SETUP INFORMATION FOR RSQ® ASSESSMENT FOR TRIAGE

A) Access & Log In as the Reviewer

1) Type or copy and paste the following URL into the address window on your browser: https://www.thesullivangroup.com/multiaudit/org_admin/login.asp?audit=3

2) Under “Members Area” click on “Reviewer Login”.

3) Enter your User Name and Password exactly as written in the “General Information” section on the e-mail instructions. The login fields are case sensitive.

B) Edit Your Information

The Reviewer can edit their user name, password, first name, last name, and email address. If the Reviewer decides to change their password, they should enter their new password and confirm new password. Click the “Update” button to confirm the changes.

C) Enter Records by Chief Complaint

To begin entering records, the Reviewer clicks on the “Enter Records by Chief Complaint” link.

The Reviewer selects a chief complaint and clicks on the link to open the template (e. g. Adult Chest Pain template). All sections of the template will be on the same page. The Reviewer must scroll down to see all the questions.

D) Learning Components

The template contains two sections to display educational feedback to the Reviewer. The goal is to guide the triage nurse toward triage decisions based on either ESI or CTAS criteria, practice standards, and a solid knowledge base related to clinical presentations.

Triage Considerations (Displayed at the top of your template) As elements are documented in the template, triage considerations associated with specific triage levels drawn from ESI and CTAS criteria, best practices, and evidence based practice are displayed. It is important to understand that many elements of both ESI and CTAS are not absolutes; rather they require the nurse to “consider” specific criteria. Critical Thinking Tips (Displayed to the right of your screen) Each section of the template contains an area where “Critical Thinking Tips” are displayed; the tips depend on the items checked by the Reviewer. These critical thinking tips are the foundation knowledge to help frame validating your triage decisions.

E) Demographics In this section, the Reviewer enters patient information. “Account #” needs to be unique for each patient. “Age” is a calculated field. “Immediate Bedding” this question only appears if this is your process and your Facility Administrator enables it. The Facility, Chief Complaint, Triage Nurse, and Reviewer are not questions; they are all pre-populated by the System.

F) Submit the Template Complete the template and “Submit” At the end of each template the Reviewer must enter a Triage Level Post Review and may enter comments. The Triage Level Post Review may be the same as the initial triage level, or it may be different. Enter what you think the triage level should have been after entering all of the information into the template and reviewing the Triage considerations and Critical Thinking tips. Click “Submit” to upload the record to the database. Click “Clear” to completely clear the template and start over.

G) View List of Records Entered

When “View List of Records Entered” is clicked, the system displays a list of triage records that have been entered by or for a Triage nurse.

The Reviewer will have the ability to view:

1) all records they have entered as the Reviewer, and 2) all records listing them as the Triage Nurse

All of the reviewer’s records entered in the system will appear in the table below. If the reviewer wants to change the parameters to drill down to a specific chief complaint or triage level, use the drop down boxes at the top of the page.

The “Triage Nurse” dropdown contains the name of the Triage Nurse - in this case “McRoberts, Debbie.”

The “Reviewer” dropdown contains Reviewer's name - in this case “McRoberts, Debbie”. The name of the Reviewer may be different if a record was entered by a Peer Reviewer.

The “Chief Complaint” dropdown lists the chief complaints that have been entered into the system.

If your Facility does not use the process of “Immediate Bedding” then the Immediate Bedding filter and column should not appear. If you click the “View” link in the far right column, you will be able to view the entire completed record you entered.

The system will display your completed records. As the Reviewer you will have “read only” privileges, you will not be able to edit your own records. Only the Facility Administrators can edit a record once it has been submitted. If you make an error when entering a record, let your Facility Administrator know and they will be able to edit your record. Your Facility Administrator will require the following information to easily find the record and make the edits.

• Chief Complaint • Account Number • Date of Visit

All sections of the template will appear on one page. The Validator's name will only appear if your Facility Administrator has allowed the Reviewers to see the Validator’s name.

H. View Matching Triage Level Report

The “View Matching Triage Level Report” displays the percent to which the original triage level matches the Reviewer's triage level Post Review.

The report includes all records where the Reviewer is the Triage Nurse. The Reviewer can apply filters and then click on the “Run Report” button to update the report.
