Practicum / Internship Manual Updated 5/01/10

Manual Overview

The rehabilitation counseling education process consists of two primary components. The first is the didactic instruction received in the core classes of the degree plan, and the second is a clinical educational experience. The clinical education component is divided into two parts, the practicum and the internship. These are two distinct stages of clinical education, and each has unique objectives and educational activities.

We want to emphasize that the clinical experience component of your education should be based on your professional needs, goals, and interests. While there are certain requirements that must be met (i.e. supervision, hours required, etc.), there is a great deal of flexibility for the designing of a meaningful clinical experience. We encourage you to take this opportunity to explore new areas of interest, new skill sets, and new systems of service delivery. Before continuing through this manual, please take a moment to answer the following questions:

1. What do you want to accomplish while engaged in your clinical experience?2. Are there certain populations with whom you are interested in working?3. Do you have specific interests you would like to explore during the clinical

experience?

This personalizing of the practicum and internship is essential to having a meaningful experience. It may be tempting to simply want to complete the experience to fulfill the requirements for graduation. We encourage you to put effort into this component of your education. The amount that you benefit from your clinical education experiences will be directly proportional to the amount of effort you put into it.

The Practicum The practicum includes “classroom instructional experiences (audio-visual tapes and individual and group interaction) dealing with rehabilitation counseling concerns, and clinical experiences (on or off campus) that facilitate the development of basic rehabilitation counseling skills” (CORE, 2009). The practicum is essentially the time that the student prepares for internship in skill and knowledge acquisition, administrative activities (i.e. arranging internship sites, obtaining professional malpractice insurance, etc.), and introduction to the clinical setting. The practicum is a graded course, which requires the documentation of 100 hours of engagement in practicum activities. Of the required 100 hours, 40 must be clinical exposure (client contact) hours completed at a clinical site. The practicum must be successfully completed before continuing on to the internship, except in extenuating circumstances with full faculty approval.

Practicum / Internship Manual Updated 5/01/10

The Internship

The internship is a 600 hour clinical experience, conducting the professional activities of a rehabilitation counselor, under the supervision of a certified rehabilitation counselor. Of the required 600 hours, 240 must be direct (client contact) hours completed at a clinical site. In addition to onsite supervision, the student will receive supervision from a faculty member regarding the professional activities in which the student is engaging. Evaluations of the student, onsite supervisor, and internship site are essential components of the internship process. The internship is graded on a pass / fail basis. Satisfactory completion of the internship is required for graduation.

Clinical Sites

The clinical sites utilized for the purposes of completing the objectives of the practicum and internship may be the same or different, depending on the interests and needs of the student. All sites must be approved by MRC program faculty and an affiliation form (located on page 18), must be submitted, by the student, to Amy Doty prior to the accumulation of any practicum or internship hours. In addition, the affiliation agreement between TTUHSC and practicum/ internship site must be completed prior to accumulation of any practicum or internship hours. The form Practicum/Internship Site Description is to be completed and submitted to your advisor for faculty approval. On rare occurrences the faculty may request the student to change clinical sites if the site is not meeting the needs and clinical requirements of the MRC Program. In all cases hours may not be counted toward the fulfillment of required hours until the necessary approvals have occurred (including the site contract, site orientation, acquisition of malpractice insurance, etc.). Hours may not be counted retroactively.

Course Enrollment

Prior to enrolling in the practicum or internship, students must have the approval of their academic advisor. Once approval has been granted, the student needs to locate an acceptable site to do their clinical experience. The student will then give the following information to their advisor: Organization Name (complete legal name), Address, Supervisor and exact title, Email, Phone Number and the start date. All of this has to be completed before the student can begin their clinical practice. It is also the responsibility of the student to be familiar with the policies and procedures of the practicum and internship prior to the start of the semester in which they are enrolling.

It is the purpose of this manual to provide detailed directions to students, through policies and procedures, for the successful completion of the clinical education component of the Master’s degree in Rehabilitation Counseling.

Clinical Skill Development

The focus of the practicum includes the development of micro-skills of counseling, development of client focused counseling skills, gaining an understanding of the roles and functions of the rehabilitation counselor, building skills of self-evaluation and introspection, the importance of consultation and supervision, and the development of necessary attitudes for rehabilitation counselors. In addition to these basic skills that are requisite for rehabilitation counseling practice, we will introduce the case conceptualization process.

Practicum Objectives for Clinical Skill Development

1. Develop Micro-skills of Counseling – The Micro-skills process for counselor education is a predominant method for instruction in counseling skills. It focuses on the basic skills that are utilized in the counseling process. Students will receive didactic instruction on the micro-skills of counseling, and will demonstrate proficiency through videotaped counseling interviews.

2. Develop Case Conceptualization Skills - Case conceptualization is the process of considering all of the information in a given case, coming to a conclusion of the variables that are impacting the person’s functioning in a negative way, and designing appropriate intervention strategies with your client.

3. Understand and Implement A Client Focused Approach – In as much as the utilization of empathy, genuineness, and unconditional positive regard are essential to the development of an effective working relationship, students will explore this process within the context of their work with persons with disabilities.

4. Understand the Components of Ethical Practice – Special attention will be given to the importance of ethical conduct for rehabilitation counselors in accordance with the CRCC professional ethical standards and state laws.

5. Demonstrate Competence in Ethical Problem Solving – Students will be required to demonstrate familiarity with the process of ethical problem solving, and competence in their implementation.

6. Develop Self-Awareness and Reflection Skills – An important component of being an effective counselor is the development of self-awareness and reflection skills. Since counseling is an interpersonal process, counselors need to be in a constant state of reflection and be self-aware, or the counseling interaction may be negatively impacted. While you have had reflection exercises in the didactic component of your education, you will have an opportunity to experience self-reflection in the context of the counseling relationship, particularly as it relates to the interaction with the consumer.

Practicum Required Activities

Each student will participate in activities that will provide the opportunity for skill development. Skill development will occur by practicing the skills outlined by the course instructor, and submitting videotapes and case studies/conceptualizations for evaluation and feedback.

The following are guidelines for your practicum activities:

Role-playsEach student will be required to participate in specified number of role-plays as outlined by your faculty supervisor for presentation to the class. The class and faculty supervisor will provide the student with feedback on the counseling skills demonstrated during the interview.

Case Studies/Conceptualization Case studies will be conducted to develop case conceptualization skills. Case studies will follow a prescribed format.

Peer Feedback Every student will provide feedback to other students regarding their interviewing skills. This is important for both students (i.e. the evaluator and person being evaluated) as it will increase your skill at recognizing the counseling skills, and their appropriate implementation. Forms for providing feedback will be provided.

Practicum Clinical Supervision

The MRC professor is the instructor for the practicum course. Students will participate in 1 ½ hours of group clinical supervision each week with their practicum instructor and classmates. Supervisory sessions will consist of role plays, evaluation and feedback on counseling skills, discussion of case conceptualization, addressing personal growth and development of personal insight and self-reflection. Supervisory sessions will take place utilizing web conferencing technology.

As clinical supervision is a synchronous activity, students will need to schedule a time to engage in supervision with the practicum instructor. Weekly supervisory sessions are mandatory.

Should you experience technical difficulties that prohibit you from actively participating in weekly supervision sessions, it is the student’s responsibility to address these issues with Curt Finger, prior to the next supervision session. You must notify your faculty supervisor of the status and/or resolution of the issue. Failure to follow this protocol may result in an inability to complete the requirements for practicum. If the technical issues cannot be resolved prior to the next supervision session, a separate supervision session

12

Page 13: Introductionsoahs1.ttuhsc.edu/mrc/documents/Practicum_Internship... · Web viewIntroduction to agency staff and personnel, and instruction on the roles and functions they provide.

Practicum / Internship Manual Updated 5/01/10

must be scheduled with the faculty supervisor. Multiple absences from supervision sessions are unacceptable and will result in a one on one meeting with the faculty supervisor, and possibly program director, to determine the appropriate action.

Once the student begins participating in activities within the clinical site, the site supervisor will provide at least one hour of uninterrupted, face to face supervision each week (This is in addition to the 1 ½ hours of group supervision.)

Physical Oversight/Supervision

Any student who is at a practicum site where 33% of their work is without physical oversight, meaning their supervisor is physically located in the same office, must adhere to the following additional requirements:

1. Students will be required to video or audio tape three sessions with a client (one at the beginning, one at the middle and one at the end) to be reviewed and evaluated by the faculty supervisor.

2. The middle tape will be evaluated by the site supervisor.3. A weekly log of activities must be turned in to the faculty supervisor every

week during the duration of the clinical.4. Faculty must have a meeting with the site supervisor prior to site approval to

explain the additional requirements.

If a student is working at a business they own, they will have to also hire an outside supervisor who has a Master level degree in counseling or closely related field and have current certification and/or licensure.

Ongoing EvaluationPeriodic reviews of the student’s progress occur throughout the semester. There will be a mandatory midterm progress review. Then following the mid-term review the student will receive a written appraisal of the student’s progress from the professor.

Technology RequirementsFor the provision of clinical supervision in the practicum and internship, students will need to obtain a web conferencing camera and microphone, as well as necessary software. Information on specific technology is provided on the MRC Student Portal as well as the practicum syllabus. Web conferencing capabilities will be utilized for group clinical supervision.

FeesThe TTUHSC School of Allied Health Sciences requires a $50 graduation fee for students planning to graduate. As the practicum initiates the final phase of the degree program, the graduation fee will be assessed to students entering the practicum course.

Practicum / Internship Manual Updated 5/01/10

Administrative Procedures

Early in the practicum, students are expected to take care of the administrative process of setting up their clinical site.

Locating a Practicum Site – The first task is to locate a practicum site. The site should be one in which rehabilitation counseling services (serving people with disabilities) are provided. If you plan to engage in your internship at the same clinical site as the practicum, you will want to consider the match of the agency to your needs, interests, and preferences. Ideally practicum sites should be arranged before course starts however, the site must be prearranged before the 3rd week of class. All sites must be approved by faculty. You will benefit most from an agency with a broad range of clinical services.

Assessing the Potential Practicum and Internship Experience –The clinical site should provide as many clinical opportunities as is possible. The agency hosting the student should be willing to provide an onsite supervisor that will engage in weekly supervision. We encourage the student to work with as diverse a population as possible. Additional requirements of the internship site are spelled out in the internship contract.

Contracting with Practicum Site – Each student needs to inquire with their prospective practicum site about the needs of the agency. Students may need to complete a volunteer or practicum contract with the practicum site. Ask the practicum site what their needs are for paper work (e.g., volunteer contract, applications).

Affiliation Agreement –The TTUHSC Affiliation Agreement must be on file with the university before the student is allowed to deal directly with clients regardless of where you are employed. Once the practicum site is selected, contact your advisor to discuss and obtain approval for the practicum site. Once the site is approved, the student will provide our MRC Administrative Assistant, Faculty Supervisor, with the information she needs to make the arrangements for the agreement.

Joining a National Professional Organization -- Students are encouraged to join a professional organization, but it is not required. Professional organization membership is an important part of the process for professional identity development. It assists in the process of gaining additional skills and knowledge through continuing education, and staying current in the field. It facilitates the socialization process, and provides new professionals the opportunity to engage the profession at a national level. Most organizations have a reasonable student rate for membership. Students are encouraged to join a national rehabilitation counseling professional organization. Many professional organizations also provide additional liability insurance at discounted rates for students.

Practicum / Internship Manual Updated 5/01/10

Practicum Clinical Experience

Each student is required to spend at least 40 direct client contact hours (out of 100) of the practicum in a clinical setting. The purpose of the clinical experience in the practicum is to provide an opportunity for the student to get oriented to the clinical site, to observe the range of rehabilitation counseling services provided in agency settings, to begin to exercise clinical reasoning and intervention skills, and receive an introduction to the workings of a rehabilitation agency. Observation may be used for a portion of direct client contact hours with the amount of hours counted to be determined in consultation with your faculty supervisor.

Client Contact Hours Documentation:

Direct client contact hours are defined as time spent personally counseling or advising clients, testing or evaluating the client, interviewing, job development, “staffing” or group problem solving / planning with other professionals, transferability of skills analysis, performing a job analysis for the specific consumer, or any other service done directly with the client. Essentially, if the consumer is present count it as a contact hour.

In addition, time spent completing role plays can be counted up to 2 direct client contact hours, over the course of the entire semester.

Time spent in orientation, time on paperwork, file review, filing, time spent in learning administrative procedures, and time with your site supervisor does not count as a direct client contact hour. In other words, to meet the requirements of the Practicum course, this will require significantly more than a single 40-hour workweek to complete the practicum experience. If there are questions about what constitutes a clinical contact hour, contact one of the instructors of this course.

Expectations for Practicum Clinical SitesDuring the practicum, clinical sites will be expected to provide students the following activities:

1. Clinical Site Orientation (See Orientation Checklist).2. The site supervisor should be a CRC. If a CRC is unavailable, please contact your

practicum instructor. A copy of the site supervisor’s resume and their CRC certificate is required.

3. Provide meaningful activities (as described above) for the student to engage in during the required 40 hours of clinical experience for the practicum.

4. Provide student evaluation as outlined by the Rehabilitation Education Program. 5. Provide evaluation of the rehabilitation education program, and the working

relationship with the agency providing the practicum site.6. Maintain an open communication with the rehabilitation education program.7. Provide the following clinical experiences at a minimum:

a. The student will conduct intake interviews.b. The student will have an opportunity to interview and interact with

consumers.

Practicum Clinical Site Evaluation Procedures

Evaluation is an essential component of the practicum process. Evaluation is a multifaceted process, which involves all of the individuals involved in the practicum process. Students will be evaluated on their knowledge, skills, attitudes, and professional behavior. Students will also provide evaluations of supervisors and of the clinical site. Site supervisors will provide an evaluation of the student, as well as of the support provided by the MRC program. The following are the evaluations that must be completed and submitted in order for the student to receive a grade in the practicum:

1. Student Evaluation of the Placement and Supervision.2. Student Reflection Paper (details provided in the syllabus)3. Case Conceptualization 4. Student Evaluation of Faculty Supervisor.5. Student’s progress will be evaluated on an ongoing basis.

All evaluation forms are provided in the practicum forms section of the manual. Please make copies of all evaluation forms and retain for your records. All evaluations must be received by the last day of class (before finals begin) or credit will not be granted for the practicum, with the exception of the role-play evaluation forms which must be included with each tape.

Permission to Videotape/Audiotape Consent Form

The process of videotaping/audio taping counseling sessions plays a large role in the education of rehabilitation counselors. Such tapes assist in the evaluation process for student counselors. Additionally, the rights of participants are of utmost importance in this process. The following are program guidelines that the student counselor will follow to ensure the participant’s confidentiality.

1. Before videotaping/audio taping any participant, a signed release of confidentiality form must be signed and filed in the clinical chart, as well as with the MRC Clinical Director.

2. All recordings will be kept in a secure environment.3. All recordings will be erased or destroyed at the conclusion of the practicum or

internship.4. Videotapes/Audiotapes will be viewed by a course supervisor, and other students.

All reviewers of the material will be expected to maintain confidentiality about the material they see or hear.

5. Even if a consent form has been signed, participants have the option to stop or deny taping at any time.

6. Any concerns regarding this process or about consent of videotaping/audio taping activities may be addressed with the MRC Program at 1-806-743-4208.

Course OverviewYou should expect to participate in the following clinical experiences as part of your internship:

1. Orientation to the agency serving as the internship site, and the services they provide.

2. Orientation to the agency’s policies and procedures.3. Introduction to agency staff and personnel, and instruction on the roles and

functions they provide.4. Review of confidentiality procedures.5. Review of the CRC Code of Ethics.6. Continued Clinical Skill Development7. Weekly Clinical Supervision8. Responsibility for specific cases in the internship environment.9. Documentation, including:

a. Case File Maintenanceb. Agency Specific Documentationc. Internship Time Logsd. Activity Summariese. Weekly Progress Reviews

10. Clinical experience in all pertinent services rendered by the agency.11. Participation in agency staff meetings and rehabilitation team meetings.12. Evaluation Processes

Orientation The first five items listed above consist of the orientation process. If the internship site is the same as the practicum site, and these items were adequately covered during the practicum, then you may indicate so on the Internship Orientation Checklist. If these activities have not occurred within the internship site, then all activities must be addressed with the site supervisor.

Clinical Skill DevelopmentThe continued development of clinical skills is the primary focus of the internship experience. Students will be expected to continue to address competency with the site supervisor, as well as address continued skill development in the internship class. Internship activities that may facilitate continued clinical skill development may include videotaping counseling sessions, case review, case conceptualization, and other activities as determined by the clinical supervisor.

Practicum / Internship Manual Updated 5/01/10

Case AssignmentsEach student will be assigned, by site supervisor, primary responsibility for specific cases. While the total number of cases assigned to the student is up to the discretion of the site supervisor, the total number should not be less than three.

DocumentationStudents will be expected to learn and complete the necessary documentation for the clinical site where the internship is taking place. In addition to specific case documentation, students will be expected to complete the necessary documentation to receive credit for the internship. The documentation for course credit includes:

1. Affiliation Agreement 2. Internship Services Agreement Form (To be completed the first week of the

internship)3. Internship Time Log4. Documentation and assignments as assigned in the internship class5. Evaluation Forms6. A copy of the site supervisor’s resume and their CRC certificate is required.

Clinical ExperiencesInterns should have an opportunity to participate in as many clinical services as are offered at the internship site. This should be planned with the site supervisor.

Staff and Team ParticipationStudents will participate in the staff and rehabilitation team meetings that may occur in the clinical site. They should be given an opportunity to report on their specific cases, when appropriate, and be actively involved in the clinical discourse. Opportunities for asking questions and gathering additional information should be provided.

Administrative ProceduresIf the clinical site for internship is different than that used for the practicum, then the following administrative procedures listed in the practicum manual will need to be repeated. The steps include:

Locating an Internship site – The first task is to locate an internship site. The site should be one in which rehabilitation counseling services (serving people with disabilities) are provided. You will benefit most from an agency with a broad range of clinical services.

Assessing the Potential Practicum and/or Internship Experience –The clinical site should provide as many clinical opportunities as is possible. The agency hosting the student should be willing to provide an onsite supervisor that will engage in weekly supervision. We encourage the student to work with as diverse a population as possible. Additional requirements of the internship site are spelled out in the internship contract.

29

Page 30: Introductionsoahs1.ttuhsc.edu/mrc/documents/Practicum_Internship... · Web viewIntroduction to agency staff and personnel, and instruction on the roles and functions they provide.

Practicum / Internship Manual Updated 5/01/10

Contracting with Site – Each student needs to inquire with their prospective clinical site about the needs of the agency. Students may need to complete a volunteer or clinical contract with the clinical site. Ask the clinical site what their needs are for paper work (e.g., volunteer contract, applications).

Affiliation Agreement –The TTUHSC Affiliation Agreement must be on file with the university before the student is allowed to deal directly with clients regardless of where you are employed. Once the internship site is selected, contact your advisor to discuss and obtain approval for the internship site. Once the site is approved, the student will provide our MRC Administrative Assistant, and Faculty Supervisor, with the information she needs to make the arrangements for the agreement.

Joining a National Professional Organization - Students are encouraged to join a professional organization, but it is not required. Many professional organizations also provide additional liability insurance at discounted rates for students

Internship Evaluation ProcessEvaluation is an essential component of the internship process. Evaluation is a multifaceted process, which involves all of the individuals involved in the internship. Students will be evaluated on their knowledge, skills, attitudes, and professional behavior, and will provide evaluations of supervisors, and of the clinical site. Site supervisors will provide an evaluation of the student, as well as of the support provided by the MRC program. The following are the evaluations that must be completed and submitted in order for the student to receive a grade in the practicum:

1. Student Evaluation of the Placement and Supervision.2. Student Evaluation of Supervision by MRC Faculty3. Agency Supervisor Evaluation of Student.4. Student Internship Self-Evaluation.5. Site Supervisor Evaluation of the Rehabilitation Education Program6. Assignments completed as assigned in the internship course.

The student evaluation of placement and supervision, and the agency supervisor’s evaluation of the student will take place at mid-term, and at the end of the semester. A conference call including the student, site supervisor, and faculty representative will take place at mid-term, to review the first evaluation and address any concerns that may be evident. A conference call including the student, site supervisor, and faculty representative may also be needed at the end of the semester, to review the internship experience. Due dates for mid-term evaluations will be provided in the internship

syllabus. All other evaluations must be received by the last day of class (before finals begin), or credit will not be granted for the internship.

All evaluation forms are provided at the back of the internship manual. Please make copies of all evaluation forms and retain for your records.

Internship Clinical Supervision

Clinical supervision for the internship occurs in three formats, on-site, in class, and individual supervision. On-site supervision will be conducted with the designated site supervisor. In class and individual supervision will take place with the MRC faculty.

On-Site SupervisionOn-site supervision must be conducted on a weekly basis, for at least 1 hour. Supervision is provided by the designated site supervisor. The site supervisor should be a CRC. If this is not the case, then the faculty supervisor can serve as a CRC. There may be additional supervision requirements should a faculty member serve as a student’s CRC. Regardless of the status of the site supervisor as a CRC, there must be at least one hour of on-site supervision on a weekly basis.

Group SupervisionStudents will participate in group supervision activities with the internship class. In class supervision may consist of case presentations professional development, and discussion of specific issues that the students are facing in the internship sites. Specific assignments for in class supervision will be made in the Internship Syllabus.

Each intern will engage in 1 ½ hours of group clinical supervision each week with a MRC faculty supervisor and their classmates. This 1 ½ hours of group supervision will be in addition to the on-site supervision. Group supervision occurs online in a group atmosphere. Supervision times will need to be scheduled with the faculty supervisor. Should you experience technical difficulties that prohibit you from actively participating in weekly supervision sessions, it is the student’s responsibility to address these issues with Curt Finger, prior to the next supervision session. You must notify your faculty supervisor of the status and/or resolution of the issue. Failure to follow this protocol may result in an inability to complete the requirements for internship. If the technical issues cannot be resolved prior to the next supervision session, a separate supervision session must be scheduled with the faculty supervisor. Multiple absences from supervision sessions are unacceptable and will result in a one on one meeting with the faculty supervisor, and possibly program director, to determine the appropriate action.

Practicum / Internship Manual Updated 5/01/10

Physical Oversight/Supervision

Any student who has more than 33% of their clinical time without physical oversight must adhere to the following additional requirements:

1. Extra supervision from site supervisor-students need an average ½ hour of supervision from their site supervisor for every 10 hrs/week working in the clinical. Minimum amount per week is 1 hour.

2. A weekly log of their activities must be turned into the faculty supervisor every week for the duration of the clinical.

3. Faculty must have a meeting with the site supervisor prior to site approval to explain the additional requirements.

Technology RequirementsFor the provision of clinical supervision in the internship, students will need to obtain a web conferencing camera, headphones with microphone, as well as necessary software. Information on specific technology will be provided in the internship syllabus. Web conferencing capabilities will be utilized in group, and at times one on one clinical supervision with the MRC faculty supervisor. As this technology should have been obtained for the practicum, students should already possess it.
