homelessness Award along with the DC Department of Human Services .

Erica Walker is happy to be where she is now . Four years ago, she was living in a shelter and working odd jobs . She now has financial stability and her own apartment and she credits her accomplishments to Community of Hope . However, before Community of Hope, Erica’s life wasn’t the one she imagined for herself . She and her three children were in shelter for over a year . She wanted more for herself and for her children, but it was hard doing so by herself; she needed help, which she found through the Rapid Re-Housing program at Community of Hope . With the help of her employment specialist, she secured a position as an armed special police officer .

Stable housing provided a stable income, which lead to a happier and stable family . She plans to spend more time with her children, travel, and work on building her credit . With a clearer view of her future, Erica is excited for what it holds . “My children keep me motivated and grounded because I know I have to provide for them . Just seeing them smile and be happy kids are all I need to keep pushing myself .”

Having the income to meet basic needs is essential to achieving family stability . But in the current economic climate, low-income residents are struggling to support themselves and their families—and they are struggling more than households in other places across the country . The DC Fiscal Policy Institute (2017) explains that “The District has a higher level of income inequality than any state in the country, with households in the top 20% of income having 29 times more income than the bottom 20 percent .” Prospects are greatly improved, however, when families are connected with high-quality nonprofits like Community of Hope .

Through our support, homeless and low-income families can sign up for adult education, job readiness, and job training programs, and obtain assistance with gaining employment . Our employment specialists meet clients wherever they

are in the process of furthering their education or career . Our clients consist of adults and their adult children (in the same household), since the earlier that young adults gain a handle on their finances, the better off they are . We also provide other critical supports including help with maximizing benefits, health insurance enrollment assistance, and connections to community partners .

Our employment specialists supported nearly 200 families on their employment journey, meeting them wherever they were on their path.

EARLY ENTRY PRENATAL CARE

Because of you, we have been able to achieve strong outcomes and grow our services. With your partnership, we are improving the health of many low-income and underserved DC residents.

In 2017, 10,802 patients accessed our health services. This includes 3,370 children and 7,432 adults.

LEADING CHANGEHigh-quality healthcare can be difficult for low-income DC residents to access, especially for people of color living below the poverty level . African Americans in DC are over twice as likely to die of coronary heart disease, six times more likely to die of diabetes, and are at higher risk of cancer, infant mortality, and pre-term births . Infrequent appointments with the doctor can lead to poor prognoses . When preventative care is not easily accessible, patients tend to visit the emergency room with rapidly deteriorating health .

With your support, we provide a medical home to address a patient’s health problems . At Community of Hope, thousands of patients find providers that they trust, and they become physically and mentally healthier .

Community of Hope collaborates with the DC government and other DC nonprofits to improve health outcomes for DC residents . We are working on several initiatives to do outreach and educate patients who regularly use the emergency room, to help them address any barriers to their health . We are using data to tailor approaches to each patient .

Our three health centers are the Conway Health and Resource Center, the Family Health and Birth Center, and Marie Reed Health Center, which was closed for renovations throughout 2017 but re-opened with the financial support of so many of you . At these

three locations, we provide primary care, maternal-child healthcare, behavioral healthcare, dental care, and patient support services .

Our highly effective interventions include:

• Screening and counseling for emotional wellness, Screening and Counseling for children and adults . We have worked to integrate these services into medical care in order to help reduce any stigma associated .

• CenteringPregnancy®, group prenatal care that offers a self-empowering and peer-oriented approach to teaching women about pregnancy, labor, newborn care, and breastfeeding .

• Chronic Disease Group Care that helps patients suffering from diabetes or hypertension manage their diseases and lead healthier lives .

• Reproductive Care Coordinators who help patients with family planning and provide reproductive health education .

• Refugee Health Services that include immunizations, vaccines, care coordination, language assistance, and workshops on hygiene, health, and navigating the US healthcare system

• Healthy Start Home Visiting Services for families with young children at risk of toxic stressors, who need extra community .

You Help People Improve Their Health

A FOCUS ON EMOTIONAL WELLNESSIn 2017 and early 2018, we have rapidly expanded our behavioral health team, supporting the emotional wellness of our patients. While we’ve provided long-term

therapy and even brief screenings for every medical patient with signs of depression, anxiety, trauma or stress, we’ve now added new forms of support to more people.

• A certified play therapist is working with young children.

• An adolescent and family therapist supports teens in individual therapy or in group teen nights.

• An addictions counselor recently joined our team to guide people out of the use of substances.

• A psychiatrist provides medication for those who need it.

• Since 2013, our team of therapists and support staff has grown from one to nearly 12 staff counseling our patients, families in our housing program and the community.

You Helped Patients Manage Their Chronic Diseases.In the summer of 2017, we launched My Health GPS in partnership with DC Health Care Finance Agency . My Health GPS benefits some of DC’s sickest residents who suffer from multiple chronic conditions . Through a team approach, providers and support staff work together to deliver integrated services that improve patients’ health and reduce emergency room visits for preventable conditions . 380 people are currently enrolled and receiving deep supports . We are excited to partner with the city on a program that targets resources to those most in need .

With Your Support, We Returned to our Beloved Marie Reed.

O U R C O M M U N I T Y

Renovations began on our Marie Reed Health Center (MRHC) in June 2016 . MRHC, which caters to refugees, asylees, and immigrants, was too cramped to handle patient flow and had an outdated heating and cooling system . With the renovation, MRHC’s footprint has increased from approximately 5,500 square feet to 9,700 square feet . With expanded space and a more efficient design, both the number of patients we serve and the number of visits we provide will increase by 40% across medical, dental and behavioral healthcare when at full capacity .

MRHC reopened in April 2018 . We held a wonderful celebration to honor our loyal patients, providers, and funders who helped make this journey possible . We are so appreciative of your support!

On October 12, 2017, 440 Community of Hope supporters attended our annual fundraiser and celebration of the impact we’ve achieved with our patients and clients, who inspire us every day . Guests participated generously in a Give Hope auction and enjoyed drinks, hors d’oeuvres, and breathtaking views of the US Capitol and National Mall .

We are extremely grateful to the sponsors, host committee members, and guests who together helped us raise almost

$415,000! We were inspired by the words of Jada Cummings, who graciously shared the impact of Community of Hope on her life and family .

Volunteers offer a wealth of hope to the women, men, and children that we serve . In 2017, 388 people applied their time, energy, and skills to our work, totaling 4,635 volunteer hours! Enhancing our services through their commitment, volunteers deepen our impact by:

• Mentoring youth

• Throwing birthday parties

• Helping out at community events

• Assisting clients with interview preparation

• Providing legal and human resources advice to staff

• Serving as doulas for women in labor

• And so much more .

Your Energy Changes Lives

Stretching from Virginia to the Nation’s Capital are bridges – real and figurative . Mentor, Kelly Peaks-Horner, is a Virginia native . On the other side of the bridge lives Nafia Brown, Kelly’s mentee . The pair lives close enough to consider each other “neighbors,” although far enough to be strangers . The bridge of Community of Hope is what connects them . Kelly, a wife and mom of two daughters, worked with kids before, as she was a teacher for many years in the Arlington County Public School system . She loved connecting with her students and developing them academically and socially .

“My life’s work has been in the pursuit of creating impact and making a difference, especially for young people,” Kelly explains . “Mentoring allows me to continue with and fulfill that internal mission, create a wonderful relationship, and make an impact for both an individual and the community .”When first connected with Kelly, Nafia was shy and very reserved . Over time, she became comfortable with her and would check in with Kelly by sending text messages . They began to bond through their outings, figuring out things they would like to do and goals they would like to achieve . Now in 2018, the two – coming from different sides of the bridge – have met in the middle with the love and trust they have for one another . “In January, Kelly was nominated for Mentor of the Year for United Way of the National Capital and Deloitte’s Mentoring Matters Coalition .

Hosting events for patients, clients, and other community members helps us build trust with our neighbors and provide vital resources . The success of our mission depends on developing meaningful relationships with the vulnerable families and individuals that we serve . We are grateful for your assistance with holding vibrant, inclusive events for thousands of people in 2017 .

LADIES NIGHT INHosted with Breast Care for Washington, this was our first community celebration in 2017 . Women in the community were treated to massages, makeovers, jewelry making, yoga demonstrations, health demonstrations, delicious food, and more .

BELLEVUE BACK TO SCHOOL BASHWith the support of volunteers, we gave away over 900 backpacks to children in our housing programs and in the communities near our Conway Health and Resource Center . Over 1,500 people attended this block-party style event and resource fair, co-hosted with the William O . Lockridge/Bellevue Library and Metropolitan Police Department . Volunteers carefully packed backpacks with school supplies and joyfully gave them out to the community . Thank you for making this fun and important celebration possible!

GIVING HOPE DURING THE HOLIDAYS At our Holiday Market, volunteers helped with greeting attendees, managing an arts and crafts table, reading stories to children, running a raffle, and more . Their willingness to lend a hand wherever needed made this delightful winter celebration for families run smoothly . We are extremely grateful to The Advisory Board Company and Urban Land Institute . We so appreciate your commitment to this event during the busiest time of year!

To help with the cost of festivities, our housing families receive donated $50 gift cards that make it easier to purchase presents and food . Through these gift cards, moms, dads and other guardians with a small amount to spend are able to offer their children and other family members a brighter holiday . Bill Conway, Helping Children Grow, Gallup, The Invisible Hand Foundation, and National Cathedral and many other supporters gave generously to approximately 350 families .

Meeting Needs in Ward 8

The Commons at Stanton Square is a collaborative effort in Ward 8 that supports a community where all children and families thrive . The Horning Family Fund, Horning Brothers, Community of Hope and Martha’s Table are working together to support the healthy development of children and their families in a safe and welcoming environment . Opening in July, this 55,000 sq . ft . campus for community programs will focus on education, youth development, nutrition, food access, emotional wellness, social services, and more . The Commons strives to advance a shared vision of positive outcomes for children and families, ensuring that families thrive and children are well-established for life by the age of eight .

Our partnership at The Commons is a natural step to bring more of our effective models of care to a new and needful

community in Ward 8 . COH will offer a selection of our core programs, including homeless prevention services for families, behavioral healthcare for children and adults, and other family supports . At full capacity, we project supporting over 3,000 people at The Commons .

Beginning in 2020, we will also provide permanent supportive housing to 13 households in a mixed-income housing development at Stanton Square . Horning Brothers is developing five acres of the eight acre property to provide affordable rental and market rate homes for purchase, totaling 160 new homes . We are excited to be collaborating with developers to help more families experiencing chronic homelessness become stably housed . Thank you for supporting our efforts to help more families in need!
