1. Statement of Purpose Our Statement of Purpose outlines the care and support we provide, how we are organized to provide that care and support and includes a general overview of the facilities, services and practices adopted to implement the effective delivery of meeting our stated purpose.

The Statement of Purpose is to be read and applied in conjunction with our wider policies and procedures.

This Statement of Purpose is available to all existing and new prospective service users, and other interested parties with an interest in our provision of care and support.

A copy of the Statement of Purpose is held by The Regulation and Improvement Authority (RQIA).

Whenever any part of the Statement of Purpose changes, we will ensure that it is updated within 28 days of that change. The RQIA will be advised of these changes.

2. Introduction to Niamh Niamh (the Northern Ireland Association for Mental Health), is the largest and longest established independent charity focusing on mental health and wellbeing services in Northern Ireland. Niamh is structured as a group consisting of four elements, Compass, Beacon, Inspire and Carecall.

3. Introduction to Inspire Wellbeing Inspire provides care and support for people with a learning disability and mental ill health in five locations in Northern Ireland.

4. Philosophy of Care and Support We aim to provide consistently high standards of professional care and support to all those who live at 28 Lime Court, Peters Hill ensuring that service users can have the same opportunities that exist for the rest of the population. We also aim to ensure that

5. Introduction to 28 Lime Court, Peters Hill 28 Lime Court, Peters Hill is a purpose built building comprising of 14 self-contained flats. 13 flats are for service users and 1 flat for staff office accommodation. The scheme will support 13 service users with either a learning disability and or mental ill health. The scheme is funded to provide day opportunities to the service users based on their assessed needs. The categories of care at 28 Lime Court, Peters Hill are Adult Mental Health and Adult Learning Disability.

9. Quality Assurance

Niamh strives to continually maintain and improve on the requirements of the following quality assurance standards.

The Northern Ireland Housing Executive’s (N.I.H.E) Quality Assessment Framework standards have been adopted and implemented within all supported housing schemes.

Domiciliary Care Agencies Regulations (2007) NI.

Niamh also works within additional quality assurance frameworks and has achieved the following accreditations:-

Ireland Excellence Award 2014 Investing in Volunteers Award, 2011 Investors in People Award, Gold 2013

A comprehensive essential training programme is in place for Niamh staff and volunteers, including an induction programme and probation period.

Area Manager x 1 – Kelly Devlin – 39 Hours

Kelly has been employed with Niamh since October 2014. Kelly has achieved BSc (Hons) Sociology – 2:2 and obtained the Q.C.F Level 5 Diploma for Leadership and Management in Social Care in 2012. Kelly has experience as a Manager for Praxis Care from 2012-2014 and as a Team Leader for Praxis Care from 2005-2012. Prior to this Kelly was employed as a Support Worker for Praxis Care from 2005.

Deputy Area Manager X 1 – 32 Hours

Deputy Area Manager has been employed with Niamh since March 2014. Claire has a Bachelor of Social Work – 2:1 in 2013 and a BSc Psychology 2:1 in 2007. Claire worked as a Team Leader or Niamh for two years. Prior to working for Niamh, Claire was a Support Worker for Praxis Care from 2009-2014.

Team Leader 4 has been employed with Niamh since July 2014. Support Worker 3 has been employed with Niamh since April 2014. Support Worker 3 has achieved an H.N.D in Childcare. Support Worker 3 has 20 + years as a support worker, working with people with a learning disability.

Support Worker 4 – 39 Hours

Support Worker 4 has been employed with Niamh since October 2014. Support Worker 4 has achieved a HND Level 5 H&SC. Support Worker 4 has 4 years experience working in the Mental Health program of care.  These 4 years were spent working with MindWise (New Vision) and feel that it has developed skills in working with individuals who experience severe mental health difficulties. 

Support Worker 5 – 24 Hours

Support Worker 4 has been employed with Niamh since April 2014. Support Worker 5 has 8 years experience as a Relief Support Worker for Positive Futures and 2 years experience as an Auxiliary Nurse.

Support Worker 6 – 24 Hours

Support Worker 6 has been employed with Niamh since May 2014. Support Worker 6 has achieved a BSc Psychology. Support Worker 7 has previously worked as a Support Worker for Praxis Care and continues to work relief shifts with Praxis Care.

Support Worker 7 has been employed with Niamh since May 2014. Support Worker 7 has achieved H.S.C – Adult, Aging and the Life Course, Promoting Public Health (H.S.C), B.TEC National Diploma in Health Studies. Support Worker 7 has previously worked as a Support Worker supporting vulnerable young adults with self harm and learning disabilities. Support Worker 7 has also volunteered with the Rainbow Project.

Support Worker 8 – 39 Hours

Support Worker 8 has been employed with Niamh since June 2014.

Support Worker 9 – 24 Hours

Support Worker 9 has been employed with Niamh since April 2014. Support Worker 9 previously worked for 2 years at the Morning Star Hostel in Belfast.

Support Worker 10 has been employed with Niamh since April 2014. Support Worker 10 has 10 years experience of working with children as a child carer, nursery assistant and child-minding roles.

Support Worker 11 – 39 Hours

Support Worker 11 has been employed with Niamh since March 2014. Support Worker 11 has achieved Q.C.F Level 1 Adult Induction into H.S.C, Diploma Level 2 Induction into Health and Social Care. Support Worker 11 during their student placement worked as an Accommodation and Support Worker.

Relief Support Worker 1 has been employed with Niamh since April 2014. Relief Support Worker 1 has achieved a BTEC, H.N.D and National Diploma. Relief Support Worker 1 has previously volunteered for 7 years through the Gateway Club and various care homes.

Relief Support Worker 2

Relief Support Worker 2 has been employed with Niamh since July 2014. Relief Support Worker 2 has achieved LLB Hons Law. Relief Support Worker 2 is currently studying Social Work. Relief Support Worker previously worked as a classroom assistant and 2 years experience a Homeless Support Officer.

Relief Support Worker 3

Relief Support Worker 3 has been employed with Niamh since March 2014. Relief Support Worker 3 has achieved Level 4 H.N.C – Health and Social Care and BA Hons English Language and Literature. Relief Support Worker 3 has previously taught English as foreign language and experience as a Volunteer Outreach Worker.

11. Aims and Objectives Supported Housing Schemes are designed to offer another option in the spectrum of community care, catering for people who have a history of enduring mental illness and or learning disability. The overall aims of the Supported Housing Schemes are:

To provide supported accommodation while offering the opportunity of optimum levels of integration and independence.

To provide accommodation which enables service users to attain their full potential (and strive for independent living).

To contribute to the reduction of hospital admissions and the length of admissions.

The aims will be achieved through the following objectives:

To provide support as appropriate to the level of need of each service user.

To target and maximise the abilities and skills of the service users while recognising the need to give help and support.

To encourage the service users to take responsibility in household tasks and develop routines in day-to-day life. Also to encourage service users to initiate new activities in daily living, provide a degree of variety and a range of choice.

To give service users the opportunity to live in mixed facilities that reflect each person’s individual choice and needs.

To ensure that service users have every opportunity to exercise choice in all aspects of daily living. Levels of choice and responsibility must be realistic, therapeutic and within the range of individual abilities.

To ensure that the facilities reflect each person’s individual choice and needs and those service users are encouraged to have their own personal possessions within the limits of the scheme.

To ensure that tenancy is available to all people from all ethnic backgrounds and religious persuasions

To ensure that the staff are trained to display attitudes which are in accordance with the Niamh philosophy.

To encourage service users to develop appropriate links within the community and to utilise relevant community facilities

The overall goal is to seek gradual and moderate improvement in levels of functioning. Such improvement, however, will still necessitate varieties of support to maintain the service users in the scheme.

12. Therapeutic Process In accordance with the overall principles of the scheme, the staff should encourage an environment which provides support, friendliness, care, appropriate independence etc. within a homely environment. The goal is to enhance the quality of the service user’s life and to:

maintain or improve self-care maintain or improve daily living skills maintain or improve social integration

maintain a structure to their day maintain or improve community integration encourage service users to take on the optimum level of

responsibility in as many life domains as possible encourage participation by the service users of the planning

and implementation of their individual support plan

13. Responsibilities Please refer to the relevant responsibilities that are included in the service agreement/contract. These would normally include:

14. Referral Groups People considered suitable for referral are:

1. Aged 18 or over;2. Those with a diagnosed form of either a learning disability

and/or mental ill health and would benefit from living in a supported housing scheme;

3. A desire to live in a supported housing scheme;4. A history of long term hospitalisation, a number of admissions

to hospital or a risk of becoming institutionalised. 5. When the socially disabling aspects of the illness become the

primary concern i.e. daily living/social skills and the person is unable to cope with everyday problems of life;- Are unable to self-medicate- Are unable to manage their finances- Are registered incapable with the Office of Care and

Protection- Are in need of 24 hour support

The scheme is considered unsuitable for those:

1. With severe dementia2. With a physical disability and who need significant assistance

in relation to this and which the physical environment of the building is inappropriate

3. Who need a high level of supervision or need a high level of nursing care

4. Where there is a forensic history as a sex offender

15. Status of the Housing Scheme Voluntary – this is a Domiciliary Care home managed by Niamh and registered under the HPSS (Quality Improvement and Regulation Authority, Northern Ireland) order 2003.

17. Volunteers Traditionally the use of volunteers is essential to the work of Niamh. Currently 28 Lime Court, Peters Hill has no volunteers. This can however change once the scheme is fully operational.

Staff will update the commission H&SCTs and service users on any volunteers in scheme.

18. Admission Criteria Including Procedure for Emergency Admissions

All referrals for Peters Hill will be made through H&SCT and/or NIHE.

“During the referral process all prospective individuals are encouraged to be involved in all aspects and stages of the referral.”

NB Emergency admissions do not normally happen in Niamh housing schemes.

19. Referral and Allocation Procedure (Housing Services)

When a vacancy occurs, and a waiting list of Applicants is not held by the Area Manager/Assistant Director, Referral Agents, for example Care Managers or the Community Mental Health Team/ Community Learning Disability Team/ Resettlement Team will be informed.

It is recommended that Prospective Applicants visit the accommodation along with the Referral Agent or Carer, before an application is made. Visits will be by arrangement with the Area Manager and Housing Officer. Appropriate information will be shared and a Prospective Applicants Visit Form will be completed.

Criteria for Referral and Cost of Accommodation are outlined in the Application Pack distributed to referral agents and visitors. Referral Agents and Prospective Applicants must check both issues as early as possible.

The Referral Agent and Prospective Applicant together, complete the Referral Form and a General Housing Application Form. The Referral agent is required to supply the Scheme with a Comprehensive Risk Assessment/Risk screening tool.

These documents are returned to the Area Manager/Housing Officer and the General Housing Application Form is marked ‘Complex Needs’ and forwarded to the appropriate area Housing Support Officer for the Northern Ireland Housing Executive. A

referral panel will be convened and the outcome conveyed to both the applicant and the referral agent. If there are no current vacancies the individuals name then goes on to the waiting list of applicants held by the Area Manager/Housing Officer, and that held by the Housing Executive.

When a vacancy occurs and a waiting list of Applicants is held by the Area Manager, Referral Agents and NIHE will be contacted to confirm that Applicants continue to require accommodation.

An Admissions Panel will be established and may consist of, where appropriate Assistant Director , Area Manager, Housing Officer, Care Manager and or a representative of the Community Mental Health Team.

Applicants and Referrers will be invited to meet members of the panel to discuss their application and identify whether the scheme will meet the applicant’s needs. The Referral Agent will be contacted as to the most suitable way to structure this part of the procedure.

The Admissions Panel will complete an Admissions Panel Report, ensure that the service users needs are compared objectively with the eligibility criteria and agree collectively on the most suitable applicant for the existing vacancy. Financial requirements and Smoke Free Policy must be fully discussed and procedures agreed. Other suitable applicants may be placed on a waiting list of Suitable Applicants.

The Admissions Panel Report will include appropriate detail on the decisions taken and recommendations regarding alternative services for unsuitable applicants. The panel will agree the best way to share outcomes providing written explanations and details of the appeals procedure as appropriate.

Suitable Applicants will be offered accommodation or a trial period in accommodation, depending on their circumstances and the type of accommodation being applied for. Some self-contained accommodation may not require a trial period; other supported accommodation may require an extended trial period particularly for tenants coming from a hospital setting. During the trial period Support/Care Plans and Service user’s Notes will be maintained, to facilitate discussion and review.

Appropriate documentation for Benefits, Housing Executive and Housing Association should be completed as soon as possible, and the service user’s Personal File set up as required.

On the allocation of permanent accommodation the Area Manager will ensure that the service user understands and signs their License to Occupy and Tenancy Agreement.

All applicants have the right to appeal decisions taken by the Admissions Panel and/or decisions taken at the Trail Period Review Meeting. An applicant wishing to appeal a decision will in the first instance make this known to the Chair Person of the Admissions Panel. They will be provided with details of the Appeals Process including who will be involved, when and where it will take place and their right to be accompanied by a friend, relative, other professional or advocate.

20. Risk/Vulnerability Assessment Procedure It is the policy of the Northern Ireland Association for Mental Health to ensure that all staff know which service users present as being a significant risk of self-harm or being a danger to self or others, and which service users are vulnerable to abuse or exploitation. This is in order to protect the Health, Safety and welfare of service users/Staff/Volunteers and others.

On referral, Referral Agents are required to complete a Comprehensive Risk Assessment/Risk Screening Tool and complete the Safety/Risk/Vulnerability Assessment form.

The Referral Agent/prospective service user completing the form are asked to note situations or circumstances that contribute to, or trigger safety/risk/vulnerability issues, to outline service user strengths and how they cope with safety/ risk/vulnerability issues and to note the dangers that exist for the service user and others if the safety/risk/vulnerability issue is not dealt with appropriately. The purpose of this is not to apportion blame but to ensure that safety/risk/vulnerability issues are acknowledged during the referral process and become an integral part of the planning and review process.

This being so, the completed Safety/Risk /Vulnerability Assessment is part of the information available to the Admission Panel.

Depending on circumstances Referral Agents and/or service users may be contacted for further clarification.

On admission the Safety/Risk/Vulnerability Sheet becomes part of the service users File and a dynamic document that is then adjusted throughout the year to reflect changes in Safety/Risk/Vulnerability, for example as a result of a Review, an Incident, or to reflect a change in circumstances. A new Safety/Risk/Vulnerability Assessment is completed each year as part of the service users annual assessment.

Statistical information regarding the number of service users requiring support/care with Safety/Risk/Vulnerability issues will be collated and sent to the Assistant Director.

Inspire Wellbeing recognise that risk is part of life and individual risk taking and managing identified risk becomes part of ongoing individual care and support planning.

21. Arrangements for Completing Reviews with Service Users

Inspire service users of Niamh Housing schemes now have the opportunity and choice to participate in the completion of their files, all are actively encouraged to be involved in their monthly summaries, support plans and support plan reviews.

The following points need consideration.

The review should be implemented as informally as possible. The Service User’s daily notes and support plan form an

integral part of the review. The review report should be completed prior to the review with

the Service User Review of any safety/risk /vulnerability issues should be part of

the review. Next review date should be agreed at the meeting.

Planning and Review Procedure (Housing Services)

The Support Plan and Support Plan Review are Niamh procedures used to assess and evaluate areas of need and to revise and update issues for planning purposes.

In 24 Hour Supported Housing Schemes, Service Users will be appointed a Key Worker (Team Leader) and Co Key Worker (Support Worker) for the purposes of providing support with this procedure.

Following consultation with Service Users, Key Workers will extend invitations to referral agent/key worker/carer to participate as required. If they are unable to do so they should be invited to make comment at appropriate stages.

The Planning and Review Procedure will be implemented formally or informally, depending on the needs of the Service User.

Stages in Assessment, Planning and Review

a) Trial Period Review.

Following the offer of Permanent Accommodation the reasons for referral will form the basis of Support/Care Plans in the first instance.

b) Service Users Notes.

Service User’s notes are recorded on a daily basis, and used for issues relevant to the individual. They are maintained in a binder, completed in sections for each Service User, marked Private and Confidential, and stored in a locked filing cabinet. They are then transferred to the Service User’s Personal File at the end of each month. The service users notes are written on pink paper using black ink for support provided and notes are written on white paper using black ink for care provided.

c) Monthly Summary Sheets.

These are completed monthly with the Service User and are used to formally monitor each month looking at current issues, how things are going, and planning issues already noted. They will also pick up Areas of Need highlighted in the Support/Care Plan, reviewing and adjusting objectives for the month ahead as necessary.

d) Assessment and Support/Care Plan

The Assessment is to be completed prior to admission to scheme this forms the initial care and support plan. Both are completed with the Service User, and include information from the referral process and an evaluation of initial planning based on the reasons for referral. The Assessment is completed annually from this point,

and will include information from the Support/Care Plan Review. The Support/Care Plan is a dynamic document being adjusted to reflect changes throughout the year; outcomes will be noted prior to the annual review and a new Support/Care Plan drawn up following the annual assessment. Codes must be used to indicate area of need and reason for work, Care, Support, Safety/Risk/Vulnerability or any combination of these.

Care and Support provision is funded through H&SC by Trust Care and through Supporting People funding, if eligible. There is no addition charges for care within Peters Hill.

e) Safety/Risk/Vulnerability Assessment Sheet

While initially completed by the Referral Agent this form is then used to update on Safety/Risk/Vulnerability issues following Untoward Incidents or as part of a Support/Care Plan Review, Emergency Review, or to reflect a change in circumstances.

f) Daily Schedule of Activities.

This form may be used to support work that requires detailed planning on a day to day basis. It will enable a clear outline regarding daily routines, responsibilities, activities, etc.

g) Support/Care Plan Review Form

This form is used to evaluate areas of need and to revise and update issues for planning purposes. It is completed annually following the date of assessment but can be used to review care and support more frequently if required. The form is completed as part of a review meeting. Following consultation with the Service User this is organised by the Area Manager and may include the Service User’s key worker, the statutory key worker, the referral agent, the Housing Officer, the carer, and other significant parties as appropriate. Following an annual review the issues raised are fed into the annual assessment and inform the planning for the coming year. The Safety/Risk/Vulnerability Assessment will also be updated at this meeting, again to provide appropriate information for the annual assessment.

h) Emergency Review Form

This form is used to facilitate an emergency review related to a specific incident, issue or concern

i) Psychiatric Review Form

This form is used to facilitate psychiatric reviews and for information for the completion of Summary Sheets and or Support/Care Plan Reviews.

j) Service User’s Rights

While Service Users are consulted at all stages of the planning and review process they retain the right to ask for a review at any time and/or to appeal any decisions taken at a review. To do this, they will in the first instance, make their wishes known to the Area Manager. They will be provided with details of the process including who will be involved, when and where it will take place and their right to be accompanied by a friend, relative other professional or advocate.

If any of the above stages have been missed the Service User’s File should contain a note as to why.

As the above documentation forms the basis of a Service User’s file it is important that information is recorded accurately and appropriately. In line with good practice requirements all schemes should have a copy of ‘The Write Stuff’ and staff are required to read it. The policy on Access to Files details the procedure for Service User’s access to their Personal File.

Service Users can request reviews at any stage during their care/ support while in Supported Housing.

k) Each Resident has their own self contained flat with access to the communal lounge for activities, meetings for example

22. Arrangements for Service Users to Engage in Social Activities, Hobbies and Leisure

Details of activities arranged at scheme and local community are included in the Monthly Activity Planner and issued to each Service User. Social and Leisure activity and support with these are included in individual support plans based on individual choice.

All members are encouraged to participate in service user meetings and family/relative meetings, internal audits and inspections and RQIA/SSP inspections, ensuring a quality service.

23. Arrangements in Place for Consultation with Service Users about the Operation of the Housing Scheme

Regular Service User’s meeting take place normally on a monthly basis which provide the forum of consultation, sharing of ideas, suggestions and comments

Service users are encouraged to attend quarterly T.A.G meetings and quarterly Quality Group meetings. This is to provide a forum for discussion, information sharing and service users being consulted over strategic and procedural issues.

Inspire Monthly Monitoring visits are carried out by a Senior Manager not employed within Peters Hill and include the service users/carers perspective.

All information is available for service users/carers/auditors/inspectors can be provided in large print, CD, or other required.

24. Storage and Administration of Medication All Service Users are registered with a General Practitioner. He/she will prescribe the Service Users medication. Medication may also be prescribed by the Service Users’ psychiatrist.

The administration of medication is the responsibility of appropriately trained staff.

Medication is stored in locked purpose made medicine cupboards or trolleys. The keys are kept in a separate locked cabinet or in the custody of the person in charge.

Service Users who are self-medicating will be provided with lockable cabinets or drawer for the safe storage of their medication.

25. Fire Precautions and associated emergency procedures

Niamh, so far as is reasonably practicable, will manage in compliance with The Fire and Rescue Services (Northern Ireland) Order 2006, The Fire Safety Regulations (Northern Ireland) 2010, The Management of Health and Safety at Work regulations (Amendment) (NI) 2000 and other appropriate regulations.

Workplace Risk assessment: - Niamh will carry out Risk Assessments in all locations in accordance with The Fire Safety Regulations (NI) 2010.

These regulations are statutory requirements under a self-regulatory role, monitored by Northern Ireland Fire and Rescue Service and should be reviewed annually.

A Fire Safety Record File will be held within each scheme. This file should contain sections for the following records:-

Fire risk assessment Sample fire notice Annual test certificates Fire drill Record of training Records of maintenance checks carried out by the scheme

of the fire service. To ensure Service Users, visitors and members of staff can

be safely and quickly evacuated.

26. Arrangement for Meeting Service Users Spiritual Needs

Details of local Church/Chapel and any other spiritual places of Worship are available at scheme in The Members Information File. If a member requires staff to attend with them this can be arranged.

27. Arrangements in place for Social Contact with Relatives/Friends/Representatives/Local Community where practicable

Peters Hill encourages members to keep good relationships and contact with friends and family, and visitors are always welcomed.

In the interest of safety and security of the individual and other residents within Peters Hill, key/significant, relatives/friends/representatives should be identified within individual care and support plans.

Residents at Peters Hill have their own dwellings and facilities to enable privacy during visits. All staff respect individual rights to privacy.

All service users are encouraged to participate in the creation of the scheme Visitors Policy.

28. Arrangements for Dealing with Complaints As detailed in our policy and procedure manual QG 3 – Complaints Procedure. This procedure is intended to cover all persons involved in the work of Niamh (it pertains to any Service Users, Volunteers, Staff, Strategic Partner or general public) the complaints procedure is in accordance with the Performance Management Framework.

30. Details of Specific Techniques used in the Scheme and Arrangements made for their Supervision.

Currently the staff within Peters Hill are trained in ‘Respect’. This is a non-aggressive intervention for dealing with challenging behaviour should it occurs. This will be kept under regular review and will be used only in line with residents care and support plans and in agreement with the commissioning Trust.

Stage 1 – You can speak directly to staff, who will try to resolve things for you straight away

Stage 2 – If you are still not satisfied  or you did not want to speak directly to staff, you can make a formal complaint to our Services Department.  This can be done by letter, by using a complaints form or by telephone to any of the Managers at Central Office or to any staff member.

31. Maintaining the Privacy and Dignity of Service Users.

The core values of Inspire include dignity, choice, integration and respect. Dignity and respect are maintained and upheld by ensuring the Service User is involved in every aspect of his/her care and support.

Ensuring Service User preferences in relation to name, menus, activities, personalising their home are some examples. Details of these should be recorded within their care and support plans.

Providing a homely environment where the Service User can have friends and family to visit. Flexible meal times and being able to go to bed when they want to.

All Service Users have a key worker/co key-worker who will spend specified time with the Service User looking at their individual support issues and also having some social time together.

Use of C.C.T.V:

C.C.T.V is in operation throughout 28 Lime Court, Peters Hill through the exterior and internal passage ways. This is primarily for security and your protection. No C.C.T.V is used within your personal property and C.C.T.V is not a substitute for actual support provisions by staff.

32. Leaving Procedure

The procedure for a Service User leaving supported housing can be:

i) Service User's own choice - if a Service User decides they no longer wish to live in the scheme they must give four weeks’ notice, in writing, to the Area Manager. Should the Service User be seeking alternative accommodation, a member of the Learning Disability or Community Mental Health Team, involved in the Service Users care will be responsible for co-ordinating their resettlement.

ii) A Service User may be asked to leave, if he/she is not complying with the conditions of their tenancy / licence to

occupy. This would take place following a thorough investigation by the Area Manager and Assistant Director and with the agreement of the Director of Mental Health Services/Director of Learning Disability Services.

iii) A Letter of termination, giving four weeks’ notice will be sent to the Service User by the Assistant Director. Depending on circumstances it may be necessary to give 1 weeks’ notice. This will be done with the agreement of the Director of Mental Health Services/Director of Learning Disability Services. Eviction if necessary will be a last resort.

iv) If possible the Service User must have suitable alternative accommodation to go to before his/her departure. The Area Manager will provide information on homeless hostels, if necessary. All rent and arrears should be paid in full and all keys returned. Resident’s leaving form to be completed.

v) Tenancy may be terminated if a Service User has been detained in hospital for a period of six months.

33. Transfer Procedure

In the event of a decision being made for a transfer to another Supported Housing Scheme, within our organisation, or to a service outside the organisation.

(i) If a Service User is being transferred to another Inspire scheme as much information as possible should be sent to the scheme prior to transfer. The Service Users file, monies, medication etc should go with the Service User, with a member of staff accompanying.

(ii) Staff will seek the Service User's consent to pass on relevant information if requested by the transfer organisation i.e. where appropriate and with the approval of the Assistant Director. Staff will liaise with the referral agent/key worker/GP/carer or other support services as requested.

(ii) Staff will provide support to the Service User during the transfer process.

34. Record Keeping

Record-keeping will ensure that requirements of Niamh standards are met. There will be a separate book or files on the following:

3. Each file will be identified with the Service User’s name clearly displayed on the outer cover both on the front and side.

4. Files when closed should be retained for a period of five years after which they will be shredded. Closed files to have appropriate closure forms, stored securely and dated for disposal.

5. Access restricted to: Scheme Manager/Assistant, Person authorised by Scheme Manager, Service User (see Access to Files Policy), Referral Agent/Key Worker, Landlord/Housing Officers, NIAMH/HSSB Senior Officers, Mental Health Commission Representatives, External Audit – with letter of authorisation from Service User

8. Any written documentation must be completed in black ink, the only acceptation being for a person with a diagnosis of dyslexia. In this case blue or green ink may be advised. All extra entries must be signed with full staff name, (not initialled) and dated. Correction fluid must not be used on any document.

9. All documentation must be securely fastened into the file in the correct section in line with Niamh specification.

10. ' ALERT' notification - A red sticker placed on the outside of the Service User's file will draw attention to critical information contained in the Special Needs Form on the inside cover, e.g. allergies, physical health conditions..
