history Do not attach copy of shot record. Just record

date. Ratio – 4:1 (youth: adult) Signature required on BOTH front and back of

form. Medication form must be filled out and given

to nurse with meds at time of camp.

Monday, March 10th from 7:00p.m. – 8:30 p.m. at Fort Bend County Library – Sugar Land Branch, 550 Eldridge Road

Thursday, March 20 from 7:00p.m. –

8:30p.m. at Fort Bend County Library First Colony Branch, 2121 Austin Parkway (Tentative Location)

Thursday, April 8th from 7:00 pm to 8:00 pm at Round Table
