The Alma Project Privacy Notice - Referral Form Information we have about you When you want to come to The Alma Project, we ask a referee – support worker, health professional etc – to fill in a referral form. This provides us with personal information about you – your name and a contact number or email address. We also ask for sensitive information about you – your mental health issue, any information that would help us support you, and whether you have a criminal or violent history. Why we have this information We need this information so that we can contact you; so that we can assess if we can give you the support you need; and to make a risk assessment about whether we can protect our current clients and workers. The lawful basis for asking for this information is Legitimate Interests from Article 6 of the General Data Protection Regulations May 2018, and the conditions for processing this special category data are from Article 9(2)(h) of the same regulations. Conditions for processing criminal information are from Article 10 of the GDPR and Schedule 3 of the Data Protection Act 1998 and when it is passed, the Data Protection Bill Article 9(1c) and (2). Who we share this information with Only relevant paid Alma workers and Alma partners who jointly run groups with us will see this information. We will not share it with anyone else or any other organisation without your consent, unless it contains information that we are obliged by law to report, or we think there is a danger to your life or the lives of others. We will shred/delete it as soon as possible, and at least within 1 month:

a) of you communicating that you no longer want to attend the project or

b) if we have not seen or heard from you in 3 months.

Your rights about personal data

You have a right to information (this notice)

You have a right to see or get a copy of your referral form (within 30 days of asking)

You have the right to rectify incorrect data.

You have the right to make a complaint to The Alma Project Trustees (write to Stefan Tymkewycz at address below) or the Information Commissioner’s Office – ICO.org.uk or tel. 0303 123 1113
