FINAL REPORT

BACKGROUND

At the III Inter-American Minister Meeting on Animal Health (RIMSA III) and theXXXI Meeting of the Board of Directors of the Panamerican Health Organization (PAHO),both held in 1983, the participating countries undertook to root out urban rabies in the mostimportant cities of the region.

This led to convene the I Meeting of Directors of the National Rabies ControlPrograms, held in Guayaquil, Ecuador (1983), at which the strategies and the "Plan of Actionfor the Elimination of Urban Rabies from the Leading Latin American Cities by the end ofthe Eighties" were approved. Two more Meetings were held in Brasilia (1987) and PortoAlegre (1989) to assess the progress achieved by the Regional Program and to make thecorresponding adjustments.

At the end of the 1980-1989 decade, PAHO/WHO, jointly with the membercountries, carried out an assessment of the Regional Programs, presenting their results at theVII Inter-American Minister Meeting on Animal Health (RIMSA VII), held in Washington,D.C. from April 30 to May 2, 1991. (Upon close analysis of the Evaluation Report andResolution II of RIMSA VII). The XXXV Meeting of the Board of Directors ofPAHO/WHO held in September 1991, advised the Director of PAHO to persist in his effortsimplying the cooperation with the countries, suggesting an extension of the program, so asto include marginal areas and smaller populations, until dog transmitted rabies.

In the month of October 1992, the Director of PAHO/WHO called the IV Meetingof Directors of the National Rabies Control Programs, in Mexico City, Mexico, with thepurpose of strengthening the Regional Program. At this Meeting, the goal is eliminate to rootdog transmitted rabies. The original goal included the main cities, and this was extended toinclude smaller populations and rural areas by the year 2000.

Moreover, the importance of the epidemiological surveillance of rabies in wild animalswas emphasized, especially that transmitted by vampire bats.

In order to follow-up this process and analyze the Regional Programs' achievements,the Director of the Organization convened the V Meeting of Directors of National RabiesControl Programs in Latin America, to be held in Santo Domingo, Dominican Republic,from February 13 to 15, 1995.

OBJECTIVES

* To analyze the goals achieved by the Dog Mediated Rabies Control Program ofrabies transmitted by dogs.

* To analyze and to adjust the Plan of Action for Consolidating the Final Phase of theDog Transmitted Rabies Control Program, 1995 - 1996.

* To analyze the recommendations issued by the Group of Experts regarding TechnicalAspects for the Identification of Rabies-Free Areas and Animal QuarantineRequirements.

* To define strategies for the surveillance, prevention and control of rabies transmittedby wild animals.

PARTICIPANTS

The Meeting was attended by Rabies Control Program Directors from Argentina,Belize, Bolivia, Brazil, Colombia, Costa Rica, Cuba, Chile, Ecuador, El Salvador,Guatemala, Haiti, Honduras, Mexico, Nicaragua, Panama, Paraguay, Peru, DominicanRepublic, Uruguay and Venezuela. It was also attended by the inter-country official from thePAHO/WHO's Veterinary Public Health Program, representing the English-speakingCaribbean countries, and observers representing national authorities from the sectors relatedto Agriculture and laboratories producing biological compounds.

DESCRIPTION OF THE MEETING

Opening

The Meeting was opened on Monday, February 13, at 9:00 a.m. by the Secretary ofHealth of the Dominican Republic, Mr. Victor García Santos, preceded by a speechaddressed by the Representative of the Panamerican Health Organization PAHO/WHO in theDominican Republic, Mr. Merlin Fernández, and the Coordinator of the PAHO/WHOVeterinary Public Health Program, Mr. Primo Arámbulo III.

Selection of the Board of Directors

Doctor Enrique Duarte, Viceminister of Health in Guatemala, was proposed asChairman of the Meeting; Doctor Enrique Morales, Director of the Dominican Republic'sRabies Control Program, was proposed as Vicepresident; and Doctor Fernando Vargas Pino,Director of the Mexican Rabies Control Program, was proposed as Secretary. Thesenominations were approved unanimously.

Activities:

A description of the Regional Rabies Control Program was provided, including itsobjectives, goals, strategic components and current situation. A chronological analysis of theRegional Plan of Action for Rabies Control since its formulation in 1983 shows that theProgram has made significant progress, which is reflected in a reduction of cases of humanas well as canine rabies. Likewise, there is more information available regarding the problemof rabies in wild animals, which, in the case of vampire bats, is becoming increasinglysignificant.

Another of the major achievements pointed out was the consolidation of aninfrastructure that can provide the basis for controlling other zoonosis, given the trainingreceived by the staff and the strengthening of the infrastructure. It was also acknowledgedthat the Governments' political decision receives widespread social support from large sectorsof the population, thus contributing to ensure the maintenance of the Program.

During the coming years the Program must face a significant challenge due to thepossibility of a substantial redution of allocated resources the illness is close to beingeradicated. One of the prospects to counteract this challenge is the setting up of a PermanentCommission for Rabies Control in Latin America and the Caribbean, with the PanamericanInstitute for Food Protection and Zoonosis (INPPAZ) acting as its ex officio Secretariat. Thisapproach will be submitted to the Managing Bodies of PAHO/WHO for final approval.

The Report from the Meeting of "Experts regarding Technical Aspects for theIdentification of Rabies-Free Areas and Animal Quarantine Requirements" was presentedimmediately afterwards. In view of the progress achieved by the Rabies Control Program,a need has arisen to establish regulations for the identification of the rabies-free areas in theRegion in an international context, since there is an increasing transit of household animalsthat are mobilized from one region to another.

Likewise, the existence of rabies in wild animals can pose serious restrictions for themobilization of animals and the definition of rabies-free or endemic areas, establishing a needto redefine certain concepts; to promote research work destined to outline the characteristicsof the rabies virus varieties and its distribution in the different regions; to develop rapiddiagnostic tests to avoid the long periods of quarantine and have available contingency plansfor timely control in cases of reinfection of areas considered to be free of rabies.

One of the priorities highlighted is the need to carry out work aimed at thecharacterization of the virus varieties according to monoclonal antibodies, which requires thatthe countries provide the means for transporting samples to the reference laboratories.Moreover, the importance of encouraging the functioning of sub-regional referencelaboratories was discussed.

Likewise, it was advised to promote the study of the ecology and ethology of wildspecies such as foxes, coyotes, bats, mongooses and others, during rabies incubation periodsand periods of excretion of the rabies virus in the saliva. Additionally, the need to conductresearch on human behaviour and its possible influence on the spreading of the disease fromone area to another was considered.

The situation of rabies in the Region was analyzed in presentations made by thecountry representatives.

The information submitted on this occasion was accurate and consistent, as a resultof the formats for the collection of information designed by the Veterinary Public HealthProgram Coordination Unit (Annex III).

All the countries stressed the importance of carrying out joint actions in border zones,as well as the continuity of massive vaccination programs in high risk areas, with scheduledinterventions in rabies-free areas and the training of staff working in health programs for thecare of subjects exposed to rabies. Special emphasis was placed on the quality control of thelaboratory diagnosis of rabies, particularly in countries and areas free of rabies.

PAHO was requested to continue promoting political support to provide sustainencefor the programs and, particularly, to maintain or increase the financial resources whererequired.

The country representatives expressed their concern with regard to rabies in wildanimals, particularly rabies transmitted by vampire bats which is claiming an increasingnumber of human victims. It is necessary to carry out an in-depth analysis of the situationwithin the highest risk groups, such as Indians, miners, military people, etc., to supply themwith appropriate protection measures, since the current pre-exposure treatment systems aredifficult to comply with.

It was mentioned that rabies is not only a health problem, it is the result of a socialproblem, which calls for an adjustment of the approach to educational and socialcommunication, in order to render society as a whole more receptive and become involvedin programs to eliminate rabies. It was considered important to keep the communityinformed, not only regarding the control actions, but also on the results, so as to enable thecommunity to share in the program's achievements. It was proposed that PAHO shouldprepare guide a methodology in order to update the education and social communicationprograms for rabies prevention.

The point was stressed that the Governments need to maintain the responsibility forthe eradication of rabies and avoid the technical cooperation agencies' projects from takingon this responsibility.

Dr. Eduardo Alvarez, Head of Program Development of INPPAZ, presented a globalanalysis of the status of rabies in America, and specifically in connection with theperformance of the National Programs. He emphasized epidemiological surveillance, andpointed out that the countries catalogued as rabies-free do not possess appropriateepidemiological surveillance. Furthermore, some countries that managed to eliminate rabiesin the cities, became re-infected due to shortfalls in their epidemiological surveillance. Hementioned that the availability of both human and canine anti-rabies vaccines widely exceedsthe amount of doses used by the countries, thus calling for a sub-regional revision of theproduction, imports and requirements of the biological compound for each country, in orderto improve distribution. Lastly, Mr. Alvarez pointed out that the Regional InformationSystem, VERA, operates with long delays, stressing that greater efforts must be made to useweekly information for the rabies control and eradication process.

Dr. Raúl Londoño, Director of INPPAZ, made a presentation on the subject oftechnical cooperation, specifically referring to the potential cooperation between countries.

Finally, the activities for 1995 and 1996 were programmed, including the need fortechnical cooperation from PAHO. Annex IV includes the programming per country.

RECOMMENDATIONS

1. Considering that the regional production of rabies vaccines for veterinary purposesand for human beings is sufficient to meet the countries' present needs, membercountries are advised to cooperate more closely through PAHO/WHO, so that thosethat have difficulties in obtaining the biological may have access to any surplusquantities for their own control programs.

2. It was further recommended that PAHO/WHO should support the programmingefforts between the countries, so as to develop strategies for rabies control andprevention to be applied jointly in high-risk border zones.

3. It is recommended to continue with the training of human resources working in thehealth services, to improve the care of subjects exposed to the risk of rabies and thatthis training be included in specialized schools for medical and paramedical staff.

4. The countries considered to be free of dog transmitted rabies must strengthen theirepidemiological surveillance systems, maintaining laboratory testing of samples ofanimals that died from unknown causes, from nervous signs, or suspected to havehad rabies. It was recommended to conduct studies to assess the risks ofreintroducing rabies through imports of carnivorous animals, or by means of the

spreading of rabies among wild animals. These studies must include a social andeconomic evaluation in cases where the infection was reintroduced.

5. All the countries must assign special attention to the development of educationalstrategies directed at the community and the staff working in health programs in orderto involve them in the definition, planning, conduction, execution and assessment ofthe local program activities and obtain a commitment from the population to reachthe goal to eliminate rabies in the region.

6. The countries are advised to carry out an internal analysis of their epidemiologicalsurveillance and information systems, in order to follow up the data on a timely basisand facilitate the decision-making regarding the control of sources of infection andthe prevention of human and animal rabies.

7. The PAHO/WHO's technical cooperation is required for the preparation anddistribution of a methodological guide to aid countries in the development andassessment of the rabies program's educational component, strongly aimed at theschool-age population and risk groups, according to each country's epidemiologicalsituation.

8. National authorities are requested to facilitate the timely issue of import/exportsanitary permits for samples and supplies destined to diagnostic, quality control andbiological production activities connected with the National Rabies Control Programs.

9. In view of the scarcity and high cost of anti-rabies biologicals in some countries,these are advised to estimate their supply requirements during the planning stage soas to ensure their administration and to avoid losses. Likewise, it is advised that thesurplus supply be placed at the disposal of PAHO/WHO for the creation of a vaccinebank to make their utilization possible in other countries.

10. To advise the sanitary authorities of each country, not possessing diagnosticlaboratories applying the fluorescent antibody technique, to implement the same assoon as possible, in order to introduce this methods of diagnosis to theepidemiological surveillance of rabies.

11. To establish the Regional Commission for Rabies Control in America (CORERA),constituted by the National Rabies Control Program Directors, for which thePanamerican Food Protection and Zoonosis Institute (INPPAZ)will act as an ex oifcioSecretariat.

The National Program Directors will file for official status for the Commission beforethe national authorities in each of the countries.

12. In view of the endemicity of rabies in mangooses in five of the Caribbean countries,and the development of oral anti-rabies vaccines for wild animal species, it is

recommended for PAHO/WHO to offer cooperation for the studies required to adaptan oral anti-rabies vaccine to be used as an intervention strategy for the control ofrabies in this species.

13. Considering the privatization processes that are being implemented in the region'sagricultural and health sectors, PAHO/WHO is requested to offer cooperation on thissubject in order to avoid affecting the objectives of the Rabies Control Programs.
