5. Submit this form with the role, audience, format, 3 topics, and learn verb circled.
6. Submit a PRINTED version of your remediation activity and this form by the due date.

Requirements
To be considered for extra points, remediation activities must meet the following requirements.
1. Typed
2. Include a bibliography if outside sources are used
3. Minimum of 2 pages of actual writing (not including heading), double-spaced, 12 point font for letters, substitute plans, and newspaper articles
4. All formats include detailed information about the three topics chosen (even the substitute teacher plans, top ten list, and lyrics) based on the rubric

5. Printed and turned in with this document (with choices circled) by the due date
