Enclosed you will find the above -referenced document. This documentfulfills the requirements set forth in Task 2.3 of the SupplementalHydrogeologic Investigation Plans, Winston Thomas Facility and Bennett'sDump, August 1986. The requirements for each site were to provide:
A. A plot plan showing the locations of core holes and monitoring wells;B. Coring and drilling logs;C. Geophysical logs;D. Water level elevations;E. A ground-water quality sampling schedule; andF. A description of any changes made to the Supplemental Hydrogeologic
Investigation Plan.
The Data Transmittal provided is divided into two sections, one for eachof the two sites. These sections are further subdivided into thefollowing sections to address each requirement of Task 2.3:
I. Site Plan;II. Soil Boring, Rock Coring, Monitoring Well Installation Data;III. Geophysical Data;IV. Hydrogeologic Data;V. Ground-water Sampling Schedule; andVI. Changes/Additions to Supplemental Hydrogeologic Investigation

BORING, CORING AND MONITORING WELL NOMENCLATUREWINSTON THOMAS FACILITY
SUPPLEMENTAL HYDROGEOLOGIC INVESTIGATION PLAN
The locations of the monitoring wells installed during the Phase 2, Site
Investigations are presented on the proceeding figure depicting the Winston
Thomas Facility. All monitoring wells are designated by the prefix "MW-"
followed by a number designating a location. The location numbers may be
followed by one of the letters listed below:
D - Indicating a deep cased bedrock well.
i - Indicating an intermediate in depth cased bedrock well.
^- S - Indicating a shallow PVC-cased overburden well.
If no letter follows the location number, then the well is an open-hole
bedrock well.
/•->. The prefix "B-" followed by a number indicates a soil test boring at the
^ location designated by the number. The prefix "C-" followed by a number
indicates a rock coring at the location designated by the number. At most
locations the sequence of drilling activities began with a soil boring. A rock
coring was drilled next and finally the corehole was reamed to construct an
^ open-hole or deep-cased bedrock monitoring well. Therefore, for example at
Location 1, the soil test boring would be designated as B-1, the rock coring
as C-1 and the open-hole monitoring well MW-1. Continuing with this
example, if casing was installed at Location 1 to construct a monitoring well,
it would be designated as MW-1D. Any intermediate bedrock and shallow
overburden wells, if installed would be drilled at adjacent locations and
designated as MW-1i and MW-1S. If no soil test boring or rock coring was
installed, the well at Location 1 would be designated as MW-1 if open-hole or
MW-1D, if cased. In this instance, no reference to B-1 or C-1 would occur.• ^

Throughout this Data Transmittal, locations designated by a number have
different prefixes attached to them. In summary, with respect to the Winston
Thomas Facility:
1. B-1. C-l and MW-1 refer to the same location.
2. B-2, C-2 and MW-2 refer to the same location.
3. B-3. C-3 and MW-3D refer to the same location,
fl. MW-3i is located immediately adjacent to MW-3D.
5. MW-3S is located immediately adjacent to MW-3D.
6. B-4, C-fl and MW-a refer to the same location.
7. B-5, C-5 and MW-5D refer to the same location.
8. MW-51 is located immediately adjacent to MW-5D.
9. B-6. C-6 and MW-6 refer to the same location
10. MW-7 refers to open-hole bedrock well where no soil test boring or
rock coring was installed.
11. MW-8 refers to open-hole bedrock well where no soil test boring or
rock coring was installed.

locations the sequence of drilling activities began with a soil boring. A rock%*
coring was drilled next and finally the corehole was reamed to construct an
open-hole or deep-cased bedrock monitoring well. Therefore, for example at
Location 1, the soil test boring would be designated as B-1, the rock coring
as C-1 and the open-hole monitoring well MW-1. Continuing with this
example, if casing was installed at Location 1 to construct a monitoring well,
it would be designated as MW-1D. Any intermediate bedrock and shallow
overburden wells, if installed would be drilled at adjacent locations and
designated as MW-li and MW-1S. If no soil test boring or rock coring was
installed, the well at Location 1 would be designated as MW-1 if open-hole or

different prefixes attached to them. In summary, with respect to the

3. B-3. C-3 and MW-3 refer to the same location.
<J. B-fl, C-Q and MW-ft refer to the same location.
5. B-5. C-5 and MW-5 refer to the same location.
6. MW-6D refers to a cased bedrock well where no soil test boring or
rock coring was installed.
7. MW-6i is a cased monitoring well located immediately adjacent to

The four proposed monitoring well locationswill be sampled continuously through soilsand bedrock to an approximate depth of 80feet or to the top of the HarrodsburgLimestone Formation. (Note: Theboring/coring depth was modified in thePhase 1 Progress Report [January 1987] tobe to 25 to 30 feet into the aquifer).
