□ Passport: Please submit your Canadian passport with enough blank pages for visa and entry/exitstamps. **Passport must be valid for 6 months after the date of departure**

□ 1 Application Form: This form should be completed in full and signed with the applicant's original inksignature

□ 2 Passport photos: Color passport photos 4 X 6 cm on white background taken within past 6 months.Please do not staple pictures to the application form.

□ Flight Itinerary: Please provide round trip flight itinerary if you are applying for 90 days multiple entryvisa.

□Multiple Entry Visas: Multiple entry visas are valid for 30 or 90 days only. If you are applying formultiple entries, please specify "30 Day Validity" or "90 Day Validity" on the Client Information Sheet.

□ For non-Canadian passport holders only: Proof of Legal Status in Canada is required. Please include acopy of your Permanent Resident Card, Work Permit or Student Visa. Please contact our office to verifywhether the original document or a photocopy is required. If you cannot provide proof of status, yourvisa will have to be processed in the country of origin.

□ Client Information Sheet: This form should be completed in full and submitted with all visaapplications.
