CHAD Background Check Requirement

CHAD 496 Observation and Assessment Lab is a required course that will take place at the Center for Children and Families (CCF) on campus.

• Fingerprinting and background check through LIVESCAN

• Tuberculosis Test (TB)

• Proof of MMR immunity

• Immunization against Pertusis (whooping cough)

• Flu shot (between August and December or submit a waiver form if the flu shot is declined)

Students will be required to verify a clear background check including statewide, county, and federal screening (socialsecurity number, fingerprinting) before placement in the Center for Children and Families (CHAD 496 Observation and Assessment Lab). Students will also need to show proof of inactive Tuberculosis, full immunization against Measles,Mumps and Rubella (MMR), immunization against pertussis and get a flu shot (students may waive the flu shotrequirement – see CHAD website: www.csusm.edu/chad/index). Background checks or additional screening are alsorequired for students seeking to work with minors in other courses (ex: CHAD 450, PSYC 495, PSYC 499, or EDUC380/HD380).

This worksheet is intended for supplemental use only. The University will use your Academic Requirements Report (ARR)to track your graduation requirements, including those for your major. Please continue to check your Student Center and ARR for accuracy.
