If allowed by the contract. price adjustments must be requested at the time of renewal in accordance with the contract documents. Price adjustments are not automatic or retroactive and are only implemented upon request by the vendor at the time of renewal.

If any of this infomiation has changed, make corrections direcUy on the form. and return with this letter. It is essential this information be accurate for payments to be processed in a timely manner.

Thank you for responding to my letter of May 24, 2018 and agreeing to renew the contract. The contract will now expire October 31, 2019.

As provided for in the contract, an increase of 2.8% in price is allowable. The 2.8% represents the percentage increase in the Services category of the CPI-W section of the Consumer Price Index for the latest twelve months ( June 2017 through June 2018) for which statistics were available, This information was obtained online on July 12, 2018 from the U.S. Department of Labor, Bureau of Labor Statistics' web site at: http://stats.bls.gov/news.release/cpi.t04.htm.

Attached is the pricing schedule which will become effective on November 1, 2018.
