Provisional PIV I ID Badge Pre-Issuance Activities

Provisional PIV ID badge pre-issuance activities include:

1. Sponsor reviews of Applicant’s identity source documents and PIV Enrollment Package.

2. Sponsor approves the issuance of a Provisional PIV ID badge, and forwards Form AD 1197 (copy) to the Issuer.

2) Where do I store Form AD 1197 after completion?

Refer to your agency-specific policy regarding storage, consistent with the Privacy Policy.

3) What happens when an employee or contractor retires, leaves, or changes Federal Departments?

If an employee or contractor terminates employment with USDA, the ID badge must be retained, in accordance with your agency-specific policy. Refer to your agency-specific policy with regard to changes in status.
