    A.Form DS-2019

    What address/primary site of activity do I use for my Form DS-160?

    Your Primary Site of Activity on your current Form DS-2019 is IIE (809 UN Plaza, New York, NY 10017). This

    is just for your visa interview on the week of May 19th

    as it will be updated at a later date. For your Form

    DS-160 please use the address of your U.S. host institution listed on your Terms of Appointment.

    How do I access my Form DS-2019?

    You can only access a COPY of your Form DS-2019 electronically. You will receive the original Form DS-

    2019 during the visa week that you are scheduled for.

    By now, you should know how to access your Grantee Portal. Please log in to your portal account

    (http://mycusthelp.info/IIE), go to Programs, and click on your IIE ID number (as stated on your most

    recent Terms of Appointment), and download your DS-2019 COPY.

    Why are the start and end dates on my Form DS-2019 different than the start and end dates on my

    Terms of Appointment?

    Again, the purpose of this specific Form DS-2019 is for your visa interview on the week of May 19th

    and for

    you to enter the United States. We will amend your DS-2019 so that it will match your current Terms of

    Appointment (TOA) while you are in the U.S. For now, you should plan your arrival based on the dates on

    your TOA.

    What is the Exchange Visitor Program Number?

    You will need this number to apply for your J-1 Visa. The Program Number for your program is

    P-3-00006. This number is the same for every BSMP student who has visa sponsorship through IIE.

    What is my SEVIS Number?

    This number is found at the top right hand corner of the Form DS-2019 and starts with letter N followed

    by 10 (ten) digits. This number is unique for each Exchange Visitor.

    When will I receive my original Form DS-2019?

    You must pick up your ORGINAL Form DS-2019 at the address below for the location you selected in your

    Terms of Appointment. You must have your original Form DS-2019 prior to attending your J-1 visa

    interview at the U.S. Consulate or Embassy. You must adhere to the schedule below. No exceptions or

    accommodations will be made. Students with the first names that begin with the letters A through J, can

    only go during the hours of 8AM-12PM at the pickup locations below during those specific times. Students

    with the first names that begin with the letters K through Z, should only go during the hours of 1PM-4PM.

    Example: A student whose name is Eduardo da Silvawould go to pick up his DS-2019 between 8AM-12PM. A student whose name is Rodrigo de Souzawould go to pick up his DS-2019 between 1PM-4PM.

    Please understand that there is limited space availability in some of the centers below and that this

    proposed schedule is meant to ensure an efficient process in the distribution of Forms DS-2019 for

    hundreds of BSMP students.

    How early can I arrive at my U.S. host institution?

    All J-1 exchange visitors are allowed to enter the U.S. up to 30 days prior to the start date on the Form DS-

    2019 but no earlier than the J-1 visa issue date. We strongly recommend that you make sure that you

    have insurance coverage if you intend on arriving before your program start date. Please see our website

    B.Biometric Appointment (CASV) and J-1 Visa Interview

    Do I need to make an appointment for my biometric information appointment and J-1 visa interview?

    Your BSMP Group is required to schedule the biometric appointment for the week of May 19th

    What should I bring with me to my J-1 visa interview?

    You arerequiredto have your ORIGINAL Form DS-2019for your J-1 visa interview. You must alsobring

    the following with you:

    ORIGINAL current passport

    Application for Non-immigrant Visa Form DS-160

    SEVIS I-901 Fee Payment Receipt

    Any previous U.S. visas in your current or expired passport or a copy

    In addition to the above, we recommend that you bring:

    Who is responsible for paying for and arranging my travels to my biometric information appointment

    and J-1 visa interview?

    BSMP students are fully responsible for paying for and arranging travels to and from Visa Days.

    Who will pay the SEVIS I-901 fee and Visa Application Fee (MRV)?

    Neither IIE nor CAPES/CNPq is responsible for paying the above fees. BSMP students must pay for the

    US$160 Visa Application fee (MRV) and the US$180 SEVIS I-901 fee with personal funds.

    On the Form DS-160, I am asked about the Person/Entity paying for my trip. What should I select? Is the

    entity CAPES, IIE or my Host Institution?

    Please select OTHER COMPANY/ORGANIZATION. CAPESis the Entity that is paying for your trip.

    I made an error on my Form DS-160 after I have submitted it. What should I do?

    Can I bring personal items to the Consulate, such as a cell phone and backpack?

    As per the instructions stated on the Brazil U.S. Embassy website, for security reasons, visa applicants are

    not allowed to enter the Consulates or the Embassy grounds with bags (with the exception of one smal l

    C.J-1 Visa

    If I already have a Tourist Visa (B-1/B-2), can I use it to enter the U.S.?

    No. Under no circumstances should you apply for or enter the U.S. with a B-1/B-2 visa or any other type of

    visa. You must enter the U.S. with a J-1 visa.

    Will I get a multiple entry J-1 visa?

    The decision of whether you are granted a multiple entry visa is at the discretion of the U.S. Visa Consular

    Officer who will interview you.

    My J-1 visa expires earlier than the end date on my Form DS-2019 or Terms of Appointment. What

    should I do?

    Should your visa expire while you are in the U.S., you will not need to renew it as long as you have a valid

    Form DS-2019 and do not plan on traveling outside the U.S. or its territories. Please note that your J-1 visa

    start and end dates are not determined by IIE, but rather by the U.S. Visa Consular Officer.

    D.Miscellaneous

    Do I have to bring health records with me on visa day?

    While the U.S. Consulate may not require documentation regarding your health or immunizations, you

    should be prepared to provide this information to your U.S. host institution. Many institutions require

    proof of immunizations prior to course registration. If you have any pre-existing conditions, you must

    inform IIE so we can make sure you have appropriate health insurance coverage and assistance. A pre-

    existing condition is any condition which (i) originated prior to your effective date of coverage or (ii) you

    received consultation/treatment/medication from a physician about prior to your effective date of

    coverage. It is recommended that you have all necessary dental care or immunizations completed prior to

    your departure.

    I paid the SEVIS I-901 fee when I applied for an F-1 visa. Do I have to pay the SEVIS I-901 fee again?

    Who is paying for my travels to the United States?

    As part of your BSMP scholarship, CAPES/CNPq will provide you with funds for your transportation from

    Brazil to your host instituion and your return to Brazil after the completion of your authorized program. If

    you have any questions regarding funding or arranging travel please contact your representative at

    CAPES/CNPq.
