Read, sign, and upload a Liability Waiver form in Symplicity: o Download the domestic/international work term forms Connect>Arts Co-op Community Course>Forms & Info o Save your completed waiver as a PDF and upload into Symplicity: Go to Symplicity>Profile>Co-op. Click Edit below to your current work term Attach your PDF in the Liability Waiver field. Pay your registration deposit for the term (Summer & Fall terms only) through the UBC Student Service Centre. Drop any courses you are registered in during your work term that you cannot take o Please contact the Arts Co-op Office if the withdrawal deadline has passed Pay your tuition for ASTU course before the deadline set by the UBC Student Service Centre. DURING YOUR FIRST/SECOND WEEK ON THE JOB: Complete your Placement Information and Learning Objectives in Symplicity. o Go to Profile>Co-op Placements o Click Edit next to your current work term o Fill in the blank fields and enter your Work Plan on the second half of the page o Click Save Meet with your supervisor to discuss your Work Plan and provide them with a copy for their reference. DURING EACH 4-MONTH WORK TERM: Projects Month 2: Submit your Professional Profile in Connect before the deadline. Month 3: Submit your Informational Interview Reflective Analysis on the assignment page before the deadline. Month 4: Submit your Skill Development Project on the assignment page before the deadline Site Visit: Forward the site visit information from your coordinator to your supervisor. Schedule a site visit with your coordinator around the mid-point of your work term. TWO WEEKS BEFORE THE END OF YOUR WORK TERM: Complete your Student Evaluation in Symplicity. Print a copy of the form to review with your supervisor. Remind your supervisor to complete the Employer Evaluation in Symplicity, using the login & password that will be provided to them via e-mail following the site visit. Your supervisor should print a copy to review with you. Meet with your supervisor and review the evaluation forms and assess your Skills Development Project’s level of confidentiality. Your supervisor will indicate the level of confidentiality in the Employer Evaluation. NOTE: Update your portfolio or e-portfolio with new work samples and revise your resume (remember to ask for permission if you plan to include work documents or files in your portfolio). You do not need to submit your portfolio to the Co-op Office. Students that miss the submission deadlines without advanced approval from the Co-op Office WILL receive a failing grade for the term, and may no longer be eligible to apply for future co-op work terms. Your final grade will not be assigned until the Arts Co-op Office receives all of the above. It is your responsibility to follow-up with your employer if you have any outstanding items.
