and neglect can be screened daily,

including weekends. The screening team will meet no later than within 24 hours of receiving the report. The case will be

assigned to a worker at the time of the screening. If the agency receives several

reports the children believed to be in the most unsafe conditions will be prioritized.

3. In cases involving criminal investigations

by Law Enforcement, the worker assigned will attempt to coordinate with Law

Enforcement to make contacts within timelines. If that is not possible the worker will make face to face contact

child/children to assess safety.

4. Supervisor will review data entry practices

to ensure accurate recording of screenings and contacts.

5. Workers will document efforts toward

compliance with response requirements where timelines are not met.

Measurement/Monitoring Plan: Agency supervisors and/or directors will run the SSIS General Report “Time to Initial

Charting Reports (Federal Indicators Round 2, Measure C1.4), Traverse and Wilkin County had 0% re-entry in 2010. Pope

County’s re-entry rate was 57.1%

Pope County will reduce foster care re-entry rate to 25% (as measured by Analysis and Charting report).
