and “Schedule” for due dates.• Internship Completion Plan:

Common Answers: *You do NOT count lunch hours, unless your supervisor approves such.
*You DO count supervision hours.
*On your internship Completion Plan (ICP), plan 450 internship hours without adding Integrative Field Seminar hours. (We will keep track of such on the Monthly time log, but you need 450 in-agency internship hours.)
Learning Agreement• Is completed on IPT
• Guides your internship• Malpractice insurance is tied to it
• Links CSWE Core Competencies & Practice Behaviors to your individual internship
• Is a dynamic form– the LA must be completed and signed before the Final Evaluation
portion of the form will be enabled.• Make sure your FI is correct on the LA.
Learning Agreement:*Is completed and stored on IPT (is a legal document)*Select at least ONE activity per practice behavior*Check any activity you think you might participate in during this internship. (Such connects to your malpractice insurance.)*You don’t have to complete every activity you check, but if you participate in an activity during your internship it needs to be checked on your Learning Agreement! *You will NOT see the Competency Deficiency Box…. Hopefully this will not apply to anyone. *Do NOT sign your LA until you have reviewed it with your Field Instructor (FI)
Often times, they will want to add something…*Once the LA is signed it is a legal document, and your malpractice insurance is tied to it.*This is what you will see online…
This is what your Learning Agreement will look like when you have completed it. Activities you did NOT check, will drop off the form.
Once you complete your LA, and review it with your FI, you sign the form. Your FI will then be able to sign the form. After these two signatures, Wendy will be able to sign the form, and it then becomes a legal document. You can make changes/additions to the form later, but it does require clearing all signatures, and having all parties re-sign the new LA. If you need to do such, please contact Wendy.
Final Evaluation
• Required for grade and continuing into your second internship.
• LA must be signed before the evaluation section is enabled.
• Review the Evaluation Rubric Training prior to assessment.
• Self-assessment, personal narrative competency reflection, and your signature come first.
• FI assessment, discussion with you, and FI signature is second.
• Last signature is Wendy’s.
1. You will first assess yourself; a drop down selection menu will appear on your evaluation form, along with a competency rubric which will help you assess your competency. You will see something like this….
2. As part of your evaluation, you will be asked to write a Narrative Self-Reflection of your Competency for each of the Nine CSWE Competencies. There is a free-write box on the Evaluation form for each competency. We will review such during the course of the semester; this will help you have a better understanding of the Social Work Competencies and how you are doing/learning/growing as it relates to each competency.3. Once you have completed assessing your competency, you will sign the form. 4. Your FI should then assess your competency, review such with you, and sign your form.5. Wendy will review, assess, and sign your Evaluation form.6. Once Wendy has signed your Evaluation form, it is complete, and becomes a part of your official education file.
Monthly Reflections and Time Logs• Required documentation of your
hours and supervision hours.• Is an excellent learning and
assessment tool for both you, as the student, and us, as educators. (And you should see what other Universities require their interns to write…!)
• Narrative comments are only read by Wendy and Ruth, when scoring your reflection log.
• These reflections constitute a large portion of your field grade.
• See the example under the “Content” tab of 654R.
Monthly time logs are due on the fifth day of the following month. (Ie. May’s log is due June 5th) The form will auto-calculate your total hours per week and per month. Do not “double count” supervision hours. (Ie. On Wed. May 16th, you are in your agency for 7 hours, 1 of which is supervision. You would put “5/16/2018” for the date, a “6” under In-Agency Internship Hours, and a “1” under Supervision Hour. This will total 7 hours. If you attend Integrative Field Seminar that day, you would add a “1” under the Integrative Field Seminar hours column.)
Direct Client Contact Hours:These are hours where you are directly working with aClient. This could be in an Assessment, Individual or Group therapy session, Case management session, phone contact, etc.
Monthly Reflection Narrative Questions
• Describe the internship activities you have performed during this month, and a success you have seen.
• What challenges have you experienced this month in your internship, and what steps have you taken to resolve those challenges?
• Please provide a case reflection from your internship that illustrates your internship activities, interactions, interventions, etc. Link your interventions to an evidence-based practice theory or modality.
• How has your competency (knowledge, values, skills and cognitive/affective processes) as a social work practitioner increased this month?
• How have this month’s internship experiences impacted you spiritually (non-graded)
• Describe what you are planning to do in the month ahead.
Trauma Informed Clinical Social Work Practice Questions on the Monthly Reflection Time Log
will help you view all your clients througha “Trauma-Informed Lens”
• How many trauma-informed internship hours (viewing clients and their treatment through a Trauma-Informed Practice lens) did you complete this month? (give # or hours)
• Considering the “12 Core Concepts for Understanding Traumatic Stress Responses in Children and Families” (NCTSN, 2012), reflect upon what you have learned about trauma-informed social work practice this month.
• You do NOT need to be taking the Trauma course Sp/Sum to complete these questions. You do however, need to familiarize yourself with the 12 Core Competencies (noted above, and linked to on the monthly reflection log form), so you can address the two trauma questions in an educated and appropriate manner.
• To receive the Trauma Certificate of Completion you will need to complete the Trauma course with a grade of B or above, answer these reflection questions monthly and complete at least 200 trauma informed internship hours across the course of your two MSW internships.
Self-Care Curriculum• Added three years ago to field curriculum
based on observed need.• Student response has been overwhelmingly
positive!
Self-Care (Cont.)
• 100% of students (every student in every cohort) said we should continue teaching the self-care curriculum!
• Purpose: To help you learn the importance of, and how to practice, regular intentional Self-Careas a Social Work Professional.
Self-Care (Cont.)
• Each week we will either:– take a self-care assessment or – discuss a different form of self-care. – All handouts are under the 614R LS “Content” tab or online at the
caps.byu.edu website, “Stress & Biofeedback” section.• Weekly Intentional Self-Care Activities- submit via Learning Suite
(614R). Each week you will submit either:– one sentence indicating what you did for self-care that week, – or a picture of you/your self-care experience that week.– a plan for what you will do next week for self-care (this helps make
your self-care more intentional in nature).• Weekly Intentional participation is important (that’s the “regular”
part…) Submissions are due each Saturday night.• Why assignments? No Guilt … Permission to practice!
