SPAIDEN – Identification FormPurpose To view student’s names, addresses and phone numbers.

To Access: Type the form name SPAIDEN in the Search box under Welcome and press Enter.

Or Type the form name SPAIDEN in the search box after clicking on the icon

ID: Type the Student’s ID number in the ID field, if you know it. The name will pull into theName Description Field.

To Search for ID within ID search box:

Name: Click on the icon next to the ID search box. The Option List is displayed.

Click on the Person Search link and select one of the options in the Add Another Field. (ie; Type the last name in the search box)

You can change the connector for your search

You can add multiple search fields to search on the name; such as Last Name, First Name, Middle Name, Change Indicator or Name Type.

Note: Enter desired query criteria, using wildcards (%) if necessary.

Case Insensitive Query radio button will default allowing your query to not be case sensitive. Press F8 or green Go button to execute your query. For additional queries, press the Clear All button to clear records, enter new information and press F8 or Go to execute.

If multiple records appear, highlight the row of the record you want and click the Select button or Cancel on the lower right hand side to start

over.

If a student’s name or ID number has changed, the Alternate Identification tab provides a history of name and ID changes. The ID and names are retained in the system so their record can be located various ways.

If multiple changes have occurred, the record indicator on the right will show the total record count. You can navigate through each record by clicking on the arrows.

The Address tab will show all addresses tied to a student, including addresses that are inactive.

To view all of the student’s addresses, click the arrows to navigate through each record.

To insert a new record, copy a record or delete a record, click on the icon on the top right hand side under the Start Over button.

The Telephone tab will show all telephone tied to a student, including telephone that are inactive.

The E-mail tab will show e-mail addresses tied to a student, including e-mail addresses that are inactive.

The Emergency Contact tab will show the emergency contact tied to a student.

The Additional Identification tab will show if the person has an alternate identification.

SGASTDN - General Student FormPurpose: To view information on a student’s term status, major, concentration, advisors and other information. A new record will be created anytime there is a change in major or status.

To Access: Type the form name SGASTDN in the Search box under Welcome and press Enter.

ID: Type the Student’s ID number in the ID field, if you know it. (Refer to previous search options described in SPAIDEN instructions.)

Optional: Click on the Ellipsis icon denoting more options of the ID box to query on student Names and Alternate IDs.

Summary: Click on to view summary of terms student has attended.

SFAREGQ - Student Class Schedule FormPurpose: To view the courses a student is enrolled in (by term), including times, dates, credit hours, locations and instructors for each course. Course schedules are kept in the Banner System for the current semester, the upcoming semester and the past couple of semesters. This form was modified by UofM to show the PAID indicator box.

To Access: Type the form name SFAREGQ in the Search box under Welcome and press Enter.

To Query within the form, click on the F7 key, the option to Add Another Field will display.

Use the Scroll bar to scroll to the right to see additional information about

each course. It will default to 10 records per page. If there are more records than 10, you can change the

number per page or click the arrows to view.

SHATERM - Academic History FormPurpose: To view a student’s complete academic history one semester at a time. A summary of credits attempted/earned and GPA will also show for each term, as well as course detail information.

To Access: Type the form name SHATERM in the Search box under Welcome and press Enter.

To Use this Form:ID: Type the Student’s ID number in the ID field, if you know it. (Refer to previous search options described in SPAIDEN instructions.)

Level: Enter Level Code for student (required). 01=Undergrad, 02=Grad, 03 = Law School

Term: Leave blank.

The SHATERM form consists of two tabs: (1) Current Academic Standing (2) Term GPA and Course Detail Information.

(1) Current Academic Standing tab:This block shows the First and Last Terms attended. In addition, it shows the student’s Overall GPA.

**Important! At the UofM, we only use Overall UofM Institution GPA! Overall Transcript GPA is for informational purposes only.

(2) Term GPA and Course Detail Information tab:The academic history displayed begins with the first term that the student attended. Transfer courses will be included.

The top section of the block will list the Term GPA. The lower section will list the Courses/Details associated with that particular term.

SFASLST - Class Roster FormPurpose: To view all the students registered for a specific course.

To Access: Type the form name SFASLST in the Search box under Welcome and press Enter.

To Use this Form:Term: Enter the term code (required field).

Course Reference Number: Enter course number (required field). If not known, use the for the CRN.

If the Course Reference Number is not known, use the for the CRN. You will navigate to an Options List. Choose Section Query from the Option List. You will navigate to the Schedule Section Query form (SSASECQ).

For additional queries, press

to clear records, press to execute.

Example: Always have a term entered before you do a query.

Note: The Sequence Number (Seq) does not relate to the total number of students in the class. It is related to the order in which a student registered during the registration process. Many students will register, than cancel classes as they juggle their schedules around. Example: Sequence 107 shows that student at the time of registration was the 107th student to register for this class. However, the total number of students registered for this class = 43 (Record Count = 43). This means there were many cancellations during the registration process.

To Query the Class Roster, click under the Start Over button.

To view other options associated with SFASLST:

Click on the Related tab for additional information regarding either the Course Section or specific Student information (you will link to other Banner Student Forms).

Important: If you have linked to a new form, to return to SFASLST, make sure you click on the X

button in the top left next to the form name.

1. Click on the Tools menu on the upper right hand side.

2. Banner will generate a .csv file. A message box will indicate where the file has been saved on the lower left

3. Click on the on lower right.

4. A new tab will open with your .csv file. You can then format/sort/print the roster from Excel.

SPAAPIN- Advisor PIN NumberPurpose: To view the advising PIN number that is assigned to students each semester. The PIN number is required in order for all undergraduate students to register via CyberBear on the web.

Important: This number is unique each semester!

To Access: Type the form name SPAAPIN in the Search box under Welcome and press Enter.

To Use this Form:

From Term: Enter the term code (required).

Appendix ASPAIDEN Student Identification Form ** Lists addresses, phone numbers and previous names.

SGASTDN General Student Information Form ** Displays information on a student’s status, class standing, major, level (undergrad or graduate), advisors and other information. A new record will be created anytime there is a change in major or status.

SFAREGQ Student Class Schedule Form ** Courses that a student is enrolled in (by term) including when and where they meet.

SHATERM Student Academic History Form (displays GPA information) ** Displays GPA data, total credits, academic standing, academic history, and institution/transfer course detail information. (Leave TERM blank for complete history).

SHACRSE Student Course Summary Form (does not include GPA data) Displays registered courses, sections, grades, etc. for a selected term; does not include GPA data. (Leave TERM blank for all terms).

SFASLST Class Roster Form Allows the user to display the roster for a particular class based on the CRN.

SPAAPIN Student Advising Number Form Shows a student’s advising number.

SHATRNS Student Transfer Course Form Provides detailed information on transfer courses accepted at UM.

SOATEST Student Test Form Shows GRE and other test scores.

SHASUBJ Student Academic History Form by Subject Shows GPAs, academic standing, and allows review of all courses taken categorized by subject (Geography, Sociology, etc.).

SHADEGR Student Degrees and Other Formal Awards Form Displays completed and conferred degrees and other awards that students have received (enter sequence “1”)

SSASECT Class Information Form Shows number of seats available in a class, when it meets, who is teaching it, and number of credits.

SSASECQ Class Enrollment Statistics Form Shows scheduled classes, enrollment statistics and rooms.

SIAASGQ Faculty Schedule Query Form Displays courses to which a faculty member has been assigned (term by term). ** Useful to Advisors.

Appendix BGUAPMNU Personal Menu Maintenance FormPurpose: To develop your own Personal Menu tailored specifically to your needs.

To Access: Type the form name GUAPMNU in the Search box under Welcome and press Enter.

To Use this Form (Basic Steps):1. Place your cursor in the Right Pane under the Object Column.2. Type in the form name you want to include in your personal menu. Use your Down

Arrow key to enter the next form name. The Description Name for the form you just typed will automatically pull in as you navigate.
