Qualifications
• Currently uninsured
• Live in the communities of Magna, West Valley City or Kearns
• Household incomes of less than $35,000.
• Have a current medical need.

Mobile Medical Clinic
• 31 ft trailer with two patient rooms.• Well-checks to all ages with emphasis on diagnosing and
treating diabetes, hypertension, thyroidism, lipidemia, and asthma.
• Focus on developmental evaluation for children. • Breast, colon, and prostate cancer screening for adults. • Minor acute care. • Mental illness care dealing with depression, anxiety,
ADD, and bi-polar disease. • Smoking cessation services and education on the
dangers of secondhand smoke.
