Next Workshop: Creating the Destiny – Making it Happen
Workshop #5 will
⟩ Reflect on what the ‘dream’ design is for the team and create a ‘destiny map’ using actions and roles from Workshop #4
⟩ Reflect on how the team members, including patients and family members, will integrate their roles into the destiny steps, plotting the actions and roles along a time trajectory for implementation
⟩ Identify what needs to be in place to move each action into performance and consider what resources are available. A strategy will be needed to secure any missing resources
⟩ Adjust the destiny map to make it happen within the available fit and consider how the team will know when each destiny step is being practiced
