1. INTRODUCTION

1.1 Context To define the minimum requirements for safe entry and work in or on Confined Spaces.

1.2 Scope This document applies to:

Any person who needs to enter or is in control of a Confined Space.

To any Confined Space at Queensland Nickel Refinery and Queensland Nickel Refinery controlled assets including, but not limited to bore field, bulk fuel handling facility and raw material handling facility at Townsville Port.

NOTE: You have entered a Confined Space when your head (i.e. breathing zone) is within the Confined Space.

3. ACCOUNTABILITIES

Note: All those with accountabilities specified below must be competent with regards to Confined Space entry.

Isolation Supervisor/Permit Issuer (* Isolation Supervisor responsibility only)

o * Isolate energies to an isolation checklist including “Try Tests”

o Develop or review the Confined Space Risk Assessment in conjunction with the Permit Acceptor

o Determine with the Permit Acceptor when, what and how often atmospheric tests need to be completed

o Ensure that the atmosphere has a safe oxygen range where it is possible to do so.

o Communicate Confined Space entry, area and equipment hazards to Permit Acceptors

o Check that a JSA has been completed

o Issue Permits to Work (PTW’s) and clearance certificates to Permit Acceptors.

Supervisor

o Ensure all Permit Acceptors and Users have the necessary certified training and qualifications to accept the CSCC (ie, national accreditation);

o Ensure all personnel required to perform a work task or operate any equipment have the necessary qualifications and experience to safely complete the work: e.g. MEWP work, must have a working at height competency if working at height, hold the prescribed occupational ticket or licence for that type of machinery.

o Review and sign confined space risk assessment and JSA.

o All tasks are undertaken in accordance with other QN Standards as applicable.

Permit Acceptor

o Give information to Isolation Supervisors so scope of job can be understood

o Ensure all team members are trained and competent to perform their role

o Develop or review a Job Safety Analysis (JSA) in conjunction with Permit Users

o Develop or review a Confined Space Risk Assessment in conjunction with Isolation Supervisor/Permit Issuer

o Develop a Confined Space Rescue Plan and ensure a Confined Space Notification form has been completed in consultation with an ERC member if worked is deemed ‘High Risk”

o Complete clearance certificates with Isolation Supervisor

o Ensure Permit Users comply with requirements on all clearance certificates

o Verify isolations

o Sign onto PTW and any other clearance certificates, JSA’s or risk assessments

o Accept PTW’s on behalf of a work team or themselves

o Place/remove red lock & personal danger tags onto a lock box

o Make sure “Stop for 6” is done before job starts and as required throughout the job duration

o Complete job or if the job is not finished inform the Isolation Supervisor/Permit Issuer so the PTW can be suspended

4. DEFINITIONS

TERM DEFINITION

Confined Space

Means an enclosed or partially enclosed space that:

is not designed or intended primarily to be occupied by a person; and

is, or is designed or intended to be, at normal atmospheric pressure while any person is in the space; and

is or is likely to be a risk to health and safety from:

an atmosphere that does not have a safe oxygen level or

contaminants, including airborne gases, vapours and dusts, that may cause injury from fire or explosion, or

harmful concentrations of any airborne contaminants or

engulfment.

Competent Person

A competent person is one who has, through a combination of approved/recognised training, education and experience, acquired knowledge and skills enabling that person to perform a specified task correctly.

Contaminant Any dust, fume, mist, vapour, biological matter, gas or other substance in liquid or solid form, the presence of which may be harmful to persons.

Enclosed or partially enclosed spaces

Spaces which, due to their design, may trap or accumulate contaminants.

Examples on-site include process vessels, storage tanks, boilers, pressure vessels, pipes, sewers, pits and shafts.

TERM DEFINITION

Engulfment The immersion or envelopment of a person by a solid or liquid (e.g. coal, process liquors, water) that is stored within or may enter the Confined Space.

Flammable airborne contaminant

Any dust, fume, mist, vapour or gas present in the air at concentrations that can propagate a flame on contact with an ignition source.

Intended or designed primarily for other than human occupancy

Is the space intended or designed primarily for other than human occupancy i.e. housing for equipment or storage of product?

Factors to consider when determining this include:

Does the space have;

-Adequate ventilation & lighting

-Safe means of access & egress

Safe oxygen range Minimum of 19.5% by volume and a maximum of 23.5% by volume, under normal atmospheric conditions.

5. PROCESS

5.1 Confined Space Identification

5.1.1 Physical Description

To correctly classify a Confined Space it must conform to the following description as per AS 2865-2009:

An enclosed or partially enclosed space that is not intended or designed primarily for human occupancy, within which there is a risk of one of the following:

An oxygen concentration outside the safe oxygen range.

A concentration of airborne contaminant that may cause impairment, loss of consciousness or asphyxiation.

A concentration of flammable airborne contaminant that may cause injury from fire or explosion.

Engulfment in a stored free-flowing solid or a rising level of liquid that may cause suffocation or drowning.

Note: Restricted means of entry or exit is not a consideration in identifying an enclosed or partially enclosed space as a Confined Space. A confined space is determined by the hazards associated with a set of specific circumstances and not just because work is performed in a small space.

5.1.2 Confined Space Classification

The below Risk Control Flowchart has been developed to assist with the process of Confined Space identification. Before using this flowchart please refer to the above explanation of what physical characteristics constitute a Confined Space. If you are in the process of classifying a new Confined Space or de-classifying a previously defined one please ensure that a risk assessment is completed documenting this process.

5.2 Requirements for Confined Space Entry as per AS 2865-2009. NO PERSON SHALL ENTER A CONFINED SPACE UNLESS---

review of the risk assessment has been completed;

written authority is provided to, or completed by, the person responsible for the direct control of the tasks in the Confined Space;

the written authority includes any risk control measures or precautions necessary, including the number of sentries required, for the safe entry and execution of the task;

they are advised of, understand and comply with the requirements of the written authority;

a record of their presence in the Confined Space is maintained;

signs and protective barriers are erected to prevent entry of persons not involved in the tasks; and

appropriate and sufficient arrangements have been made for the initiation of emergency response and, where necessary, rescue of persons from the Confined Space.

5.3 Hazard Identification & Risk Assessment In order to manage risk under this standard and WHS Regulations, personnel must:

identify reasonably foreseeable hazards that could give rise to the risk

eliminate the risk as low as reasonably practicable (ALARP)

if it is not reasonably practicable to eliminate the risk – minimise the risk so far as is reasonably practicable (ALARP) by implementing control measures in accordance with the hierarchy of control

maintain the implemented control measure so that it remains effective

review, and if necessary revise, risk control measures so as to maintain, so far as is reasonably practicable, a work environment that is without risks to health and safety.

5.3.1 Confined Space Hazards

The major hazards of Confined Spaces include:

1. Oxygen deficiency caused by:

o Slow oxidation reactions of organic or inorganic substances.

o Rapid oxidisation (combustion).

o Dilution of the air with an inert gas.

o Absorption by chemicals or other products.

o Physical activity of personnel within the space.

2. Oxygen excess caused by:

o Leaking oxygen supply, such as gas cutting or heating equipment.

o The use of chemicals that release oxygen.

3. The presence of contaminants on surfaces or in the atmosphere:

o Solid, liquid, gas, vapours, fumes, sludge or particulates.

4. Operation of moving equipment within the space:

o Conveyors, rotating or moving parts, machinery e.g. conveyers.

5. Uncontrolled introduction of substances (steam, water or other gas, liquid, vehicles operating nearby)

6. Engulfment by solids

7. Electrocution from live conductors within the space:

o Conductors either inherent to the space or from introduced electrical equipment.

8. Explosion or fire

9. Biological, mechanical, radiation, environmental, noise hazards and manual tasks

5.3.2 Risk Assessment Requirements

It is a requirement that a competent individual assesses the hazards and risks associated with Confined Space work prior to permit issue. At Queensland Nickel we do this using two risk management tools, the YHSS049b Confined Space Risk Assessment and the YHSS052a Job Safety Analysis.

This standard acknowledges that overlap does exist between factors to be considered in the risk assessment and the JSA. If a risk has been addressed in one format there is no need to replicate it in the other.

5.3.2.1 Risk Assessment

Assesses the risks associated with entering the space to perform work and emergency rescue. The risk assessment should be retrieved from the Confined Space Location Register prior to the day of entry, where possible, and reviewed. This will ensure that sufficient time is available for;

review and revision if there is evidence that there is a change in the risk

5.3.2.2 Job Safety Analysis

Identifies and controls risk as it relates to performing work within a Confined Space. It shall be developed/ reviewed by the Permit Acceptor and team members and sighted by the Isolation Supervisor prior to issuing a clearance certificate.

They shall take into account: (refer to appendix 6 YHSS049c Confined Space Risk Factors)

1. The tasks required to be conducted, including the need to enter the Confined Space.

2. The range of methods by which the task can be performed.

3. The hazards involved and associated risks involved with the actual method selected and equipment proposed to be used

5.4 Safety of Atmosphere

5.4.1 Clearing and Purging

The Isolation Supervisor/Permit Issuer is responsible for ensuring that a Confined Space is cleared and purged before handing over a Confined Space to the Permit Acceptor.

Where practicable all solids and liquids, which may present a hazard to persons inside the Confined Space, should be removed prior to entry. Clearing of the contaminants must be performed using a suitable purging agent.

When flammable contaminants are to be purged, equipment designed for use in hazardous locations must be used and sources of ignition carefully controlled.

Contaminants removed from the Confined Space must be removed to a location where they present no hazard to persons or equipment, and a try test conducted by the Isolation Supervisor/Permit Issuer after the space has been purged or flushed to ensure all contaminants have been removed.

The Confined Space Standard shall be followed if entry is required for either clearing or purging of the Confined Space.

5.4.2 Atmospheric Testing

The requirement to test the atmosphere within the Confined Space will be identified in the risk assessment.

The atmosphere is to be tested as near as practicable to the time of entry and repeated as conditions change, particularly after a significant break in work activity. The Isolation Supervisor/ Permit Issuer is to ensure the testing is carried out in a safe manner.

Where toxic or flammable gases and vapours may be released and/or build up, it may be necessary to monitor the atmosphere continuously.

If the Lower Explosive Limit (LEL) is lower than 5%, access to the Confined Space is allowed under the conditions of the risk assessment.

If the Lower Explosive Limit (LEL) is equal to or greater than 5% then no entry is permitted except in case of emergency response.

5.4.2.1 Safe Entry Procedure When Gas Testing

If entry is required in order to complete atmospheric monitoring the following steps shall be followed:

A sentry shall be present

Remote testing shall be completed initially.

Note: When using an extended/telescopic sampling tube, the time taken for the sample to be drawn into the monitor shall be taken into account. This information will be found in the operating manual and will differ between the types of monitor used.

Assess if it is safe to enter and if so what controls must be in place to address the risks associated with entry.

When entering the space to monitor, the atmosphere shall be tested ahead of the individuals’ progress.

5.4.3 Ventilation Plan

Ventilation of the Confined Space will need to be determined by the Isolation Supervisor/Permit Issuer. Some of the main reasons for ventilation are:

Removal of inert gases from a purged system

Dilution of flammable or toxic gases

Fume and dust extraction e.g. welding fumes

Temperature control.

Where the oxygen level or atmospheric contaminants cannot be brought to safe levels, no person is to enter the space unless equipped with appropriate PPE including air supplied respiratory protective equipment.

The Confined Space Standard shall be followed if entry is required in order to install ventilation equipment within the Confined Space.

5.5 Confined Space Clearance Certificate A confined space clearance certificate provides a formal check to ensure all elements of a safe system of work are in place before people are allowed to enter the confined space.

The confined space clearance certificate is to be supported by a Confined Space Risk Assessment and JSA, sentry checklist, Permit to Work, a completed Isolation Checklist and Confined Space Rescue Plan (if high risk).

The Isolation Supervisor/Permit Issuer is responsible for ensuring that a Confined Space Clearance Certificate (refer to appendix 1 YHSS049d) is completed before a Confined Space is handed over to the Permit Acceptor and that the period for which the Confined Space Clearance Certificate is valid for is clearly stated on the certificate.

The Permit Acceptor is responsible for ensuring that the persons doing the work are advised of, understand, and comply with, the requirements of the Confined Space Clearance Certificate.

The Permit to Work and Confined Space Clearance Certificate must be revalidated when there is evidence to indicate that there is a change in the risks affecting those within the space:

A change of the Permit Acceptor

A significant break in work continuity (greater than two hours)

A significant change in atmosphere or work to be performed e.g. after hot work has been carried out.

A significant change in the weather i.e. water or wet conditions, lightning or wind.

Increased activities outside the space with the potential to affect those inside.

Access and egress to the space is compromised.

5.6 General Requirements

5.6.1 General

The following items shall be considered as possible risk factors and suggested risk controls implemented where necessary:

5.6.1.1 Portable Electrical Equipment

All portable electrical equipment associated with the work within a Confined Space shall have an information tag attached at or on their source of supply that carries the message “CONFINED SPACE WORK - DO NOT REMOVE” and be set up and operated in accordance with the requirements of the Hand Tools, Portable Appliances and Extension Cords Standard.

Additionally, any portable electrical equipment to be used within a Confined Space shall;

be protected by an RCD located outside the Confined Space that is tested prior to every shift, or,

be supplied by an earth-free extra-low voltage power supply located outside the Confined Space.

Double-insulated electrical tools with the correct IP rating for the environment should be used where available.

Battery powered tools that are not connected to any external power supply are not considered Portable Electrical Equipment under the requirements of this section.

5.6.1.2 Explosive Atmospheres

Any electrical equipment for the detection and measurement of flammable gases or otherwise to be used within an explosive gas atmosphere should comply with the appropriate requirements of AS 2865.

5.6.1.3 Fall from Heights

The effects of personnel falling during ascent or descent shall be considered and appropriate precautions taken against accidental tripping, falling or collapsing including fall restraint equipment where appropriate, or as a second option fall protection equipment.

It is not always desirable to specify the use of a safety harness, safety lines or rescue line, as this may be impracticable or may introduce a hazard or unnecessarily hinder free movement within a Confined Space. In the event of free movement being hindered, alternative plans should be arranged for fall prevention, fall protection and possibly retrieval.

5.6.1.4 Entry/Exit

The method of entry and exit chosen shall minimise the risk of personal injury. Excessive climbing, bending or stooping shall be avoided where possible and the impact of using an external retrieval system shall be considered.

Wherever possible all services (lighting, power, ventilation etc.) shall be run into the Confined Space through a different point to that used for entry / egress.

Care should be taken to ensure that access and egress to the space is not compromised while work is carried out i.e. by placement of equipment or structural changes.

5.6.1.5 Hazardous Substances

Where contact with hazardous or toxic substances exists, such as acids, caustics, acidic or alkaline sludge, full body protection must be worn (disposable overalls, gloves, rubber boots) with respiratory protection equipment. Protective equipment shall be decontaminated at an appropriate place before removal, and removed carefully to prevent any remaining contaminant contacting the skin. The decontamination area is to be defined and prepared prior to work commencing.

5.6.1.6 Gas Cylinders and Hoses

Except for cylinders for use with self-contained breathing apparatus, no cylinder of compressed or liquefied gas should be taken into a Confined Space.

The compressed or liquefied gas supply to equipment in the Confined Space should be turned off at the cylinder valve by the sentry when not in use and gas cylinders should be secured.

Hoses supplying gas-operated equipment used in a Confined Space should be located, suspended or otherwise guarded to avoid accidental damage. These hoses should be tested for leaks prior to installation.

5.6.1.7 Signage and Barricades

Confined Spaces should at all times be secured against unauthorised entry and, where practicable, permanently signposted. Signs and protective barriers/barricades will be erected to prevent entry of persons not involved in the task.

The area owner is responsible for ensuring that a sign is attached near the entrance to all permanent Confined Spaces e.g. tanks, bins, silos and vessels where it is practical to do so.

The Confined Space Sentry is responsible for ensuring that any open spaces are made safe and left in a safe condition i.e. barricading and appropriate signage put in place.

Where possible danger signs shall state “Danger”, “Confined Space”, “Confined Space Clearance Certificate required before entry” and barricading shall comply with site standard requirements.

5.7 Emergency Response Readiness Due to the risks associated with Confined Space entry the need to rescue and retrieve personnel may arise. Planning and preparation through the use of risk assessment tools must be completed to ensure emergency response readiness is established prior to entry. Considerations will be given to what could go wrong, what equipment must be on hand and if one or more trained and competent individuals should be standing-by to assist in case of emergency.

5.7.1 Confined Space Notification Form

For all Confined Space entries deemed to be high risk as part of the risk assessment process it is a requirement that a Confined Space Notification form be completed and sent through to Operational Support Officers for processing. This shall be done prior to the day of entry, where possible.

This electronic form is located on the Yabnet home page via “Useful Links”, “Frequently Used Forms” dropdown window. The form is designed to provide the ERC with information to enable them to quickly locate the space and assess the risks in a rescue situation.

5.7.2 Confined Space Rescue Plan

For all Confined Space entries deemed to be high risk as part of the risk assessment processes it is a requirement that a YHSS049f Confined Space Rescue Plan be completed. This form is intended to provide Queensland Nickel Emergency Response Crew (ERC) with a comprehensive understanding of the risks associated with the planned Confined Space entry and plan for a potential emergency response. The Confined Space Rescue Plan shall be completed prior to entry and in consultation with or reviewed by an ERC Member.

5.7.3 Sentry

Entry into a Confined Space is not permitted unless there is a competent sentry outside the Confined Space, and entry is authorised. A sentry cannot be responsible for more than one Confined Space at Queensland Nickel.

The Confined Space Sentry must not leave the Confined Space, engage in other work activities outside their accountabilities or enter the Confined Space while there are persons remaining inside, including and especially during an emergency situation.

As part of the completion of the YHSS049g Confined Space Sentry Checklist, the sentry will complete a radio check through to Operational Support Officers (OSO) prior to entry and, for high risk entries, confirm that they have received a Confined Space Notification Form.

In the case of an emergency situation developing within the Confined Space the sentry will notify OSO immediately as per Confined Space Sentry Checklist.

Without entering, a sentry may also use stand-by emergency equipment to assist in the evacuation and rescue of Confined Space team members, if competent and trained to do so.

Once an emergency has occurred in a Confined Space, the only personnel that can enter the space are the Queensland Nickel ERC, Rescue Assistant/s or external emergency services.

5.7.4 Rescue Assistant

A Rescue Assistant may be used for Confined Space activities have been deemed high risk as part of the risk assessment process. The need to appoint one or more individuals to this role will be evaluated as part of the Confined Space Rescue Plan.

Like a sentry, the Rescue Assistant/s shall remain outside the space while persons are still inside. Unlike the sentry, their role is to be prepared to enter the Confined Space in order to perform an emergency rescue if required.

The Rescue Assistant shall be fully trained and competent in Confined Space entry and the use of any equipment to be used.

5.8 Communication

5.8.1 Radios

All Confined Space Sentries will be required to carry a radio when performing the role of sentry. Channel 4 shall be used by sentries to radio through to OSO just prior to entry and upon exiting. At all other times the sentry shall stay in radio communication with the Control Room Operator (CRO) via the area channel.

In the case of an emergency, in which the ERC’s resources will be utilised elsewhere, all Confined Spaces will be evacuated.

If an emergency were to occur within a Confined Space the sentry shall switch to channel 1 and follow standard emergency protocols to call through to OSO’s and initiate emergency assistance.

5.8.2 Radio Protocol

Effective radio communication requires a simple protocol be followed in order for OSO’s to be able to easily identify what Confined Space is being referred to.

When radioing through to OSO’s

identify yourself by name and area,

state the purpose of your call,

communicate that you have finished talking by saying “over”.

5.9 Managing Work within Confined Spaces

5.9.1 Multiple Tasks within a Confined Space

Within a Confined Space multiple tasks may have to be performed under the one isolation.

For more than one task to be performed under the one Permit to Work each task must have a separate JSA that captures and control overlapping risks.

Where more than one job is being performed, each with its own permit, the Master Permit should be considered to manage the numerous permits and interactions. Refer to the Permit to Work and Isolation Standard.

The Isolation Supervisor / Permit issuer should have an understanding of the multiple task intended to be undertaken if occurring simultaneously, prior to issuing additional permits.

5.9.2 Multiple Entry Points for Confined Spaces

The design of some vessels at Queensland Nickel may necessitate entry/exit from more than one point.

For all Confined Spaces where there is a need to enter/exit through more than one entry, the following will be considered:

The use of multiple Sentries, one at each entry point.

Management of the sign-on, sign-off process (written)

Any deviation from a single entry and exit must be approved by the permit issuer / isolation supervisor before the permit is issued.

5.10 On Completion of the Job The sentry shall radio through to OSO’s to inform them that the job is complete.

Before a Confined Space is closed the Isolation Supervisor/Permit Issuer must check that no person is inside and all equipment and materials have been removed.

When the Confined Space is ready to hand back, the Permit Acceptor must sign the appropriate section of the permit.

If any changes have been made to the Confined Space Risk Assessment the Isolation Supervisor/Permit Issuer will ensure that the Confined Space Location Register is updated appropriately.

5.11 Monitoring To ensure compliance to the requirements of this standard the YHSS049h Confined Space Self Assessment tool shall be used and/or Field Leadership activities.

6. TRAINING

Persons involved in ensuring the safety of those within the Confined Space and all those required to enter must be trained and recognised as competent.

Recognition will be issued by the Training Department following:

Providing proof of national accredited Confined Spaces competency.

Refer to the appendix 8 YHSS049h Confined Space Self Assessment tool as a guide before performing any tasks relating to this standard.
