1 PREFACE
This manual describes how to use the e-Form Application from a citizen’s point of view.
2 INTRODUCTION
2.1 AUDIENCE
This guide is intended for users who are responsible for using the e-Form web application in West
Bengal. Users of this guide should be familiar with the functionality of the e-Form Application.
2.2 HOW TO USE THIS DOCUMENT
This document is basically a guide for the users who will use the e-Form application. Step by step
flow is mentioned in the document. Users can follow the steps to successfully use the e-Form
application.
2.3 STANDARDS AND CONVENTIONS USED IN THE GUIDE
i. A red star (*) implies a mandatory field.
ii. Unique Application ID, generated after the submission of the eForm, should be kept
cautiously for any future reference.
3 GETTING STARTED
e-Form application enables the Citizens to avail different services of the provided by different
departments of Government of West Bengal. A selected set of application forms are available with a
web interface which can be submitted online to avail the respective services.

4 USER REGISTRATION & MANAGEMENT
The following sections details out different user registration & management activates that the
Citizen/user can perform in the eForm application.
Modify your user profile through “Edit Profile”
Change your account password using “Change Password”
4.1 NEW USER REGISTRATION
Citizens need to register themselves using the Create New Account link in the eForm Application
Login page:
Clicking Create New Account link, Citizen will be able to Register himself/herself to apply for the
eServices.

5 MANAGEMENT OF USER ACTIVITIES
The following activities can be carried out from the menu item “My Account” of the application:
View your submitted applications in “My Submitted Applications”
View your saved applications in “Draft Applications”
5.1 VIEW SUBMITTED APPLICATIONS
The submitted applications are listed in following format:
User can view/do the following on the screen:
Serial No. – Running Serial No. of the submitted applications
Service Code – The Service Code for the submitted applications (Please refer Service Code
table in Appendix – A)
Unique Application Id for each submitted application
Date of Submission of the eForm application
Click View Details link to view & print the submitted form.
5.2 VIEW SAVED APPLICATIONS
The saved applications are listed in the following format:
User can view/do the following on the screen:

Service Code – The Service Code for the submitted applications (Please refer Service Code
table in Appendix – A)

Click Edit ( ) link to open the submitted form for edit/submission.
5.3 CHECK THE STATUS OF YOUR APPLICATION
To check the status of any submitted form, click on “Check the status of your application” link or
“Status Search” in the menu bar. The following page opens:
Enter the “Unique Application ID” of a submitted application and click on the search button. This
will display the current status of the application form in the following format:
The status report can be saved & printed for future reference.

6 EFORM APPLICATION SUBMISSION PROCESS
6.1 LOGIN
To avail the e-services of the concerned departments, Citizen requires being a registered user. The
login page appears with the following features:

6.2 SERVICE DASHBOARD PAGE
On successful login the service Dashboard page containing all e-forms will appear to the citizen
user which is as follows:
This is the landing page which will be visible on successful log in. Now, the citizen needs to choose
from the given list of Departments.
If the citizen selects All from the list of Departments, then all the e-services of Departments will be
visible on the screen.
User can also select a particular department of his choice and choose from the available list of
services. For example, if the user selects Directorate of School Education, the following screen will
appear:

7 ONLINE NEW APPLICATION IN ENGLISH
Application for Swami Vivekananda West Bengal Government Merit cum Means Scholarship
Scheme for students of UG and PG Courses in general education (Arts, Commerce and Science)
and other professional courses excluding Engineering & Medical. When user clicks the link Apply
Online (English) in the Dashboard page listing eServices for the Education Directorate, the following
form appears to be filled-in, saved and submitted:

7.1 DIRECTIONS FOR FORM SUBMISSION
The following steps are to be followed to successfully submit an eForm application:
1. Fill up the eForm as per guidelines given in section 7.2 & 7.3.
2. Save the form using the Save button when all the mandatory entries have been done and all the
mandatory documents have been attached.
3. Save will lead to the view page with the details entered in the form so that the entries can be
verified further. The entered eForm gets saved at this stage.
4. In the view page, you can review the uploaded documents further to ensure that they are the
required. Based on your observations, you can do any one of the following:
a. Click the Back button to further modify anything in the eForm
b. Click the Done button to close the view page & go back to the service Dashboard page.
The saved form can be accessed later from the menu path “My Account -> Draft
Applications”.
c. Click the Submit button to submit the filled up eForm for further process by the
respective Department/Directorate.

9 OFFLINE NEW APPLICATION ENGLISH
The Offline form is a PDF file which needs to be downloaded from the Offline Form option available
in the dashboard page after selection of the respective department’s service. When user clicks the
link Apply Offline in the Dashboard page listing eServices for the Education Directorate, the
following form can be obtained, filled-in and submitted:

The offline form can be submitted using the following set of instructions:
Download the Offline PDF
1. Download the Offline PDF from the link above using the following steps:
i. Use Right Click on the Download link
ii. Choose Save link as..
iii. Choose folder to save & Save the PDF form
Requirements to work with Offline Forms
The following software are mandatory at the local machine to work with the Offline Forms:
1. To Open & Edit the downloaded Offline Forms, Acrobat Reader V9 or above is required
2. To Save the XML version of the edited Offline Form, any e-Mail Client is required Edit
downloaded PDF Form
i. Open the downloaded PDF form from the saved location
ii. Fill up all the required Fields. Entering of data in the (*) marked fields is must.
iii. Click Validate button to check whether any field is missed
iv. If error is reported, correct at the required places & Press Validate Button again
v. Click OK button in the validation confirmation window
vi. Button Save As XML appears
vii. Check the Internet Email option & Click OK button
viii. Choose folder to save & Save the XML form (at the time of saving the entered data as
XML, please note the name of the saved form)
ix. Close the PDF Form
Upload saved XML file
1. In the Apply Offline link of the respective service page, click Choose File button.
2. File selection screen appears
3. Select the saved XML file
4. Click the Upload button to show the Offline Form data on the e-Form screen.
5. Follow the instructions on the screen to Save & Submit the filled up form.

11 ONLINE RENEWAL APPLICATION IN BENGALI
The submission of Bengali Application form can be done using the same set of instructions as
mentioned in Sections 10.1 to 10.3 of the English form. When user clicks the link Apply Online
(Bengali) link in the Dashboard page listing “Application for Renewal of MCM Scholarship of
Education Directorate” for the Education Directorate, the following form appears to be filled-in and
submitted:

12 OFFLINE RENEWAL APPLICATION IN ENGLISH
The Offline form is a PDF file which needs to be downloaded from the Offline Form option available
in the dashboard page after selection of the respective department’s service. When user clicks the
link Apply Offline link in the Dashboard page listing “Application for Renewal of MCM Scholarship of
Education Directorate” of Education Directorate, the following form can be obtained, filled-in and
submitted:

i. Use Right Click on the Download link
ii. Choose Save link as..

Requirements to work with Offline Forms
The following software are mandatory at the local machine to work with the Offline Forms:
1. To Open & Edit the downloaded Offline Forms, Acrobat Reader V9 or above is required
2. To Save the XML version of the edited Offline Form, any e-Mail Client is required Edit
downloaded PDF Form
i. Open the downloaded PDF form from the saved location
ii. Fill up all the required Fields. Entering of data in the (*) marked fields is must.
iii. Click Validate button to check whether any field is missed
iv. If error is reported, correct at the required places & Press Validate Button again
v. Click OK button in the validation confirmation window
vi. Button Save As XML appears
vii. Check the Internet Email option & Click OK button
viii. Choose folder to save & Save the XML form (at the time of saving the entered data as
XML, please note the name of the saved form)
ix. Close the PDF Form
Upload saved XML file
1. In the Apply Offline link of the respective service page, click Choose File button.
2. File selection screen appears
3. Select the saved XML file
4. Click the Upload button to show the Offline Form data on the e-Form screen.
5. Follow the instructions on the screen to Save & Submit the filled up form.
