What do we mean by prevention?
Primary prevention/promoting wellbeing:
– aimed at people who have no particular social care needs or symptoms of illness
– focus is on maintaining independence, good health, and promoting wellbeing
Secondary prevention/early intervention:
– aims to identify people at risk and to halt or slow down any deterioration
– actively seek to improve a person’s situation.
Tertiary prevention:
– aimed at minimising disability or deterioration from established health conditions or
complex social care needs.

Why commission for prevention?
• Delaying and reducing the need for care and support
• Shifting the model from ‘doing to or for’ to enabling people to ‘do for
themselves’ before a time of crisis or significant deterioration in ability
• Seeking to protect our most vulnerable residents and helping people to
lead independent and fulfilling lives giving them more choice and
control over their care and support
• Supporting and maintaining residents ability to engage positively in
their own communities
• Enabling residents to manage their own health and social care needs
