1 Document Overview This Business Rules for Access Service Request (ASR) document describes the fields used for the communication of ASR business process requests between CLECs and Verizon. The document contains a description of the forms and fields details.

1.1 Document Structure Section 1, Document Overview describes document organization and the contents of each

Section. Section 2, ASR Forms Components, describes the components of a form’s data element. Section 3, ASR Forms Tables, contains tables that provide specific information regarding valid

field entries for ASR forms. Section 4, Appendices, contains tables that summarize and highlight business rules changes.

2 ASR Form Components This section provides specific information regarding certain components of an Access Service Request as well as the definition of each form field.

2.1 Form Table Definitions The form tables contain eight (8) specific columns. Following is a description of each one of these columns: COLUMN 1 This column provides numbering of fields for easy reference. FIELD This column provides the field name abbreviation for each field on a particular form. DATA DESCRIPTION This column provides a full description of the field. LENGTH This column provides the maximum length of each field. TYPE This column provides specific data characteristics for each form field. Alpha/Numeric (A/N) means that there are no restrictions on the type of characters permitted for that element; Alpha (A) indicates that only alphabetic characters are permitted and Numeric (N) indicates that only numbers are permitted. USAGE This column provides the appropriate requirements for the population of each field within a particular request.

another field as specified in the “Notes and Conditions” column

3 ASR Forms Tables This section contains tables that describe the forms used when ordering Unbundled Network Elements from Verizon via Access Service Requests.

3.1 Introduction All forms have a unique set of fields. The tables in this section provide a detailed presentation of the fields associated with a form and the usage of those fields that will result in a service request.

References to New York (NY) include Connecticut.

3.1.1 Typographical Conventions Different typographical conventions are used throughout Section 3 and are as follows:

�� Gray shaded fields and valid entries are not applicable in Verizon when the UNE field on the ASR Form is “Y”. The use of shaded fields and/or valid entries is at your discretion.

�� Italic text indicates fields needed to permit implementation of local Verizon requirement. These fields must be provided as defined within the field Notes and Conditions. Verizon cannot process your service requests for unbundled network elements without you following this information.

1. CCNA Customer Carrier Name Abbreviation

Identifies the COMMON LANGUAGE IAC code for the customer submitting the ASR and receiving the Confirmation Notice Form (CN) and the Clarification / Notification Form (C/NR). This code is established prior to the submission of the ASR.

When UNE field = “Y”, “CUS” is prohibited. 2. PON Purchase Order Number 16 A/N Required Identifies the customer’s unique purchase-order or requisition number that authorizes

the issuance of this request or supplement. The Purchase Order Number may be reused after two years from the due date of the original request.

3. VER Version Identification 2 A/N Required Identifies the customer’s version number. Any reissuance can use this entry to uniquely identify the form from any other version.

4. ASR NO Access Service Request Number

18 A/N Conditional Identifies the number that may be generated by the provider’s mechanized systems, pre-assigned to the customer by the provider, or manually assigned by the provider to identify a customer’s request for service.

Required when ASR NO is pre-assigned Required on all supplements when PON is not unique. Otherwise optional.

5. SPA Special Action Indicator 2 A/N Optional Position 1 is for customer use only Position 2 is for provider use only

An indicator used to identify an order being sampled for quality control purposes.

6. ICSC Interexchange CustomerService Center

Identifies the provider service center.

7. CC Company Code 4 A/N Conditional Identifies the Exchange Carrier requesting local services. A four alpha/numeric character code structure for all Exchange Carriers in North America and certain U.S. territories maintained by NECA.

Required when ordering local interconnection services or unbundled network elements. Required when UNE field = “Y” or ordering local services. Otherwise prohibited. Use Operating Company Number (OCN)

Optional when the CC field is populated and the first position of the REQTYP field is “M”, “S”, or “L”, otherwise prohibited

Required when ordering UNEs. See REQTYP field notes for UNEs. 9. D/TSENT Date and Time Sent 17 A/N Required Identifies the date and time that the Access Service Request is sent by the customer.

10. QA Quote Authorized 1 A Conditional Indicates that a quotation charge for special construction is authorized.

Optional when the ACT field is “N”, “C” or “T”, otherwise prohibited. 11. CBD Call Before Dispatch 2 N Conditional Identifies a customer request for the provider to call the LCON at a desired time

prior to “dispatch out” of a service technician for service installation or disconnect. Prohibited when the UNE field is “Y”, otherwise optional.

12. DDD Desired Due Date 10 A/N Required Identifies the customer’s desired due date. The actual due date may be different from that desired because of factors such as

availability of facilities and quantity, complexity, and impact on local service of circuits involved.

On disconnect requests, this date represents the date billing is to stop on the involved circuit(s) and can be no earlier than the date the request is received by the provider.

When different due dates are required, these dates are stipulated using a separate request for each desired due date. For example, a total of 50 circuits are desired and the customer wants them at a rate of 10 per day. Therefore, five ASR forms may be submitted stipulating this requirement.

13. FDT Frame Due Time 6 A/N Conditional Provides special handling instructions for the connection or disconnection of this request, such as, time of day when service should be disconnected.

Prohibited when the ACT field is “R”, Prohibited when the UNE field is “Y” Otherwise optional.

14. PROJECT Project Identification 16 A/N Conditional Identifies the project to which the request is to be associated.

Required when the ACT field is “N” or “D” and the CCVN field is populated, otherwise optional.

15. CNO Case Number 12 A/N Optional Identifies the quotation tracking number assigned by the provider in response to a provisioning arrangement inquiry, e.g., diversity.

16. PPTD Project Plant Test Date 10 A/N Conditional U.S. Standard Metric Format

Identifies the pre-negotiated plant test date for the start of overall testing of the service requested on this ASR.

Optional when the PROJECT field is populated, otherwise prohibited. 17. PFPTD Project Facility Plant Test

Date 10 A/N Conditional U.S. Standard

Metric Format Identifies the pre-negotiated facility plant test date for the start of overall testing of the switched access facilities being requested on this ASR.

Optional when the first position of the REQTYP field is not “R” and the PROJECT field is populated, otherwise prohibited.

group of ASRs being ordered. In order to facilitate the process, at least the first related ASR should describe or list

the total RPONs used when RPON is not the same for all such requests. For example, six ASRs are to be associated and individual PONs and RPONs are being used.

All service types must be identical for such grouping of like orders.

Required when the TQ field is populated and a total of two or more ASRs are associated with the translation questionnaire, otherwise optional.

19. LUP Intrastate IntraLATAUsage Percentage

3 N Conditional 1 to 100 Identifies the percent Intrastate IntraLATA usage for use with IntraLATA competition. This field may apply whenever PIU is less than 100.

Prohibited when the PIU field is not populated or is “100”. Prohibited when the first position of the REQTYP field is or “R”. Otherwise optional.

20. BSA Basic Serving Arrangement

1 A Conditional Y = unbundled ordering Identifies the requirement for a Basic Serving Arrangement (BSA), which is the minimum necessary transport arrangement for the delivery of the unbundled network features and functions, or for a Basic Service Element (BSE) associated with a Basic Serving Arrangement.

Prohibited when the CC or WST field is populated, otherwise optional.

Identifies the type of service being requested and the status of the request. The first character of REQTYP specifies the type of service/element. The second character of REQTYP specifies the status of the request in the four step order process.

When UNE field = “Y”, first position entries below are the only valid entries and represent: “L” = SS7 Unbundled STP Port “M” = Dedicated Trunk Port and Extended Dedicated Trunk Port (EDTP) Type 1*

& Type 2* “S” = Unbundled Interoffice (IOF) Transport, Unbundled Multiplexer (Mux), Unbundled DS1, DS3, OC3 and OC12 Loops, DS1 and DS3 Multiplexed Expanded Extended Loop (EEL), and Voice Grade, DS0, DS1 & DS3 Expanded Extended Loops, Unbundled Dark Fiber, Dedicated Transit Service (DTS), Unbundled Feeder Sub-loop Element (UFSE)

22. ACT Activity 1 A Required N = New Installation or increase in capacity D = Disconnection or decrease in capacity M = Inside move of the physical

termination within a building T = Outside move of end user location C = Change or modification to an existing

service. R = Record Activity for ordering

administrative changes.

Identifies the activity involved in this service request.

“C” may not be used to migrate to or from Unbundled Network Elements. In DC, DE, MA, MD, ME, NH, NJ, NY, PA, RI, VA, VT & WV for UNE IOF and

DTS, valid entries are “N”, “D”, “C” and “R”. “M” and “T” are not applicable. In DC, DE, MA, MD, ME, NH, NJ, NY, PA, RI, VA, VT and WV for UNE DS1, DS3,

OC3 and OC12 Loops and Voice Grade, DS0, DS1 & DS3 EEL Loops, valid entries are “N”, “D”, “C”, “T” and “R”. “M” is not applicable (VZ is not responsible for inside wire).

“T” is only associated with Loops or EEL Loops combined with EEL transport. For UNE Dark Fiber, valid entries are “N”, “C” and “D”. "C" may only be used

associated with “parallel provisioning” For UFSE, valid entries are “N” and “D”. “C”, “M” and “T” are not applicable.

24 QSA Quantity Service Address Location Information

2 N Conditional Identifies the total number of Service Address Location Information Forms being sent by the customer.

Required when the ACT field is “N” or “T” and the first position of the REQYTP field is “S”, “E”, “W”, “V” or “X” and the first position of any PRILOC / SECLOC field is an “E”.

Required when ACT = “N” or “T” and 1st position of REQYTP = “S”, ordering UNE DS1, DS3, OC3 and OC12 Loops, Voice Grade, DS0, DS1 and DS3 EEL Loops in combination with EEL transport or UNE Dark Fiber Loops*.

Required when the ACT field is “N” or “T” and the 1st position of the REQTYP field is “R” and the 1st position of the PRILOC field is an “E” and the SPOT (PRI) field is not a CLLI Code on the RING or ARI Form.

Required when the ACT field is “N” or “T” and the 1st position of the REQTYP field is “A” and the NSL field on the FGA Form is populated.

Prohibited when the first position of the REQTYP field is “M”: or Prohibited when 1st position of REQTYP = “E”, “W”, “V”, “X” or “R” and UNE

Identifies the type of confirmation response requested by the customer.

“D” is prohibited when the first position of the REQTYP field is “E”, “R” or “W”, or the ASC-EC field is populated, or the second position of the TQ field is “N” or “X”.

“D” is prohibited when the ACT field is “M” or “R”. “S” is prohibited when the ACT field is “M” or “R”. “N” = no response requested. A numeric entry indicates the number of working days that the customer requires to

confirm the Design Layout Report (DLR) and will be added to the overall interval. “1-10” prohibited when the ACT field is “M” or “R”.

“D” is prohibited when the UNE field is “Y”.

4 = Correction (indicates that this request is being issued to correct a previous request for which a firm order confirmation has not been received)

A supplement is any new iteration of an ASR. The entry in the SUP field identifies the reason for which the supplement is being issued. For valid entry of “1”:

If the pending order was already completed as ordered, a separate request must be sent instead of the supplement.

Valid for Service Requests and Firm Orders whether or not they have been through the confirmation phase.

For valid entry of “2”: Indicates the pending order requires a change of due date. The new due date is

specified in the DDD field, desired due date. The new desired due date can be no more than 30 calendar days greater than the original due date. If the request is to establish a due date sooner than the current due date then the EXP field must contain a “Y” when less than the standard interval.

Valid for requests which have a Firm Order Confirmation or a Design and Order Confirmation.

For valid entry of “3”: This may affect the previously agreed upon due date. Partial cancellations should also be entered with a “3” in the SUP field. A request for a change in Due Date in conjunction with other changes to a pending

order should be submitted with a “3” in the SUP field. The new desired due date can be no more than 30 calendar days greater than the original due date. If the request is to establish a due date sooner than the current due date then the EXP field must contain a “Y” when less than the standard interval.

This entry is also to be used for supplements that are a result of provider changes after the Firm Order Confirmation (FOC) or a Design and Order Confirmation (DOC).

Valid for requests which have a Firm Order Confirmation or a Design and Order Confirmation.

Requires an entry in the REMARKS field to identify the changes. The remainder of the request must be identical to the original request issued.

For valid entry of “4”: Valid for Firm Orders that have not been through the confirmation phase and

Service Requests that may or may not have been confirmed. Requires an entry in the REMARKS field to identify the changes. The remainder of

the request must be identical to the original request issued.

Prohibited on initial requests. Prohibited when changing a service request to a firm order. Prohibited when changing service type which results in a change to first character of
