⟩ To identify where current practices fit the ‘dream’

⟩ To identify what changes are needed to move the ‘dream’ to a full reality

⟩ To assess the fit of the ‘dream’ with patient’s ideal

⟩ To explore the roles of team members to achieve the ‘dream’ design

⟩ To consider the fit of the ‘design’ into existing organization and service structures

⟩ To create a beginning design for the interprofessional patient-centred collaborative practice ‘dream’ within the team

⟩ All participants will consider which aspects of the visions should be retained for depicting how the model will work

⟩ Agreed upon vision will be retained for the next stage in the design development.

Using your flip chart with your small group:⟩ Discuss what does not need to be changed in your current practice to shift from

current practice to the ‘dream’

Note: Ensure that you do not lose the strengths of your current teamwork during this process

⟩ Discuss what needs to be enhanced to achieve the vision for the IPCP model

⟩ Be prepared to share your work with the total group



Sharing Ideas to Arrive at the ‘Dream’

⟩ Share each small group’s set of ideas

⟩ Consider which ones are duplicates

⟩ Discuss which should be retained as key to the ‘dream’

⟩ Move the others to a ‘parking lot’ for later consideration

⟩ Move to an integrated set of ideas for further work on the ‘dream’

Small Group Work – Visualizing

Use your flip chart and take each of the ‘shared’ integrated ‘dream’ ideas and visualize:

⟩ How you would actualize each one within your IP patient-centred collaborative team?

⟩ Make any adjustments required to ensure the actions that will be needed reflect all members of the team (including patients/family members)

⟩ Ensure your group has explored the actions needed for moving the IPCP model into a design



Sharing Actions to Visualize the IPCP Model

⟩ A representative from each of the small groups will share its set of actions to achieve the vision

⟩ All participants should consider the actions and decide which must be retained and which can be placed in the ‘parking lot’ for now

⟩ Finally create a collaborative set of actions to move on to the next decision making stage

⟩ Consider the roles (in a general context) of all members of the team (including patients and family members)

⟩ To assist you in this work, consider the actions you have identified for the ‘dream’ to move to ‘design’

⟩ What working relationships need to be integrated?

⟩ How will your principles for working with each other be included?

⟩ How will collaborative leadership be integrated?

⟩ How will disagreements be addressed?

⟩ How will work be allocated across the team?



⟩ Create three columns on your flip chart and record:

o Column 1 = enter the integration action

o Column 2 = what roles need to be considered?

o Column 3 = what teamwork strength is already in place to support each action & what needs to be developed?

⟩ Choose those that resonate with the ‘dream’

⟩ Save the others in the ‘parking lot’ for future consideration

⟩ Come to agreement on the roles needed

⟩ Your team is but one grouping within your service and organization

⟩ The fit can act as a pilot to help meet your organization’s values and strategic plan

⟩ Your team can become the ‘IPCP champions’ in the organization

⟩ Take each of the agreed upon actions and and work on what needs to happen to make the actions become a reality

Consider three aspects:

⟩ What changes are needed at the organization level?

⟩ What changes are needed at the service level?

⟩ What changes are needed within the team members?

Next Workshop: Creating the Destiny – Making it Happen

Workshop #5 will

⟩ Reflect on what the ‘dream’ design is for the team and create a ‘destiny map’ using actions and roles from Workshop #4

⟩ Reflect on how the team members, including patients and family members, will integrate their roles into the destiny steps, plotting the actions and roles along a time trajectory for implementation

⟩ Identify what needs to be in place to move each action into performance and consider what resources are available. A strategy will be needed to secure any missing resources

⟩ Adjust the destiny map to make it happen within the available fit and consider how the team will know when each destiny step is being practiced
