Staff Trainingo One overall program for all staff
o Separate sections for each area of staffing (Nurses, Psychologists, etc.)

Client Satisfactiono Surveys will be sent out and/or given to clients or care takers
o Incentive to have surveys filled out and returned – raffle, gift card, etc.
o Short, yet direct survey
o Surveys will be compiled and answers will be entered into database
o Database will spit out Pareto chart – weak points, strong points, reoccurring weak areas

o Who client directly deals with as for staff

Case Recordso Opening page – search for specific client record or open list of all
oInput case information – program would specify what percentage is complete
o If client hasn’t attended UCP in 3+ years, records go to holding Records would delete after 30 days, freeing up space Client would have to fill out new paperwork if they return
o Rank clients based upon frequency of visits Clients who visit most often – top of list, most accessible Clients who visit the least – bottom of list
o Status will notify staff how complete records are

o Check biweekly o Information checked by number of persons involved, redundancy checks for accuracyo Program able to report reoccurring incidents & staff/clients involved with multiple incidentso Rating scale of incidence seriousness – code red and code yellowo Emails sent out to staff or clients who are at risk
