Bending Section:

Transition between insertion tube and bending section destroyed and partially detacted.

Distal Tip:

Scope’s biopsy channel separating and lifting, also multiple kinks causing restriction

Root Cause:

It appears that there are multiple causes related to the scope’s damage; first normal wear and seal failures allowing fluid to enter the scope could be responsible for transition break down. Second, possible over torque or bending could have assisted in the damage to the biopsy channel and bending section.

Corrective Action:

Rebuild the scope replacing all internal components as well as the insertion tube.

Root Cause:

It appears that the biopsy channel was punctured allowing fluid to enter the scope. Once present the fluid attacked internal components causing further damage.

Scope is all original, never been touched outside Manufacturer. Biopsy channel completely dissolved in B.S., only the wire wrap remains.

Corrective Action:

Due the fluid damage to the internal components scope requires an internal rebuild.

Received with internal fluid, very cloudy, but cleaned up.

Root Cause:

Scope received with major biopsy channel leak allowing fluid to invade and attack internal components. Biopsy leak appears to be a tear or cut from a forcep. Bending section damage is a result from prior and past fluid. The corrosion shown is a result of long term fluid exposure.

Corrective Action:

Due to bending section, insertion tube, and fluid. Scope should be completely rebuilt.

Root Cause:

Due to the fluid entering through the insertion tube and biopsy channel scope suffered image bundle damage. Insertion tube damage is from normal use and leak at the biopsy port as well is from normal use.

Corrective Action:

Scope requires a complete rebuild
