A Centre of ExcellenceL V Prasad Eye Institute (LVPEI), located in Hyderabad, India, is a world-class eye institute, encompassing clinical care, education, research, rehabilitation and high-impact rural eye care. Since its establishment in 1987, LVPEI has grown into a Centre of Excellence in the field of eye health. Today, LVPEI is a World Health Organization Collaborating Centre for prevention of blindness and a pioneer in developing eye health models for underserved areas of the developing world. The Institute is governed by two registered trusts: the Hyderabad Eye Institute and the Hyderabad Eye Research Foundation. While the Hyderabad Eye Institute focuses on service delivery, all research programmes are carried out under the aegis of the Hyderabad Eye Research Foundation.

The Hyderabad campus of LVPEI, a Centre of Excellence named the Kallam Anji Reddy Campus in recognition of one of our largest donors, is complemented by 3 tertiary centres – the Bhubaneswar campus, GMR Varalakshmi Campus in Visakhapatnam and the Kode Venkatadri Chowdary campus in Vijayawada – all offering comprehensive, superspecialty eye care to all those in need. LVPEI’s expertise now extends further into communities with a network of over 109 secondary and primary care centres that cover the remotest rural areas in the state of Andhra Pradesh, India and will soon spread into rural Orissa.

The Institute’s coordinated and interlinked functions of patient care, training, research, rehabilitation, community eye care and product development serve a wide population of persons in need of eye care services and a large network of practitioners and students. A web of national and international partnerships provides a solid framework of support. At the core of all our efforts are these desiderata: efficiency, equity and excellence.

India is a study in contrasts. The most sophisticated systems coupled with the ability to pay for them and more. However, a large percentage of India’s 20 million vision impaired can neither access nor afford such systems. The loss of productivity due to blindness or visual impairment affects the poor at a higher level of intensity. At LVPEI, a staggered system of cross-subsidy allows us to offer free high-quality care to the economically underprivileged, who form more than 50 percent of our patient base.

Our Single-point FocusExcellence in Patient CareThe moment a patient steps through our doors, a system swings into action. From comprehensive documentation to a complete eye examination, followed by specialist attention where needed, our attention is fixed on the individual who needs care. The examination rooms are equipped to handle all subspecialties, which means the patient stays in one place and the specialists come to see him.

LVPEI’s comprehensive patient care system caters to over 2500 outpatients and handles over 200 surgeries every working day. The infrastructure comprises close to 300 examination rooms, diagnostic and therapeutic chambers, operation rooms with state-of-the-art equipment and other ancillary services, across the network. LVPEI’s diagnostic infrastructure includes the LVP-Zeiss International Academy for Advanced Ophthalmic Education, a state-of-the-art centre for diagnostics using computerised imaging systems. All patients – paying and non-paying – access these facilities without any discrimination.

Clinical practice draws continually from understanding gained through applied and basic research on eye dis-ease, and in turn feeds back into studies insights from patient care. The close cooperation between clinical and basic scientists is what gives eye care at LVPEI the added edge of new knowledge.

Eye BankCorneal blindness is one of the most common forms of blindness. The only known cure for corneal blindness is corneal transplantation, using healthy corneas donated by individuals upon their death. The demand for donor corneas far exceeds the supply, and an active eye bank-ing movement is the only way this problem can be ad-dressed.

LVPEI set up the Ramayamma International Eye Bank (RIEB) in 1989. The RIEB is a member of the Inter-national Federation of Eye and Tissue Banks (IFETB), USA, and the Eye Bank Association of India (EBAI). It is now the nodal eye bank for the whole of South East Asia, particularly the Indian sub-continent. Apart from harvesting and distributing corneas, the Eye Bank also runs training programmes for eye bank technicians and grief counsellors and is considered an international train-ing and resource centre. Eyesight International (ESI), Canada, supported the setting up of RIEB, a Center of Excellence for Eye Banking. ESI has also supported the eye banks at LVPEI’s tertiary centres in Bhubaneswar and Visakhapatnam.

“Special thanks to you”My son was operated upon by you when he was barely 11 months old, way back in 1988-89, and continue to be under the able care of your doctors. Now 21, he stood first in his bachelor’s program in physics in Mumbai University. He is now pursuing a Master’s course in astrophysics at Bonn University, Germany.

Bringing Vision to LifeIncurable is not the end of the road at LVPEI. Patients who come to us with a visual impairment that cannot be treated or cured are referred to our low vision and rehabilitation centres. Here, they receive support to live a reasonably productive and independent life, through a combination of skills training and prescription of low-vision aids. Our engagements with patients are often long and involved – but every time, they have resulted in a marked enhance-ment of the patient’s quality of life.

LVPEI is unique in having a low vision and rehabilitation facility as an integral part of its patient care services. Here too, the focus is on finding solutions that fit the patient’s need and budget, with low-cost locally developed aids and techniques.

The accumulated experience of two decades of work in Rehabilitation and Low Vision is now being shared with professionals in the area through a new long-term fellow-ship program, apart from several short-term programs for special educators, rehabilitation professionals and community workers.

Low vision services are available at all levels of eye care in the LVPEI pyramid – ranging from the simplest at primary care levels to the most advanced at the tertiary levels and the Centre of Excellence.

Enabling AccessIn 1996, Lighthouse International reorganized its interna-tional programs to ensure that its projects would more effectively address vision rehabilitation on a global scale. L V Prasad Eye Institute (LVPEI) stood out as a potential ideal partner to work with us to build a program to address the low vision needs in southern India and throughout the region.

The LVPEI-Lighthouse partnership has enabled expanding numbers of people with low vision to receive the care that they need to function effectively in family, career, and com-munity life. The partnership has done this through enhanced direct services to the patients of LVPEI and by enabling professionals from other institutions and those working throughout the region to develop clinical low vision care and vision rehabilitation skills to expand their services. The pro-grams and networks established are expanding while staying true to the commitment to quality and sustainability.

We have learned a great deal from working with the staff of LVPEI and also taken pleasure in seeing what we have been able to share reflected in the work that LVPEI is doing.

IT Resource CentreHarnessing new tools that can extend the ability of the partially sighted is an ongoing effort at LVPEI’s Rehabilitation Centres. The IT Resource Centre for the Visually Impaired is the first such resource bank in the country, with materials ranging from magnify-ing software such as ZOOM to reading software that converts text from different sources to audio, and a growing Braille library.

The Mahindra Satyam Foundation (MSF) supports 2 centres, offering specialised services at the Hyderabad campus: ‘Program for Early Intervention in Children’ and ‘IT Training Centre for Visually Impaired’.

Early Intervention Goes a Long WayDoctors and rehabilitation professionals at LVPEI take a team approach to the diagnosis and care of visual impairment in very young children. Our early inter-vention programme networks closely with neonatal wards and communities in sensitising medical practi-tioners and parents about developmental problems that could lead to vision loss, such as retinopathy of prematurity and developmental glaucoma.

This has been developed in collaboration with Bartimeus of The Netherlands.

No Limits for SaidaSaida was born with albinism, a pigmentation disor-der that robs the skin of its color and affects vision. Albinism causes reduced visual acuity and extreme sensitivity to bright light. His parents were farmers in a small village and had never been to school; but they struggled to give him an education. Eager to learn, Saida completed school by standing next to the chalkboard.

When he came to LVPEI in 1997, Saida underwent a detailed eye examination. His vision could not be recovered, but surgery stopped the shaking of his pupils (nystagmus). He was referred to the Institute’s Vision Rehabilitation Centres, where he was pre-scribed glasses for short and long distance, as well as a telescope to help read bus numbers. The glasses and telescope were given free of cost, enabled by the generosity of a long-time Indian donor based in the US, who supports the provision of low vision devices to underprivileged patients.

Twelve years later, Saida had a commendable aca-demic track record. After his father’s early demise, Saida’s mother continued to support his education. On graduating from college, Saida pursued a Master’s in Business Administration program in his district, working as faculty in the same college that he studied in. He topped the university and took up a job as a team leader in a premier Indian bank, leading a group of 18 youngsters.

Yet Saida was not content. He studied hard and qualified for a senior managerial position at India’s leading bank. The job promises him assured income and recognition. Today he is a confident young man, sharing an excellent rapport with his colleagues. He is also a good cook and singer, performing at religious events regularly.

Extending ExpertiseWe believe that expertise and experience must be generative. Worldwide, there is a huge shortfall of trained medical personnel, and the problem of blindness and visual impairment can be contained only if there are enough trained people in the right places. As a WHO Collaborating Centre for prevention of blindness and a major resource for the VISION 2020: The Right to Sight global initiative to eliminate avoidable blindness, LVPEI plays a significant role in addressing this need. The offering spans continuing education programmes designed to update the skills of practitioners, to fellowships in various subspecialties, to college-level programmes in optometry and ophthalmic techniques and programmes for mid-level ophthalmic personnel, including technicians, nurses and eye care management staff.

The ophthalmology training programmes are designed to help fellows develop critical clinical skills, perform effective diagnostic evaluation, and acquire the latest surgical techniques and an excellent patient care approach. More than 14,000 trainees from all across India and a significant number from overseas have participated in the Institute’s training programmes. Training takes place across all three tertiary centres, making use of video conferencing technology to create a single seamless learning environment.

In 2009, LVPEI launched a postgraduate training program in ophthalmology affiliated to the Diplomate of the National Board.

Exchanging ExpertiseThe L V Prasad Eye Institute (LVPEI) and the University of Rochester Eye Institute (UREI) have developed a comprehensive cooperative agreement of mutual benefit in education and research.

As testimony to the tremendous international prestige of the LVPEI, our relationship has become a major draw for the UREI residency training program, which now offers, as a regular part of its program, a month of elective to residents in ophthalmology. This elective introduces these trainees to issues surrounding delivery of ophthalmology care in India, to increase their exposure to diagnosis and management of diseases not regularly seen in the United States, and to learn advanced surgical techniques developed or perfected by the world-renowned LVPEI faculty.

We are committed to a long-term collaboration with LVPEI that will become the model for partnerships in international ophthalmology.

Knowledge Sharing in Vision ScienceThe Global Leaders Lecture Series, launched by the Education Centre in 2008, brings global expertise to LVPEI’s learning community through videoconferencing. Leaders in vision science and eye health practice are invited to share their ideas and knowledge in regular interactions that span the three tertiary campuses. Videoconferencing is also put to good use in making routine lectures and continuing education programs accessible to all the campuses.

The Bausch & Lomb School of Optometry and the LVP-Zeiss International Academy for Advanced Ophthalmic Education form part of our training infrastructure. The Ramayamma International Eye Bank of LVPEI also offers courses in eye banking techniques in collaboration with Eyesight International and ORBIS International. In con-ducting our training programmes, we collaborate globally with institutions like the Case Western Reserve University and University of Wisconsin in USA, and University of New South Wales, Australia.

Beyond the BasicsAll research at LVPEI is carried out under the aegis of the Hyderabad Eye Research Foundation. The future of any medical specialty lies in the hands of those who push the limits of the known and the possible, to develop new treat-ments, new procedures, and new ways of doing and seeing. From investigating the genetic and molecular basis of eye diseases, to arriving at creative modes of managing vision-threatening conditions, our scientists work at the cutting edge of eye research. Clinical faculty too are actively engaged in research, translating experience into knowledge through extensive publication and presentation. LVPEI is also involved in a number of clinical trials that investigate the safety and efficacy of new drugs and devices.Many faculty members have been recognised by professional bodies such as the American Academy of Ophthalmology for continued excellence in education as well as clinical and basic research.

Research at LVPEI has been funded by competitive grants from national and international funding bodies, including the Government of India’s Council of Scientific and Industrial Research, the Indian Medical Research Council, the Gov-ernment of India’s Department of Biotechnology, and the United States National Eye Institutes and National Institutes of Health, among others.

Efficiency in ActionWe chose to support L V Prasad Eye Institute based on what we think are three critical success factors:1. The focus is on providing a high level of eye health care

to the poorest of the poor.2. We view the operation of LVPEI as being run strategically

and efficiently, much the same way a successful business would be run.

3. The research and education components of LVPEI’s work help modify processes and procedures to suit lo-cal needs so that this research can be used to educate care givers and the client base on how to live healthier lives.

Recently we decided to support groundbreaking research being done at LVPEI on Stem Cells and their ability to cure certain forms of blindness. LVPEI’s work in this area further demonstrates to us that it is at the forefront of eye health care and its efforts continue to benefit tens of thousands of patients annually who would otherwise suffer needlessly and be unable to lead productive lives.

LVPEI and UsWe work with LV Prasad Eye Institute because, of all the eye care institutions in the world, they probably have the best balance of research, education and caring for those in need.

We work closely with the Institute in test-ing new vision correction and care sys-tems. LVPEI has become a world premier site for the performance of clinical trials, and our collaboration has contributed to new understanding of the eye, and to the development of new methods of vision correction that can bring clear sight to thousands of people around the world.

We have gained considerable scientific understanding and skills from our col-laboration with LVPEI. Stem Cell Research at LVPEIResearch at L V Prasad Eye Institute is geared toward finding new ways of managing and overcoming eye disease. One of the major causes of blindness worldwide is corneal disease or damage, for which corneal transplantation is the only cure. The availability of donor corneas for transplantation falls far short of the need. A revolutionary approach that is now being studied – and put into practice – involves the generation of reparative tissue in the labora-tory, and using this to replace damaged or diseased tissue. Stem cells, or undifferentiated cells from a donor eye, are cultured in a special medium to produce corneal epithelium that can then be transplanted onto the eye of a person to treat corneal blindness. LVPEI is one of a handful of institutions across the world engaged in this frontline area of research and practice. Now this research is entering a new phase at LVPEI; scientists are working to push adult stem cells to transdifferentiate into cells of neuronal lineage. This work holds great promise for another area of eye care – retinal disease – that at this point has no cure.

Translational Research:From Lab to Clinic and BackFinding answers to the problems in vision care and arriving at an understanding of eye function that has direct bearing on treatment approaches is what drives research at LVPEI. The Champalimaud Translational Centre for Eye Research (C-TRACER), set up in collaboration with the Champalimaud Foundation of Portugal, is an advanced facility that will provide an opportunity for the exchange of ideas and expertise between the two countries. The Centre focuses on the following:

Beyond our WallsMaking eye care accessible is the primary mission of LVPEI. On a global or even national scale, this can be done only if a successful model is created and replicated in areas where it is most needed. The International Centre for Advancement of Rural Eye care (ICARE) is at the core of our outreach efforts. We train various cadres of eye care professionals who take their expertise to serve populations in the remotest rural areas, and also in underprivileged urban areas. Our Eye Health Pyramid model of eye care delivery, which is a successful attempt at making the most of local resources for providing quality eye care, has already taken root in several rural areas of India. ICARE has established 10 secondary eye care Service Centres and 88 primary care Vision Centres in Andhra Pradesh and oversees the functioning of several affiliate centres in the state, providing management, medical and technical consultancy.

The LVPEI Service Centres in the districts of Andhra Pradesh have been functioning as effective secondary care centres, operating at the same level of excellence as the tertiary centre in Hyderabad. Further, the Vision Centres offer primary eye care to clusters of villages, ensuring that people do not have to travel more than a few miles to access high quality care. At an even more local level, Vision Technicians and Vision Guardians (trained community workers) work in villages to ensure the reach of quality eye care services.

A group of 10 Vision Centres attached to a secondary centre, along with all the associated community programs, is known as a Village Vision Complex. The Village Vision Complex, through hospital based and community services, can address nearly 75% of avoidable vision impairment.

ICARE also conducts epidemiological research to establish accu-rate baseline data on blindness and visual impairment in different communities. This research forms the basis for policymaking, thus becoming an important advocacy tool in blindness preven-tion efforts. The Andhra Pradesh Eye Disease Study, completed in 1999, was one of the first such studies in the country, provid-ing crucial inputs for planning eye care services.

Pyramid Model of Eye Care Delivery — Objectives

Vision Centres can eliminate 25% of all blindness •due to the need for spectacles, detect potentially blinding problems, address conditions like Vitamin A deficiency, cataract, some glaucomas and diabetic retinopathy.

Service Centres can manage 75% of all avoidable •blindness.

Thus only 25% of people with visual impairment need •to travel to cities to access tertiary care.

LVPEI’s pyramidal approach to sustainable eye care has been adopted as a model by the Indian government and the World Health Organization, and is being successfully replicated in other Indian states and across the developing world.

A Privileged PartnershipOperation Eyesight chose the L V Prasad Eye Institute as its Global Training and Resource Centre because of its international reputation for uncompromising qual-ity and its innovative approach to sustainable models of comprehensive eye care. We were looking for a partner in the developing world that could deliver WHO standards of eye care, demonstrate financial sustainability within a comprehensive model of care and make these eye care services available to anyone regardless of their ability to pay. At LVP even the most complex and costly procedures are provided free of charge to poor patients. The LVP staff and consultants have worked closely with Operation Eyesight and its hospital partners to improve every aspect of their performance. With LVP’s expertise in designing more efficient and attractive facilities, reorganizing operat-ing systems and training to upgrade technical skills our partners have doubled and in some cases, tripled the number of patients treated since 2005 and doubled their revenues. The partnership with LVP is helping us to establish enduring, quality eye care wherever we work in the developing world.
