OPT Request & Recommendation Form

Form I-765 : Please visit Form I-765 link, review the instructions, and verify the fee (click on Filing Fee) prior to filing. As of 12/23/2016, the fee is $410 and is subject to change. When completing the I-765, you must use the ISSC’s address if applying for post-completion OPT. Fill out I-765 as shown below:

If applying for Pre-completion OPT, indicate the appropriate code by writing (c) (3) (A) If applying for Post-completion OPT, indicate the appropriate code by writing (c) (3) (B)

Check or money order for the I-765 OPT application fee. Please use the pictures of the check and money order below for guidance as to how to fill out. Not all checks are the same, and money Orders will look differently.

Two (2) US passport style photos with very specific measurements. Specifications for the size of the photo and other details can be found on page 20 of the Instructions for Form I-765. Print your name and I-94 number on the back of each photo.

Please be sure you have copies of all the following documents before uploading via the eform. Failure to do so will result in a delay in processing. DO NOT INCLUDE THE SEVIS 1-901 FEE PAYMENT FORM.

Copies of all SEVIS I-20s, back and front. Not the instruction page. Copy of biographical page of current valid passport - just the page with your picture on it. Copy of current visa Copy of most recent I-94 Copy of any previous Employment Authorization Documents (EAD cards)

For email notification of receipt of your application by the Department of Homeland Security: Form G-1145 : Although not required, this form, if submitted with the application, will enable you to get an

email/text notification that your application has arrived at the DHS Texas Lock Box address.

According to new guidance from the U.S.C.I.S., you can now receive your I-20 either electronically or hard copy. Please note If you request to have your OPT I-20 electronically sent to you, YOU will be responsible to have your own large envelope to send the packet. You can obtain the certified mail forms at the USPS.

SEVIS I-20 with OPT request: This will be created after you upload your completed OPT application with all materials on this checklist into the eform. When you receive your I-20, be sure to keep the original I-20. Make a copy of the new I-20 and include it with the application. Along with your OPT I-20 you will receive a white mailing envelope addressed to the USCIS, United States Postal Service (USPS) certified mailing forms, a small glassine envelope for your pictures. On the green certified mailing form, you must add your name as shown here in red:
