Clinic News Clinic Volume

Despite a January hampered by extreme weather, patient volume for University Village Center is still above last year’s levels by 5%. New faculty and additional resident clinics due to the residency expansion have contributed to increased capacity. Expanded hours have also helped improve access to appointments. Thanks to Brian Dudkiewicz (top left) and Sandy Pfantz (top right) for the Friday 7 AM appointments and to Ariel Leifer (bottom left) and Matthew Johnson (bottom right) for the 9 PM closing on Tuesdays and Thursdays. Thanks also to all the staff who adjust their hours to provide this access for patients. These appointments are very popular, and are a great opportunity for patients to connect regularly with their personal physician/APN.

Measuring Our Progress We will soon be inundated by a tidal wave of data measuring our perfor-mance in all aspects of the clinical enterprise. Specifically:

1. The number of patient satisfaction surveys has increased by over 350% in the last month. Before this fiscal year ends we will have reliable satisfaction data down to the individual clinician level.

2. Our clinical quality data is in a data warehouse, and the process is in motion to be able to tap into it in a reliable and understandable fashion.

3. Our financial data is now also available down to the individual level with greatly improved accuracy and timeliness.

Collating, interpreting, and presenting all this data in ways that allow us to improve our performance will be the challenge of the coming year.

Quality Improvement Initiatives Second-year residents, faculty, and clinic staff have been meeting in teams this year to develop 4 quality improvement projects in our clinics. These projects are entering the implementation phase:

1. Improving timeliness of follow-up of patients discharged from the hospital.

2. Improving follow-up of abnormal pap smears to ensure appropri-ate testing is completed.

3. Improving patient communication through development of a clinic Facebook page.

4. Improving our sensitivity to the needs of LGBT patients. We look forward to hearing reports on these initiatives at the June Schol-arship Day.

Welcome to the Department Cathy Albecker (right, top) and Kimberley Andrews (right, bottom), the new nurse care coordinators. “A nurse care coordinator is a member of the Care Coordination Team, which is a collaborative ef-fort between the Medical Director, Pharm D’s, Clinical Health Coach and the nurse care coordinators. Our goals are to: improve health outcomes for populations with chronic diseases; increase the number of individ-uals receiving preventive health interventions and screenings; and reduce high cost service utilization (ED, unplanned hospitalizations and re-

hospitalizations). Our strategies will include: continued use of Pharm D expertise and outreach to supplement chronic disease management; use of datasets to identify pa-tients who are missing chronic disease measures; outreach to outliers to schedule ap-pointments; use of patient visits and chart reviews to identify measures or screenings that are due or out of range; write orders for with notification to the MD or NP; use of the Discharge Project care transition process that Drs. Rotert and Hall initiated with the inpatient team to promote and schedule outpatient follow up; utilization of com-plex care management strategies and protocols with eligible patients and in collabora-tion with the patient and physician; and use of telephone and in-person health coach-ing conversations. With the goals in mind and the strategies agreed upon, we are looking forward to increasing our pace and making an impact on the health measures and outcomes of the diverse patients populations at U of I Family Medicine Center. You will be seeing more signs of us, including Clinic Notes in Powerchart! We look forward to working with physicians and nurse practitioners.”

The project for the IOWA Chronic Care Consortium was titled “System En-hancing for Health Outcome Improvement”. The purpose of the project was to create processes and structures that will fa-cilitate population health management. The focus of the project is to reduce the barriers of effective healthcare utilization and enhance the use of internal resources. Focus will be using available systems (i.e. nursing appointments, to recheck elevated blood pressure measurements). The processes and structure will be expanded to identify and work with patients who need additional and on-going care team management and evaluation.

ALSO | February 14-15, 2014 This past February, our department sponsored the AAFP Advanced Life Sup-port in Obstetrics (ALSO) Provider Course for the 7th consecutive year. 47 residents (including our 8 interns), midwives, and family physicians from all over the Midwest region attended the two-day course to learn how to better manage obstetrical emergencies within their own practice. Based on the participants’ reviews, Dr. Evelyn Figueroa co-directed a very successful course this year with the help of Drs. Keia Hobbs, Ariel Leifer, Cari Benbasset-Miller, Janee Ware and other 10 instructors from FM resi-dency programs throughout Chicago. “[I] definitely will feel more comfortable on L&D”, shared one par-ticipant after completing the course, “and have an organized approach to emergencies.” When asked what they liked most about the course, one participant answered, “great leadership, fun, interesting, [and] interac-tive”.

We had a very successful “First Annual DFM Spring Celebration/Appreciation Dinner". Over 35 people enjoyed them-selves at The Rosebud restaurant on Friday April 11th. Those who attended included a number of UIC M4 students who matched in family medicine residency programs, many community faculty, our own faculty, and many of those groups' significant others. Patient-centered Care: Moving the Medical Home Forward

The Department of Family Medicine continues its efforts in advancing patient-centered care and bridging the gap between medical education and practice delivery. The Patient-centered Medicine (PCM) Scholars Program car-ried out another active year filled with a variety of learning experiences. In addition to orientation sessions for the different program components and students’ ongoing direct work with patients, highlights of the year included the Lunch & Learn discussion seminars, symposia led by experts discussing hot topics in the era of health care reform, and the end-of-year Service Learning Showcase. Our M1 Lunch & Learn series commenced in November, with Dr. Richard Stringham presenting “The Patient-Doctor Inter-action: The Patient-centered Medicine Perspective”; in Janu-ary, Dr. Mark Potter presented “PCM and EBM, Where From Here?”; and the series concluded in February with Dr. Samuel Grief and Dr. John Hickner presenting “Healthcare Coverage and Costs” and “Patient-centered Care Coordination” re-spectively. Each Lunch & Learn focused on engaging students in critically ana-lyzing real-life patient care situations through the lens of patient-centered care,

and furthering their under-standing of integrating theory and applied practice of the PCM. The M2-M4 Service Learning Program (SLP) held a sympo-sium in November, titled “Changing Healthcare Policy: Special Considerations for Vulnerable Populations”. Pan-

el speakers included Bryan Becker, MD., Associate Vice President for Opera-tions, University of Illinois Hospital and Health Sciences System; David Ernesto Munar, President and CEO, AIDS Foundation of Chicago; Gita Rampersad, JD, MHA, Director of Communications, Blue Island Community Health Coalition; and PCM M1 Scholar Samuel Wainwright, UIC College of Medicine. After seed talks by the four presenters, participants engaged in a moderated town-hall style discussion that sparked a lively debate on best approaches to meet patients’ needs in our challenging health care system. In January, Dr. Bechara Choucair, Commissioner, Chicago De-partment of Public Health, presented “Healthy Chicago: Ap-proach to Health Disparities”. His insightful presentation led to a lively discussion surrounding the importance of understanding the root causes of health disparities and the role medical students can play now and in the future in reducing them.

(Cont’d) In March, we held the 9th Annual Service Learning Showcase - Reflections and Lessons from Campus-Community Engagement to Reduce Health Disparities. Dr. John Hickner and Dr. Memoona Hasnain provided a warm welcome and introductory remarks. Dr. Hasnain presented a summary of program history, current focus and future direction. She highlighted the emphasis on integrating an interprofessional training element in the M4 component of the PCM Scholars Pro-gram, utilizing community-based participatory research (CBPR) and quality improvement. M2 and M4 Scholars from each of the five concentrations, Domestic Violence, Geriatrics, HIV/AIDS, Homelessness and Immigrant & Refugee Health, shared their experiences and reflections. As in prior years, students from all five concentrations presented outstanding work. Faculty concentration leaders and community partners were also present and shared their thoughts. Congratulations to all the Scholars for carrying out meaningful work with individual patients as well as community-engaged participatory advocacy, action and scholarship that positively influences the lives of vul-nerable and underserved individuals and builds healthier communities. Special congratulations to Class of 2014 PCM Program Scholars graduating this spring.

Health Track

The Department of Family Medicine at UIC continues to grow our innovative and impactful Global Community Health track which aims to improve health care delivery within low- and middle income country community health care systems as well as provide training in global health and cultural competency for U.S. health care trainees. To date, six UIC Family Medicine alumni have graduated with additional training and experience in Global Health, including on-site experi-ence working with local partners on cur-rent projects. An additional eight current residents are involved in the program, which has enriched the residency’s re-cruitment efforts and training with a rec-

ognized Global Health curriculum. Our program is a leading innovation on campus. Several departments (Medicine and Med/Peds with others interested) have developed full global health tracks based on our program. To facilitate this, Andrew Dykens has recently been appointed as the Director of Postgraduate Global Health Education through our Center for Global Health.

Senegal Project Summary 2010 – 2014 This partnership between the health region of Kedougou, Senegal; Peace Corps, Senegal; and the University of Illinois at Chicago (UIC) Departments of Family Medicine and Obstetrics / Gynecology has a primary focus of cervical cancer preventive services implementation. This was determined by the assessment of local health concerns, the status of the health service de-livery infrastructure, and the priorities of the local health care leadership. The Health Region of Kedougou is located in the South-Eastern part of Senegal with an estimated population of 143,000 inhabitants and comprises twenty-three health posts, three health centers, and the re-gional hospital. This project to date has implemented regional-level cervical cancer prevention guidelines, a service accessible to an estimated 10,000 women in the target population, trained 63 health workers in visual inspection of the cervix with acetic acid, and trained 1 health work-er in cryotherapy as a treatment modality for precancers. We have just completed a region-wide prevalence study with data indicating 2.27% prevalence of cervical dysplasia. Identified next steps are formal policy development through a community participatory approach, the horizontal scaling of this policy and health service to the neighboring region of Sédhiou, and the implementation of a quality improvement process for the clinical services.

Dominican Republic Project Summary 2012 – 2014 The Peace Care Dominican Republic (pcDR) partnership began in 2012 and is a partnership among the Guaymate, Dominican Republic, Peace Corps Dominican Republic, and faculty and trainees from University of Illinois-Chicago (UIC) (Family Medicine, Medicine, and Med/Peds) and Northwestern University (NU). A community-driven assessment of the local health systems and health services needs identified a gap in health services quality at Guaymate Mu-nicipal Hospital and a desire to improve health education of the Guaymate community. Peace Corps-Dominican Republic recommended youth sector volunteer Rosa Lopez has been the principal Peace Corps Volunteer collaborating on this project. Rosa and her replacement, Dyl-an, serve in the municipality of Guaymate in eastern Dominican Republic, a community in great need of health resources and organization.

(cont’d) The community assessment phase began in September 2013, when members of the Community Advisory Board (CAB) conducted focus groups discussions. We conducted a site visit in September 2013 to conduct participatory stra-tegic planning. In February 2014, our team returned. Eight members from UIC and NU participated in this site visit, which began the implementation phase of the partnership. The team introduced EngenderHealth’s Client-Oriented Pro-vider-Efficient (COPE) quality improvement tool to the staff at Guaymate Hospital, which has been used in over 50 LMICs since the 1980s. With external facilitation by a local health educator experienced in using COPE, hospital staff completed self-assessments and client interviews to help arrive at an action plan to improve health services. A COPE Committee was established, and concrete steps were discussed to implement in the near future. Our team also partici-pated in knowledge sharing with health care providers from Guaymate Hospital and Clinica de Familia La Romana. The site visit concluded with a community-based health promotion fair, with leadership provided by the Community Advisory Board and youth group volunteers
