Patients who receive stress ulcer prophylaxis suffer more ad-verse events with proton pump inhibitors (PPI) than with hista-mine 2 (H2) receptor antagonists, a new study shows.

Critically ill patients can develop bleeding caused by stressulceration, so clinicians often prescribe a PPI or H2 receptorantagonist as prophylaxis. But these medications come with arisk for adverse events .

The researchers looked at the records of patients admitted toa 54-bed medical-surgical intensive care unit (ICU) from July2009 to February 2013.Of the 14,280 patients who receivedstress ulcer prophylaxis, 5718 were excluded because they wereon acid suppression prior to admission, did not receive stressulcer prophylaxis in the ICU,or received both a PPI andH2 receptor antagonist.Of theremaining 8562 patients, 3681(43%) received a PPI and 4881(57%) received an H2 receptorantagonist.

Proton Pump Inhibitor Riskier Than H2 for Stress Ulcer ProphylaxisJibitha Mamachan II-Mpharm

The difference in the size of the 2 treatment groups wasstatistically significant (P <  .0001). However, the researchersfound that stress ulcer prophylaxis was indicated in only 32.3%of patients in the PPI group and 37.8% of patients in theH2 receptor antagonist group (P < .0001). Indications for thistherapy include duodenal ulcer, gastric ulcer, Helicobacter pyloriinfection, erosive esophagitis, mechanical ventilation, coagulopathy, and gastric reflux disease.

The increased risk for adverse events in the PPI group wasalso significant.
