Dear SOHN Colleagues, My focus in this issue of the Update is on the AAO-HNS guideline development process – taking place in concert with ENT subspecialty societies that include SOHN. As your representative to the AAO-HNS Guideline Development Taskforce (GDTF), I have experienced the concerted and extraordinary effort required to develop symptom-based guidelines that are useful to practitioners, nurses and the medical community. SOHN nurses have been involved with the five published guidelines generated by the GDTF with a tonsillectomy guideline slated for the November issue of Otolaryngology-Head and Neck Surgery. SOHN nurses were directly involved in the development, peer review of the literature and language used to construct the current guidelines. I am proud of the nurses that gave their time and talents to these efforts! Additional topics under consideration and development include nasal valve compromise, polysomnography for sleep-disordered breathing, CT imaging indications for paranasal sinus disease, sudden hearing loss, tracheostomy care, and perioperative and surgical management of the recurrent laryngeal nerve during thyroid surgery. These are common topics that ORL nurses encounter daily. As I participate and vote in the GDTF meetings, a prime consideration is will it be useful for ORL nurses? Many other topics have been presented for consideration. Topics investigated for development must weather an exacting formal process and are voted on by the GDTF. Careful thought is given to justify the time and effort of the taskforce and AAO-HNS staff that assist development of the guidelines. The GDTF includes physicians, nurses, allied professionals and consumers (patients). Dr. Richard Rosenfeld, the GDTF chair, is a recognized leader in practice guideline development and evidence-based medicine. The process has not been without controversy as the newly adopted clinical practice guideline on hoarseness demonstrates. Collins (2010) noted this in ENTtoday from the Combined Otolaryngology Spring Meetings (COSM) in the article on “Hoarseness Guidelines Continue to Draw Scrutiny”. Many otolaryngologists including voice and laryngology subspecialists argued that the guideline did not reflect common sense and current clinical practice patterns. The AAO-HNS provided a response to the criticism, concerns were communicated and discussion with the GDTF is continuing. This debate demonstrates that Evidence- based Medicine is a melding of clinical expertise, patient preference and the medical literature. Often the guideline development process may elucidate areas for further clinical research. Some changes to the hoarseness guideline are expected based on feedback from these experts. Dear SOHN Colleagues,

Tuesday, September 28 ~ 2:15 p.m.

Partners In Health (PIH) was founded in 1987, two years after the Clinique Bon Sauveur was set up in Cange, Haiti to deliver health care to the residents of the mountainous Central Plateau. The clinic was just the beginning of successful projects designed to meet the health care needs of the residents of the poorest area in Haiti. In the last 20 years PIH has expanded its operations to eight other sites in Haiti and five additional countries.

Currently the clinic sites in Haiti have been testing and treating 10,000 patients each week. The most common diseases and conditions treated include: diarrhea and gastrointestinal infections; malaria; coughs and respiratory infections; headaches; malnutrition and urinary tract infections.

During fiscal year 2009 and early 2010 SOHN board members faced daunting challenges created by economic constraints and increased pressure to do more with less, while still maintaining the Society’s mission. Always striving to find the most creative and innovative means to support its members during tough economic times, SOHN focused its attention on offering new and enhanced resources to its membership:

In early 2010 the Society’s newsletter, the Update was placed online allowing easy access to its members.

Meeting arrangements were streamlined in order to reduce costs. An emphasis continues to be placed on our green initiatives. The Spring Seminar Series held April 29 – May 1 in Las Vegas, Nevada was very successful with interesting topics and outstanding speakers. The Comprehensive ORL Nursing Course traveled to Iowa City, Iowa where the SOHN Heartland Chapter hosted the day long event. As September draws

near, final arrangements for the 34th SOHN Annual Congress and Nursing Symposium are being implemented. An excellent educational program with a wide variety of topics and speakers has been planned, as well as exciting array of social events. Our list of international attendees continues to grow.

SOHN’s financial status remains constant. Fiscal year 2009 ended with the Society in the green. Again, the board has taken strategic steps to insure the Society’s financial integrity. In March SOHN was chosen for a random audit of a non-profit organization by the IRS. The Society passed with flying colors.

SOHN President, Cindy Dawson, represents SOHN on the American Academy of Otolaryngology and Head-Neck Surgery Foundation’s Guideline Development Taskforce Committee. SOHN’s Practice/Research Committee continues to work toward completion of the evidence-based guideline on tracheostomy care. A special session on the 2010 Congress program will be devoted to a review of the progress of this initiative.

Our Membership and Chapter Coordinating Committees are working diligently to meet the needs of our members and chapters. A membership survey was conducted in March/April 2010 to assess benefits of membership and to capture member suggestions.

SOHN’s Education Committee recently completed an additional online continuing education offering: Parotid Masses: Face the Facts. SOHN’s ANCC accreditation renewal application will be submitted this fall. A self-study document will be written to fulfill the reaccreditation requirements.

The joint ONS/SOHN Otolaryngology Telephone Triage book is nearing completion. Cindy Dawson, Margie Hickey and Susan Newton are the editors. The release date will coincide with our annual meeting.

SOHN is secure in its organizational foundation and remains mindful of the dynamic economic climate. As the Society approaches its 35th Anniversary, SOHN looks toward the future for strategies to advance its evidence-based projects and new resources to advance the specialty and education of its members.

During the past year the SOHN Board of Directors have engaged in an active dialogue concerning the expansion of non-RNs into our membership as adjunct SOHN members. Some may ask – why should we do this when we are a traditional nursing group? Clearly this is not intended to dilute our current membership’s role or shift our organization’s focus, goals or fundamental attributes. One of our abiding interests is to establish and expand the fund of nursing knowledge in Otolaryngology. If ORL/ Head and Neck Nurses do not take the leading role then others will set the agendas and write

the additions to our field. I believe all of us have seen the expansion of unlicensed personnel in our offices, clinics and departments. We must help set the competency standards for these workers and an adjunct role in our organization may be one way to contribute. Healthcare is changing and SOHN can help direct and educate new personnel with our organization benefiting as well.

The following have been proposed as revisions to the SOHN National Bylaws and will be presented to the membership for approval through voting procedure at the business meeting of the SOHN Annual Congress, Tuesday, September 28, 2010.

Rationale: In the ever changing health care system an increasing number of allied health care professionals are providing the direct patient care to our clients. In an effort to provide educational and networking opportunities, we propose to offer an associate membership in SOHN to these individuals.

Current sections with proposed changes:

Article II BTo unite licensed (and non-licensed) professional

Otorhinolaryngology and Head-Neck (Nurses caregivers) for the purpose of maintaining a Society dedicated to the constant endeavor of promoting the highest professional standards of Otorhinolaryngology and Head-Neck Nursing for the better best and safer safest care of the our patients.

Article III Section 2 AA registered professional nurse or a, licensed practical or

vocational nurse, (or an allied health caregiver) with an interest in Otorhinolaryngology and Head-Neck Nursing living in an area where a chapter exists.

Texas:Texas Board of Medicine Files

Complaint against Physician involved in the Winkler County Nurses Case. The Texas Medical Board (TMB) has filed a formal complaint against the physician in the case involving the two Winkler County nurses, who were criminally charged for reporting him to TMB. The complaint’s last charge alleges nine practice violations and one charge of witness intimidation. The Texas Nurses Association is following this case closely and has provided support to the two nurses involved. The federal civil suit Vicki Galle and Anne Mitchell filed against the hospital and others for retaliation is still scheduled for trial in November. The court had ordered a second mediation that occurred in May, which was unsuccessful.

FNA successfully lobbied to amend this Bill to eliminate the requirement that school nurses train other school employees on the care and treatment of diabetic children. School nurses will work with the schools to accommodate the needs of these children and provide a safe environment, however, school nurses cannot be required to fully train employees to provide care for these children who have complicated health care issues. The bill which has been presented to the Governor simply prohibits school districts from requiring students to attend specific schools based on their diagnosis. Also, children who are independent with their care may carry their own supplies.

New Regulation by Board of Nursing-HB 1337/ SB 2530:

FNA successfully lobbied to include the proper definition of nurse accrediting agencies so only NLN-AC and CCNE are acceptable. If a program is already accredited by NLN or CCNE it will essentially be exempt from the Board of Nursing’s jurisdiction. This bill has been signed into law by the Governor.

Council Announces 2011 Guest Board Attendee Program

The SOHN Council of Past Presidents (COPP) is currently accepting applications for the Guest Board Attendee Program. This successful leadership mentoring project was initiated in 2005. To date 10 members have attended the Midwinter Board Meeting held each January. The latest attendees were Deena Hollingsworth and Ashley Laursen. Partial funding of this innovative project is provided by the COPP in order to promote the development of new leaders

in SOHN. Funding of the Guest Board Attendee Program now replaces the COPP Congress Grant. The Council was formed in 1985 and has continually supported SOHN special projects. Applications can be obtained from SOHN National Headquarters or from COPP members and board members during the annual congress.
