This PowerPoint reviews the responsibilities after testing, and how to pack and ship all the materials back to ACT.

I have developed a checklist that I hope will be helpful for you and your staff to use. This checklist will be emailed to you. It lists all the steps you need to take before, during, and after testing. It also references which pages those directions can be found in the Supervisors Manual 2012-2013

Test materials must be kept secure after students complete the tests, then packed and returned to ACT immediately 1Room Supervisor Responsibilities:Verify the following:All student names are printed and gridded properly with bubbles completely marked (grid) Test Booklet Number and Form Number Student Demographic Information is completeStudents have signed the Terms and Conditions on the test bookletsAccommodation Code if usedErase all stray marksAll necessary reports have been completed:WorkKeys Roster (student names and verification)WorkKeys Test Materials Tracking LogWorkKeys Irregularity ReportWorkKeys Seating DiagramRecorded test time of start, 5 minute warning, and stop times (pages 40, 45, 49 in Supervisors Manual)WorkKeys Report of Accommodated Tests (WorkKeys Special Testing Readers Agreement and WorkKeys Special Testing Interpreters Agreement, if applicable)Return all materials to test supervisor

After the Tests

Make sure all of the demographic information has been entered and bubbled on the Answer Document. Erase any stray pencil marks. Make sure student has signed the Terms and Conditions on the test booklets. Make sure all forms have been completed and turned in (WorkKeys Roster, WorkKeys Test Materials Tracking Log, WorkKeys Irregularity Report, WorkKeys Seating Diagram, WorkKeys Report for Accommodated Tests, etc.) 2All materials must be returned to ACT immediately after testingIf needed to store materials at any time, securely store all materials, in accordance with ACT guidelines (locked cabinet within a locked room)

Materials will need to be separated by: Scorable and completed formsNonscorable Documents and Unused FormsEach package(s) will be mailed to different addresses.

Three forms are required for packing: the WKEYM Pack/Return Slip, Site Supervisors Header, and Scoring/Reporting Order Form. Make a copy of the WKEYM for each box being returned including one for the non-scorable materials. The Building and Group Headers are used to separate scoring reports between buildings and Room Supervisors, they are optional .5Site Supervisors Header - Required

Refer to the WKEYM Pack/Return Slip for needed information (was included in the shipment of test materials)

Complete sections A, B, E, F and G

Clearly write and bubble each section

Sections C, D, H, and I do not apply

Complete sections A, B, E, F, and G of the Supervisors Header. You will need information from the WKEYM to complete sections B and E. See page 81-82 of the Supervisors Manual for details.6Building Header - Optional

The Building Header is not required. If your site is testing at more than one building and you want your score reports sorted and printed for each building, complete a Building Header for each building.Use the Test Supervisors name and mailing address on this form or the results will be mailed to the other building(s).Details are on page 83 of the Supervisors Manual

If all of your students tested in one building, then you do not need the Building Header. If students tested at multiple locations and you want test results separated by building then complete a Building Header for each testing site. It is important to use the same contact information on all copies of the Building Header or tests results will be sent to separate buildings. Details are on page 83 of the Supervisors Manual.7Group Header - Optional

The Group Header is not required. If you want your score reports sorted and printed for each group tested, complete a Group Header for each group or class. Use the Room Supervisors name on this form, not the Test Supervisors nameDetails are on pages 83-84 of the Supervisors Manual

The Group Header is not required, unless you had multiple testing rooms and want results separated by the Room Supervisor. Use the Room Supervisors name, if you complete this form. Details are on pages 83 and 84.8

Packing Shipment Box(Not using Building or Group Headers)If you only tested in one building or want all results to be combined, then:Place the completed Answer Documents in the bottom of the box.Place the Site (Test) Supervisors Header in next.Place all related forms in the box (Roster, Tracking Log, Irregularity Report, and Seating Diagram, etc.)Finally, put in the Scoring and Reporting Order FormMake sure your Site ID Number is on the Scoring / Reporting form!!!

9Packing Shipment Box(Using Building or Group Headers)

The diagram on page 85 of the Supervisors Manual shows how to pack tests from multiple buildings with multiple Room Supervisors.The Answer Document Folders go in first, separated by class and then by buildingThe Group Header goes in next followed by the Building Header.This alternates until all Answer Documents are placed in the box.Next put in the Site (Test) Supervisors Header.All other test administration forms go in (Rosters, Tracking Log, etc.The Scoring and Reporting Form should be on top. Do NOT use the Next Business Day Scoring NoticeDO NOT put unscorable tests materials in this box! They will be shipped separately.

If you had multiple testing sites and room supervisors AND you want the results separated by building and room supervisor, then use the packing instructions from page 85 of the supervisors manual. Do not us the next business day order form, the extra cost is not covered by this project.10Once you have placed the scorable documents, headers, used test administration forms, and order form into the box and sealed it:Label each box with your return address and the institution/site code and ACT WorkKeys ScoringThe shipping address is:

With all used tests documents and headers in the box, seal it for shipment. Label the box with your return address , the institution code and the phrase ACT workkeys scoring. Congratulations!!

The make-up test days is March 18-22. You will follow the same process for the make-up test days and shipping of the materials back to ACT. The next step is to wait for the results from ACT. Please continue to follow the established timeframe for WorkKeys with retesting dates, etc.
