Quick Tips Here are some helpful tips to set you up for success:

• Once you complete your Health Risk Assessment, you will need to join a team (or create your own if you haven’t already) in order to advance to filling out the Non-Smoking Attestation.

• Complete the Non-Smoking Attestation under the “Wellness Credit” tab directly after completing the Health Risk Assessment so you don’t forget.

• The Non-Smoking Attestation is not the question referring to smoking in the Health Risk Assessment. This attestation is a completely separate step located on the “Wellness Credit” tab.

• If you forget, don’t worry we will send you personalized reminders specific to the OCHS program. If you receive email reminders from StayWell: County of Orange Wellness Program, you still need to take action on completing the steps.

• If you registered with a personal email address, be sure to check that email address for OCHS reminders and notifications.

• Reach out for help before the August 20, 2021 deadline. You can call, email, use chat or reach out to the Onsite Program Manager. See the yellow box to the right for ways to contact us. We’re all cheering for you!
