Your Future Your Choice

Program Participation Consent Form Dear Parent / Guardian:

Campesinos Sin Fronteras’ Your Future Your Choice program has developed a program uniquely structured to meet the needs of

students requiring additional behavioral support at Southwest Junior High School. These presentations will help address students’

behavior and discipline providing them with tools to achieve their best academic performance. This is an opportunity for teens to

engage in a learning environment designed to encourage them to take leadership roles and become exemplary youth in the

community and school. The following topics will be imparted by Your Future Your Choice’s Facilitators:

• Self-esteem * Values *Relationships (Family & Friends) *Influence

• Communication *Goal Setting/Decision Making

We will be working with Southwest Junior High School every Monday from 1:15 PM to 4:15 PM. For this reason, we ask the

parents’ permission for their children to participate in these presentations of knowledge and skill acquisition, and to allow us to take

pictures during these presentations that could be used by Campesinos Sin Fronteras to promote future programs.

El programa Tu Futuro Tu Decisión de Campesinos Sin Fronteras ha desarrollado un programa estructurado de forma única para

satisfacer las necesidades de los estudiantes que requieren apoyo adicional en el área de comportamiento en Southwest Junior High.

Estas presentaciones ayudarán a los estudiantes con temas tales como el comportamiento y la disciplina para lograr un mejor

rendimiento escolar. Esta es una oportunidad para que los adolescentes se involucren en un ambiente de aprendizaje diseñado para

animarles a asumir papeles de liderazgo y convertirse en estudiantes ejemplares en la comunidad y la escuela. Los siguientes temas serán

impartidos por facilitadores de el programa Tu Futuro Tu Decisión:

• Autoestima * Valores * Las relaciones (familia y amigos) *• Influencia

• Comunicación • La fijación de objetivos / Toma de Decisiones

Vamos a trabajar con la escuela Southwest Junior High todos los Lunes de 1:15-4:15 Pm. Por esta razón, solicitamos el permiso de los

padres para que sus hijos participen en estas presentaciones de conocimiento y la adquisición de habilidades, y nos permiten tomar

fotografías durante estas presentaciones que podrían ser utilizados por Campesinos Sin Fronteras para promover programas a futuro.
