How Did We Get here

• Supported by IPC with Outcomes Workshop• Outcomes Report to assist shaping approach• Pulling together the different areas

– Environment (culture)– Processes (care planning)– Arrangements (service delivery)

• Senior Management support to form Outcomes Project Group• Investment in Person Centred training programme

Page 4: Using Person Centred Ways of Working to Identify Outcomes.

Using the Toolkit

• Need to create an understanding of the issues involved around the personalisation agenda and the links into outcome focused ways of working

• Important to debate what outcomes are – we still have a long way to go

• Need to start looking at the person differently from the start of the assessment process

• Monitoring of outcomes is essential – otherwise why are we doing this

Putting it into Practice - a real life example of how we are working with people differently

That I continue to do my dog walking every Thursday afternoon. That I see my mum and dad on

alternate weekends. That I am told in advance of any

changes that may be happening. That I always wear my watch. That I always look nice and smart. That I go go-carting regularly, I

really enjoy it and I am pretty good at it. I like cars and really en-joy talking about them.

My routine is very important to

me, I like to do things at certain times , it helps me cope with my day.

What is important To me

What those who know me , say they like

and admire about me…..

HE HAS A GREAT SENSE OF

HUMOUR.

SUCH A NICE YOUNG MAN

KIND & THOUGHT FUL

HELPFUL & CONSIDERATE.

HE ALWAYS LOOKS SMART

AND TRENDY.

I have a fear of hot water, I can cope with it being a little warm, I don't like hot food either.

Dogs that are not on a lead frighten me.

I like to keep clean, I don't like being dirty, I need to wash my hands if they get dirty and I don't like the hot air that comes out of the hand driers.

I do not cope well with “ screaming kids” and prefer not to be in crowds, I need my own space.

Times are very important to me ( breaks etc.) I like to stick to them and I don't like to be late otherwise I get really stressed and may say rude things to people, if Iam like that just ignore and give me time to calm down.

I do like to give people hugs, I know I shouldn’t but sometimes I forget , I need reminding to greet people with a handshake and to give people there own space.

Sometimes I get really agitated and I might get a little bit stroppy and say some rude things, I may need a little space somewhere quiet.

I like keys , its best not to leave any lying around it may be too tempting for me.

I sometimes work better if you give me some responsibilities.

Nice to talk about things. Happy to talk to you.

Assessors View:

Jamie spent a lot of time with us talking about himself, he was very comfortable and relaxed. Jamie expressed how happy he is at his home and with the staff and how they support him with his interests. We feel Jamie should be encouraged to keep his independence, by continuing with skills development modules and working on his social skills, turn taking, team work and building his confidence and self esteem.Carers View:
